Neuropathy of dorsal scapular nerve (DSN) following neck dissections or radiotherapy has not been reported so far nor has its treatment in the form of hydro-dissection. Hydro-dissection of nerve under ultrasound guidance has been receiving more attention in the recent past and it is a minimally invasive procedure. We report here a case of neuropathy of DSN following radiotherapy in a patient for whom we could at least provide pain relief as a palliative measure during his past 6 months of life.

PURPOSE The authors describe a minimally invasive procedure for occlusal rehabilitation in a young patient presenting with mild mandibular prognathism and loss of occlusal vertical dimension caused by dental erosion from chronic gastroesophageal reflux.

Transcanal endoscopic ear surgery is a minimally invasive procedure that allows a clear visualization of the middle ear. Recently, indications for endoscopic surgery have been expanding. We performed combined underwater endoscopic and microscopic surgery for external auditory canal cholesteatoma, the computer tomography of which indicates the possibility of cholesteatoma not only in the canal wall but also in the mastoid. The 30° endoscope and underwater technique makes the surgical view clear, and we could remove the cholesteatoma without canalplasty. To the best of our knowledge, no case of external auditory canal cholesteatoma treated with underwater endoscopic and microscopic surgery has been previously reported. This case indicates that the procedure could be a good indication for external auditory canal cholesteatoma.

Abstract Transperitoneal laparoscopic ureterolithotomy is a well-established minimally invasive procedure for management of large impacted ureteric stones since the last 3 decades. We present a case of a 51 years old gentleman, who presented with bilateral large upper ureteric calculi with obstructive uropathy and azotemia, managed successfully with bilateral synchronous 3-port transperitoneal laparoscopic ureterolithotomy, and to our knowledge is the first such case to be reported in literature.

AbstractThe purpose of this paper is to assess the clinical effectiveness of percutaneous laser disc decompression (PLDD) for patients with radicular pain due to intervertebral disc hernia and to describe our experience in this field in Bangladesh.PLDD is a procedure in which herniated intervertebral discs are treated by the reduction of intradiscal pressure by laser energy. This is introduced by a needle inserted into the nucleus pulposus under local anesthesia and fluoroscopic monitoring. Small volume nucleus vaporization results in a sharp fall in intradiscal pressure, with the consequent migration of herniation away from the nerve root. PLDD has been reported to be a safe, effective, minimally invasive treatment option for patients with a herniated disc.This observational random prospective analysis was carried out during the period from November 2007 to December 2011. PLDD was performed on 4622 herniated discs of 2580 patients (1420, male; 1160, female), whose age ranged between 18 and 85 years. A total of 47 cases were failed back surgery syndrome (FBSS). All patients were followed up (ranging from 1 month to 36 months, with a median of 12 months) and were evaluated according to MacNab criteria.Amongst the 2580 patients, 2296 (89.0%) reported immediate pain relief, 230 (8.9%) patients were uncertain and the remaining 54 (2.1%) did not have immediate pain relief. In the subsequent follow up, according to MacNab criteria a good response was seen in 1935 (75.0%) cases and a fair response in 335 (13.0%) cases. The response to the treatment was poor in 310 patients (12.0%). A second session of PLDD was performed on 292 (11.3%) patients made up of 162 patients (6.3%) due to multi-level (>2 discs) prolapse, 102 patients (3.9%) due to an extensive prolapse of one or two discs and 28 patients (1.0%) because of a relapse. In two cases of a prolapsed dorsal spine (TPLDD is an effective and minimally invasive procedure which has almost no side effects or complications, can be performed under local anesthesia, results in no scarring or spinal instability, and still does not hinder the scope of open surgery in failed cases. PLDD is also effective in cases of FBSS.

It is a great challenge to establish aesthetic excellence creating harmony between beauty and the function of the smile with the delicacy and complexity of the lips and components of the face. The use of soft tissue fillers with hyaluronic acid is a worldwide popular and minimally invasive procedure based on numerous studies. Due to its relative simplicity and favorable security profile, it is becoming easy to accept. The present case report aims to describe the lip filling technique, using a total volume of 1 ml of hyaluronic acid, to improve the patient’s aesthetic dissatisfaction. The result was satisfactory, with minimal side effects and optimizing the patient’s self-esteem.

The rhinoplasty is also an aesthetic process, where it depends the correct form to be made. Small nasal defects can be corrected with fillers. Hyaluronic acid is one of the most commonly used fillers lately due to efficacy, predictability, reversibility, local anesthesia, rapid patient recovery, and few postoperative complications. The objective of this study was to present a case report based on rhinomograms as a safe application protocol and its advantages to the surgeon. It was concluded that rhinoplasty with hyaluronic acid is a minimally invasive procedure and few side effects when respecting the limits of each region. Regarding the clinical case, there was an evident improvement in the eformities and nasal contour in the photographic evaluation performed immediately after the reported procedure. The patient informed that she’s pleased with the result.
