Periapical Healing After Direct Pulp Capping With Calcium-enriched Mixture Cement: A Case Report

SUMMARY This article describes a successful direct pulp capping of a mature symptomatic mandibular second molar in a 14-year-old girl. The patient was referred with sensitivity to cold beverages and pain on chewing on the second left mandibular molar. Clinical examinations revealed extensive coronal caries and sensitivity to percussion. Radiographically, the tooth was mature and had a widened apical periodontal ligament (PDL) and a narrow periapical lesion. The concluding diagnosis was symptomatic irreversible pulpitis with symptomatic apical periodontitis. Treatment included caries removal under rubber dam isolation, capping of exposure sites with calcium-enriched mixture (CEM) cement, and permanent coronal restoration. At three-, 10-, and 15-month follow-up, the tooth was functional, had normal response to cold test, and did not have sensitivity to percussion. The PDL space regained its normal width, and the periapical lesion healed.

The aim of this case report is to present an innovative combined orthodontic-surgical technique to disimpact mandibular second molar (MM2) using an orthodontic miniscrew and an elastic chain. The impact on the Oral health-related quality of life (OHRQoL) was also evaluated. Using the present techinique, it is possible to expose the impacted tooth, insert a self-drilling miniscrew in the retromolar area, and remove the bud of third mandibular molar. At the same time the orthodontic force is applied with the use of an elastomeric chain that connects the head of miniscrew and vestibular and oral buttons bonded on MM2. A close traction is performed for the whole treatment time without the reactivation of the elastic force. The use of skeletal anchorage allowed the disimpaction of impacted MM2 in a short treatment time (about three months) avoiding the typical biomechanical side effects of traditional orthodontic appliance and increasing the effectiveness of the treatment. Further studies are necessary to evaluate the real advantages and disadvantages of this combined orthodontic-surgical approach.

The C-shape configuration in molars it’s an anatomical variation that difficult the diagnosis and treatment. The aim of this study was to report a case of C-shape endodontic configuration in mandibular second molar. The radiographic examination of one patient revealed the extent of caries in the mesial aspect of mandibular second molar, without the presence of periapical lesion and was clinically noted the C-shape configuration of the root canal, extending from the mesiobuccal to the distal canal. Endodontic therapy was performed and after the root canal obturation with gutta-percha cones and endodontic cement the tooth was restored. We conclude that the anatomical condition in C-shape, although it brings many difficulties for the endodontic treatment, does not preclude the tooth rehabilitation.

The purpose of this paper was to report the five-year success of Biodentine™ partial pulpotomy in a young permanent molar, with signs and symptoms indicative of irreversible pulpitis and periapical lesion, in a nine-year-old girl. Preoperative clinical examination revealed a large carious lesion of the left mandibular permanent first molar. The patient reported pain on percussion. The tooth responded positively to the electric pulp test and had lingering pain after cold testing. A periapical radiograph showed a deep carious lesion and periapical lesion. Based on the clinical and radiographical examination, the tooth had signs and symptoms indicative of irreversible pulpitis and periapical lesion. During caries removal, pulp exposure occurred, and 2-3 mm in depth of pulp tissue at the exposure site was removed. Haemorrhage was controlled within four minutes with 2.5% sodium hypochlorite-moistened cotton pellets. Biodentine™ was then applied as both a pulp dressing and a temporary restoration. At the following visit, composite resin was placed over the Biodentine™ as a final restoration. During a five-year follow-up, the tooth was asymptomatic, had positive responses to sensibility tests, and had no discolouration. Follow-up radiographs showed a dentine bridge and periapical healing.

ABSTRACT Objectives: To investigate the retromolar space available for molar distalization in patients with mandibular prognathism. Materials and Methods: Using cone-beam computed tomography, the posterior mandibular dimensions in 110 consecutive patients with Class I or Class III malocclusion were measured (mean age, 27.0 ± 7.1 years). The shortest linear distances from the distal root of the right mandibular second molar to the inner border of the mandibular cortex were measured at the level of root furcation and 2, 4, and 6 mm apical to the furcation along the sagittal line and the posterior line of occlusion. The retromolar distances were compared between the Class I and Class III malocclusion groups using general linear mixed models. Results: The retromolar space measured through the sagittal line showed no significant intergroup difference. Among the distances measured through the posterior line of occlusion, the space measured at depths 0 and 2 mm to the furcation were significantly greater in the Class III group than in the Class I group. Conclusions: Patients with Class III malocclusion have greater retromolar space for mandibular molar distalization along the posterior line of occlusion only at the level of the second molar furcation.
