Introduction. One of the mechanisms of pathologic oxidative stress in neonates is intensification of membrane lipid peroxide oxidation. Malonic aldehyde (MA) is a secondary product used as an indicator of lipid peroxidation processes and therefore a marker of cellular membrane damage. Meanwhile, nowadays the mechanisms determining organ peculiarities of lipid and protein peroxidation as well as resistance to ischemic kidney damage are not studied completely and require further development.   Objective: to determine diagnostic value of MA in urine as a marker of renal dysfunction in full-term neonates with perinatal pathology of various degrees of severity.   Material and methods. One-centered cohort prospective study was carried out including 41 full-term children with disorders of general condition of a moderate degree of severity (І group); 36 full-term children with disorders of general condition of a severe degree without acute kidney damage (ІІА group); 30 full-term children with disorders of general condition of a severe degree with acute kidney damage (ІІB group) and 40 healthy children (ІІІ group). МА level in urine was determined by means of reaction with thiobarbituric acid at the end of the 3rd day of life. Results. The lack of a negative test diagnostic value with determination of MA in urine was determined when renal dysfunction had been found in full-term neonates with signs of perinatal pathology of a moderate severity. It was evidenced by AUROC 0,53 (95% CІ 0,50; 0,65, р>0,05) with a threshold value of the parameter ≤9,57 mcmol/L. A perfect discriminating ability to determine MA level in urine was demonstrated in diagnostics of disorders of the functional kidney state in children who had signs of severe postnatal adaptation disorders. It was evidenced by AUROC 0,81 (95% CІ 0,71; 0,91, р<0,001) with a threshold value of the parameter ≥ 9,58 mcmol/L; specificity  97,6% (95% CІ 87,1%; 99,9%), prognostic value of a positive result 95,7% (95% CІ 75,7%; 99,4%) and likelihood ratio of a positive result 25,1 (95% CІ 3,55; 76,7). Diagnostics of acute kidney injury in critically sick term neonates found a proper quality of the diagnostic pattern with determination of MA level in urine.  It was evidenced by AUROC 0,80 (95% CІ 0,66; 0,89, р<0,05) with a threshold value of the parameter ≥12,9 mcmol/L; specificity 91,4% (95% CІ 76,9%; 98,2%), %), prognostic value of a positive result 85,7% (95% CІ 66,2; 94,9%) and likelihood ratio of a positive result 7,0 (95% CІ 2,28; 21,5). Conclusions. Considering the value of reactions of pathologic oxidative stress in the formation of maladjustment syndromes, determination of MA in urine is suggested to be used as one of the possible markers of renal dysfunction in neonates which reflects the state of lipid peroxidation processes in the kidney structures and degree of their damage.

Background: Soluble triggering receptor expressed on myeloid cells-1 (sTREM-1) is regarded as a biological marker of infection. We aimed to evaluate the diagnostic value of endotracheal tube (ETT)-sTREM-1 concentration in neonatal ventilator-associated pneumonia (NVAP), to explore the difference of (ETT)-sTREM-1 between preterm and full-term, and to investigate the influence of extrapulmonary infection on (ETT)-sTREM-1 concentration.Methods: In this multicenter, controlled clinical trial of 60 preterm and 33 full-term neonates on mechanical ventilators, we measured concentrations of ETT-aspirate and serum sTREM-1, serum C-reactive protein, and serum procalcitonin, as well as white blood cell count. We initially divided cases into eight groups, based on three categories: preterm of full-term; NVAP or non-NVAP; and extrapulmonary infection present or absent. Groups were compared, and logistic regression analysis and receiver operating characteristic (ROC) analysis was performed to determine diagnostic value.Results: The mean gestational age (± standard deviation) of preterm and full-term neonates was 28.9 ± 2.2 weeks and 39.5 ± 1.7 weeks, respectively, and 32/60 were male. The ETT-aspirate sTREM-1 concentration was higher in NVAP cases than in non-NVAP cases, irrespective of extrapulmonary infection. ROC analysis revealed that ETT-aspirate sTREM-1 concentration had an area under the curve (AUC) of 0.986 and a cutoff value of 228.0 pg/ml (sensitivity, 94.3%; specificity, 96%) in preterm neonates; the same values in full-term neonates were 0.938 and 245.5 pg/ml (sensitivity, 100%; specificity, 93.7%), respectively. The optimal combination of indicators was ETT-aspirate sTREM-1 and serum C-reactive protein concentration. All indicators were present at lower levels on days 8 and 10 of ventilation in neonates who ultimately recovered than in those who did not.Conclusions: ETT-aspirate sTREM-1 and serum C-reactive protein concentrations may be useful for the diagnosis of NVAP.

