Experience of air medical teams of the Scientific and Practical Center for Emergency Medical Care of the Moscow City Health Department for 2015—2019

Relevance. Due to busy roads of large cities and the resulting slow movement of ambulances with severely affected patients in need of specialized emergency care, new organizational technologies for medical evacuation support as well as modern means of transporting patients to qualified hospitals are required.Intention is to analyze activities of the aviation medical teams of the Scientific and Practical Center for Emergency Medical Care in Moscow in 2015-2019.Methodology. Using the automated information-analytical system “Disaster Medicine of the Moscow City”, activities of aviation medical teams over the last 5 years (2015-2019) were analyzed retrospectively in the Scientific and Practical Center of Emergency Medical Care. Ambulance helicopters are fully equipped to provide comprehensive intensive care for severely affected patients without noticeable deterioration of their condition, and utilization of this equipment was also assessed.Results and Discussion. Main stages of air ambulance formation in Moscow are described. Experience of aviation medical teams over the past 5 years in providing emergency medical care at the pre-hospital stage to those affected in emergency situations and severely ill patients is presented. The description of medical equipment of ambulance helicopters intended for providing emergency medical care to adults and children, including newborns, is given. The number of calls for aviation medical teams over the past five years is provided along with basic reasons and amount of night flights.Most often helicopters were used for transporting patients with severe concomitant and multiple injuries as a result of road accidents. Medical team composition of ambulance helicopters and the number of helipads on the territory of medical organizations of the city and the territories attached to Moscow are given.Conclusion. Based on the analysis of aviation medical teams activities, further ways to improve and develop the aviation medical service of the city of Moscow can be outlined to expand its use.

The activity is analyzed of specialists of the Scientific and practical centre for emergency medical care (CEMP) of Department of health of the city of Moscow (DZM) in the context of the COVID-19 pandemic. The structure of trips of CEMP teams in the fight against COVID-19 from March 1 to July 1, 2020 is presented. It is noted that although all the structures under the jurisdiction of the DZM, starting from March 2020, are working on high alert in connection with the threat of the spread of coronavirus, CEMP specialists continue to work on providing emergency medical care (EMP) and medical evacuation of victims of road accidents and other emergencies, as well as emergency somatic patients with various nosologies.

Relevance. Acute respiratory failure (ARF) and acute cardiopulmonary failure (ACPF) are unavoidable, and often – the leading syndromes in any critical condition, especially in case of multiple organ failure syndrome as a regular stage in the course of previously incurable conditions. Even a full range of intensive care measures in this category of patients does not always give the desired result. According to various authors, with the development of acute respiratory distress syndrome, the mortality rate reaches 80 %.Intention is to determine the role of the aviation medical teams of the emergency medical service-disaster medicine in ensuring the timely availability of high-tech methods for patients who need them, regardless of the patient’s location.Methodology. Activities of the aviation medical teams of the Scientific and Practical Center for Emergency Medical Care of the Moscow City Health Department for the medical evacuation of patients and victims in emergency situations with ARF and ACPF were analyzed.Results and Discussion. There were analyzed interactions of aviation medical teams with the emergency medical response units of the territorial center for disaster medicine and with medical organizations during the medical evacuation of patients with ARF and ACPF. An algorithm and a scheme of interaction between air medical teams are proposed to optimize medical evacuation of these patients.Conclusion. An analysis of the activities of air medical teams revealed their advantages in providing care for patients with ARF and ACPF.

The aim of the study is to study the prevalence, structure and nature of injuries, to identify the frequency of deaths at the stage of medical evacuation, and to determine the number of injured people who need medical care in medical organizations (LMO), according to the data on requests of the population of the Ryazan region and the city of Ryazan for emergency medical care (SMP). Materials and methods of research. Statistical data on the requests of victims with injuries for emergency medical care in the Ryazan region and the city of Ryazan are analyzed. Materials of the study – maps of calls of the SMP teams in Ryazan and statistical data for the Ryazan region for 2017-2019. The results of the study and their analysis. The most frequent causes of injuries, their nature, the frequency of requests of the population of the Ryazan region and the city of Ryazan for emergency medical care, the number of deaths and the frequency of hospitalizations for injuries were determined.

The increase in the number of patients with complications of cardiovascular diseases and the need for early diagnosis of such complications and the early initiation of treatment already at the prehospital stage leads to an increasing  need for more frequent use of helicopter medical equipment, as well as for equipping aviation medical teams with highly qualified specialists. The  formation of a network of vascular invasive centers in the Moscow region,  modern equipment and its use in air ambulance conditions require constant improvement of skills among medical workers. All this undoubtedly leads to positive results, the mortality rate from heart attacks and strokes decreases, the duration of treatment and the period of rehabilitation of patients are  reduced. It should be noted that even 10 years ago, medical helicopters were used in the elimination of medical consequences in road accidents, fires and emergencies. Today, aviation medical teams are involved in more than 50% of cases with cardiovascular diseases, which undoubtedly speaks of the  prospects for the development of medical aviation in large cities.

