Objective: The objective of this study was to assess the pharyngeal airway passage (PAP) dimension changes following functional appliance treatment in Class II malocclusion subjects with retrognathic mandibles. Materials and Methods: Two authors independently searched various electronic databases such as PubMed, Cochrane Database of Systematic Reviews, Cochrane Central Register of Controlled Trials, Scopus, NCBI, and Google Scholar for the available literature within the period from January 1, 2001, to December 31, 2018. On the basis of population, intervention, comparison, and outcome, “changes in PAP dimensions following functional appliance therapy in skeletal Class II malocclusion subjects associated with mandibular retrusion” was considered as search question of the study. After the selection and removal of duplicate articles, assessment for risk of bias was calculated and the data from the included articles were extracted by two authors independently. Results: From six databases and additional hand searching, a total of 5784 articles were extracted. Of 5784 articles, 3754 articles were screened after removal of 2030 duplicates. After going through the title and abstract, 3197 articles were excluded and 40 articles were assessed for full text. From these 40 articles, eight articles fulfilled our inclusion and exclusion criteria for the qualitative synthesis review. Conclusions: The correction of mandibular deficiency by functional appliances has minimum effect on the nasopharynx. Functional appliance treatment has a significant effect on the improvement of the oropharyngeal airway. Changes of hypopharyngeal airway passage need to be studied further among Class II malocclusion subjects with retrognathic mandibles.

ABSTRACTBased on our literature search, we found that the use of miniplate anchorage with Forsus fatigue-resistance device (FRD) has not yet been reported. Therefore, the aim of the present case report was to present the treatment of a patient with skeletal Class II malocclusion with mandibular retrusion using Forsus FRD with miniplate anchorage. Fixed appliances with 0.022-inch slots were attached to the maxillary teeth and after 8 months of the leveling and alignment of the upper arch, 0.019 × 0.025-inch stainless steel archwire was inserted and cinched back. Two weeks after the placement of the miniplates bilaterally at the symphysis of the mandible, Forsus FRD was adjusted to the miniplates with a 35-mm length of rod chosen. Nine months after the skeletal anchored Forsus worn, Class I canine and molar relations were achieved and overjet was eliminated.

Background: Mandibular retrusion is the main cause of Skeletal Class II malocclusion characterized by skeletal retrusion of mandible with skeletal or dentoalveolar protrusion of maxillary. Objectives: This study was conducted to evaluate effect of treatment with twin-block appliances on pharyngeal dimensions. Methodology: This study was conducted in in the Department of Orthodontics & Dentofacial Orthopedics in Kathmandu Medical College. Twenty patients of age group 7 to 14 years with skeletal class II malocclusion with mandibular retrusion were selected. Variables evaluated were depth of nasopharynx; height of nasopharynx; depth of oropharynx; depth of hypopharynx; soft palate length; soft palate thickness and soft palate inclination. The lateral cephalograms were obtained for all subjects before the start of treatment and after a follow-up period of approximately three months in treatment. Results: Results showed that depth of nasopharynx increased by 1 mm, height of nasopharynx increased by 0.78 mm, depth of nasopharynx increased by 1.97 mm, depth of hypopharynx increased by 0.79 mm, soft palate length decreased by 4 mm, soft palate thickness increased by 2 mm and soft palate inclination decreased by 7.01 mm. The results for depth of pharynx and soft palate thickness were statistically significant. Conclusion: This study concluded that the use of twin block appliance for class II patients corrects sagittal dimension of oropharynx and hypopharynx. Early intervention for mandibular retrognathism in class II malocclusion helps enlarge the airway dimensions and decrease potential risk of obstructive sleep apnea syndrome for growing patients in the future.

Class II malocclusion in pubertal phase presents a major and a common challenge to orthodontists. Proper diagnosis and treatment planning in early stage help in preventing and intercepting the severity of malocclusion. In pubertal phase skeletal Class II malocclusion due to mandibular retrusion are best treated with functional appliance. In recent time PowerScope fixed functional appliance is gaining immense popularity as noncompliant Class II corrector. In the present case report an adolescent male patient having Class II division 1 malocclusion with functional jaw retrusion was treated using MBT 0.022” prescription and PowerScope appliance. 7-8 months of PowerScope wear obtained stable and successful results with improvement in facial profile, skeletal jaw relationship and mild increase in IMPA. It can thus be concluded that PowerScope gives good results in Class II correction with a combination of patient comfort and ease of use that was unmatched among other appliances

: The aim of this study was to evaluate the effect of PowerScope (Class II Corrector) on skeletal, dental and oro-pharyngeal airway dimensions in class II malocclusion with retrusive mandible. Twenty patients with age group of 11 to 14 were selected for this study. Experimental group underwent Power scope class II corrector therapy and control group, alignment of only upper arch respectively. Lateral cephalometric radiographs were taken in both experimental group and control group before and after 6 month of treatment. Sixteen measurements in that eight skeletal, five dental and three pharyngeal airway were assessed to know the effect of PowerScope Class II corrector on skeletal, dental and pharyngeal airway PowerScope Class II corrector after treatment showed significant change in SNB (P = 0.01*), ANB (P=0.001*), Inferior airway space (IAS) (P=0.006*), lower incisors position (P=0.0001*) and overjet (P=0.0001*) where as these values were insignificant on comparison with control group except for lower incisors position (P=0.001*) and overjet (P=0.0001*) indicating that PowerScope corrects class II malocclusion mainly by dento alveolar changes not have significant effect on skeletal and oro-pharyngeal airway. Power Scope (Class II corrector) corrects skeletal class II malocclusion mainly by dental changes and has insignificant effect on skeletal and oro-pharyngeal airway.

Functional orthopedic treatment seeks to improve skeletal and dental relationship of the jaws. The challenging task is to correctly position jaws antero-posteriorly and vertically with correct overbite, overjet and Centric relation. The Churro Jumper is an efficient, inexpensive and uncomplicated fixed flexible functional appliance. It is used to evaluate the efficacy of the Churro Jumper appliance in treatment of skeletal Class II malocclusion with retrognathic mandible. Churro Jumper contributes in correction of Class II molar relationship by dento-alveolar effects on both jaws. There was up-righting of maxillary incisors and proclination of mandibular incisors. Churro Jumper is clinically efficient as well as effective appliance to correct skeletal class II malocclusion. The only problem with this appliance is its frequent breakage and oral hygiene maintenance causing inconvenience to both patient as well as operator.