The pattern of urinary proteins in healthy full-term neonates was examined by sodium dodecyl sulfate polyacrylamide gel electrophoresis (SDS-PAGE), coupled with determination of few parameters related to urinary protein excretion. Twenty healthy full-term neonates were included in the investigation. Five urine samples from each subject were collected on days 3, 7, 14, 21 and 28 after birth. Determination of total proteins was performed using turbidimetric method with sulfosalicylic acid. Urinary creatinine concentration was determined by Jaffe method. Urinary proteins were separated by horizontal gradient SDS-PAGE according to Görg. The highest values for total urinary proteins and for protein/creatinine ratio were detected in urine samples excreted on days 3 and 7 after birth. Three types of SDS-PAG electrophoretic profiles were observed. The first type of electrophoretic profile was characterized by the presence of proteins of mixed glomerular and tubular origin with molecular weights from 10 to 160 kDa. Typical for the second type of electrophoretic profile was the presence of two faint fractions with molecular weights of 78 and 90 kDa and several intensive low molecular weight fractions (14-67 kDa). In the third type of electrophoretic profile only low molecular weight proteins (10-67 kDa) were detected in all five urine samples. These findings express the transitory immaturity of the glomerular filter and tubular protein reabsorbing system of the newborn kidney. Apparently, the tubular protein handling normalizes later than the glomerular filtration of proteins.

SummaryWe studied the diagnostic value of recently introduced ELISA’s for the determination of thrombin-antithrombin III (TAT) complexes, fibrin degradation products (FbDP), fibrinogen degradation products (FgDP) and total degradation products (TDP) for deep venous thrombosis (DVT) in plasma of 239 consecutive outpatients, suspected for DVT by their family doctor. DVT was confirmed by impedance plethysmography in 60 patients. Using the 95th percentile range of 42 healthy volunteers the sensitivity for the detection of DVT was: 37% for TAT, 95% for TDP, 92% for FbDP and 90% for FgDP. Specificity was: 88% for TAT, 16% for TDP, 20% for FbDP and 25% for FgDP.We conclude that these assays are of little value in the diagnosis of DVT in outpatients.

ABSTRACT The 17-oxosteroids were estimated in the urine of 27 patients with Cushing's syndrome by gas-liquid chromatography (G. L. C.). The values of the various steroid fractions are compared with those of normal subjects, patients with thyrotoxicosis and obese subjects. The effect of the age of the patients on the diagnostic value of the invidual 17-oxosteroids and their ratios is discussed.

Abstract Background and aims. Obese individuals have insulin resistance status assessed in the present study by the HOMA index (“Homeostasis model assessment”). This prospective study assessed renal disorders in the insulin resistance in obese patients. Material and Methods. The study included 73 young obese patients. The assessment included the HOMA index before meal and parameters of renal function (glomerular filtration rate, albuminuria, β2-microglobulinuria). Results. In young obese, insulin-resistance patients, glomerular hyperfiltration and β2-microglobulinuria are found in 77.0 and 93.4% of cases respectively. The albuminuria is noted in some cases, which reduces diagnostic value. Conclusions. In young obese patients with insulin resistance, glomerular hyperfiltration and β2-microglobulinuria are main diagnostic markers of renal dysfunction.