The results of the research conducted at the All-Russian Centre for Disaster Medicine “Zaschita” are presented. Goal of research: study of organizational issues of the system of emergency medical care for the persons injured in emergency situations, study of participation of territorial centers for disaster medicine in organization and rendering of emergency medical care to patients and persons injured during their work in the ordinary course of their day-to-day activities, study of models of functioning and interaction of emergency medical services in the course of their work in various modes of activity in the subjects of the Russian Federation. In conduct of the research the method of expert assessment by questioning of the experts participating in the organization and provision of emergency medical care in 58 subjects was applied. 242 experts participated in the survey. Analysis of the results of the research confirms the necessity of the further development and introduction into practice of the certain organizational activities aimed at perfection of the system of emergency medical care for the population both in the emergency situation and in the mode of daily activities. Thus, this refers to the construction of a functional vertical of the system of coordination and monitoring of the emergency medical care provision in various modes of activity with interrelated and well-functioning regional and federal segments.

The work of medical workers is associated with constant psycho-emotional stress, which is caused by close contact with human suffering, the need to make immediate decisions, uncomfortable conditions of the pre-hospital stage and cases of aggressive and violent actions by patients or third parties. Statistics show that 54 to 84.8 % of medical workers have become victims of verbal or physical aggression annually. In 2013-2017, 543 crimes against life and health of medical workers on duty were registered in Ukraine. The purpose of the research was to study the causes, nature and risk factors of violent actions against emergency medical personnel with finding the ways to normalize the situation. Material and methods. An anonymous non-personified survey was conducted among 127 workers of the Sumy Regional Centre for Emergency Medical Care and Disaster Medicine. A modified questionnaire “Violence and aggression in the Health Service” (B. Mullan, F. Badger, 2007) was used in the study. It has been established that 74.8 % of emergency medical care and disaster medicine personnel were victims of violence caused by patients, their relatives or friends. Most often, the reasons for aggressive behaviour of the patient or third parties were the time of waiting for a medical worker and the suspicion of incompetence. According to the results of the survey, 35.43 % of employees believe that it is possible to improve the situation by completing and forming ambulance teams of a mixed type. Almost a third of the surveyed medical workers (24.41 %) indicated the need to provide personal protective equipment, another 14.96 % of respondents wanted better legal support and assistance. The authors come to the conclusion that it is necessary to solve the problem of the safety of a medical worker during an emergency call at the state legislative level. Particular attention should be paid to the further improvement of legal assistance, as well as to the development of measures to prevent violence.

The aim: Is to study and analyze the dynamics of the indicators of the emergency medical service of the Ukrainian and the Poltava region in the context of the reforms of the healthcare system in Ukraine. Materials and methods: In this work, the indicators of development of the emergency medical service of the Ukrainian and the Poltava region were studied and analyzed. Conclusions: The provision of emergency medical care in the Poltava region is provided by the Poltava Regional Center for Emergency Medicine and Disaster Medicine. The structure of the center includes 4 emergency medical stations, which are located in cities such as Poltava, Kremenchuk, Lubny, Mirgorod. А modern telemedicine center was built іn 2018 for emergency counseling on-line in new directions was carried out: ultrasound and endoscopic diagnostics, radiology, counseling during surgical interventions, laboratory diagnostics, etc. Emergency medical care reform Poltava Regional should be aimed at increasing the efficiency of the use of resources; provision of the EMC system by the relevant vehicles; qualitative training of doctors in emergency medicine; informatization of the EMC system.

Relevance. The probability of emergency situations in the Barents region determines the need of improvement of forces and means of rendering first and emergency medical care to victims. In September 2017 the large-scale international exercise of rescue services of Russia, Norway, Sweden, and Finland under the code name “Barents-Rescue 2017” was carried out in the Republic of Karelia. The exercise worked oved rendering first-aid and emergency medical care to victims of emergency situations.Intention. To summarize experience in preparation, rendering and result assessment of first-aid and emergency medical care to victims of emergency situations in the framework of the international exercises.Methods. Medical document analysis (hospital records, medical triage records, expert assessment records), statistical analysis of groups using Student’s t-test. The exercise involved 76 victims with injuries of various severity levels, among them 30 were of minor severity, 19 were of medium severity and 27 were of heavy severity. They received first-aid and emergency medical care by 42 rescuers and 67 health workers (doctors, paramedics, nurses) of rescue services of Russia, Norway, Sweden, and Finland. Results assessment was made on the basis of the developed hospital records, medical triage records and expert assessment records and also statistical analysis methods.Result and their analysis. The average expert assessment of a first-aid rendered by rescuers was sufficiently high and was equal to 4.6 ± 0.1 on a 5-score rating system, where 5 points means providing full assistance in accordance with the established standards of its provision. A qualitative medical triage was provided competently, the ordering of evacuation was right, life-threatening damages and injuries were properly defined. At the stage of medical evacuation all the victims were provided with full assistance, there were no divergence of diagnoses, some differences from final diagnose were permissible at the stage of medical evacuation and had no effect on victim’s condition. In addition, a part of the victims in the hospital had medical cards, where was information about the diagnoses and the measures taken. These medical documents were also analyzed in terms of the timeliness and quality of the provision of specialized medical care to injured people in the hospital.Conclusion. The results of expert assessment during the international exercise of rescue services of Barents Rescue 2017 showed that the quality of first-aid, medical triage and emergency medical care during the evacuation and also in the hospital is high enough, but there were some remarks too. Suggestions and comments were addressed to the Exercise organizing committee to improve the engagement during first-aid and medical care to victim of the emergency and they were included in the final report on the exercise.
