Latar Belakang : Hipertensi merupakan salah satu penyebab utama kematian dini di seluruh dunia dan dapat mengakibatkan beberapa masalah seperti jantung koroner, gagal jantung, stroke, ginjal kronis, gagal ginjal, dsb atau bisa dikatakan hipertensi sebagai The Silent Killer, akan tetapi hal itu bukan berarti tidak dapat dicegah atau diterapi, salah satu terapi nonfarmakolgi yang dapat digunakan adalah terapi pijat refleksi kaki. Tujuan dari terapi ini adalah penurunan tekanan darah responden hipertensi sehingga masalah keperawatan yang muncul dapat teratasi. Aplikasi terapi ini melibatkan 3 responden hipertensi di Kecamatan Lowokwaru, Kota Malang. Tindakan keperawatan terkhusus terapi pijat refleksi kaki untuk mengatasi diagnosa keperawatan yang muncul yaitu Risiko Perfusi Serebral Tidak Efektif melalui pemantauan tekanan intrakranial. Metode terapi pijat refleksi kaki yang digunakan adalah pemijatan kaki selama 30 menit dengan panduan yang valid dan teruji sehingga dapat menghasilkan hasil yang maksimal. Hasil intervensi keperawatan menunjukkan terjadi penurunan tekanan darah sistol maupun diastol setiap responden berkisar 10 hingga 20 poin. Hal itu disebabkan karena terapi pijat refleksi kaki dapat menghasilkan relaksasi oleh stimulasi taktil jaringan tubuh menyebabkan respon neuro humoral yang komplek dalam The Hypothalamic-Pituitary Axis (HPA) ke sirkuit melalui jalur sistem saraf hingga pada akhirnya tekanan pada dinding-dinding pembuluh darah dapat berkurang dan aliran darah menjadi lancar sehingga tekanan darah akan menurun. Kesimpulan dari aplikasi ini adalah terapi pijat refleksi kaki efektif digunakan untuk penurunan tekanan darah pada responden hipertensi.

SummaryThe following article reviews nuclear medicine techniques which can be used for assessment of endocrine disorders of the hypothalamic-pituitary axis. For planar and SPECT imaging somatostatin-receptor- and dopamine- D2-receptor-scintigraphy are the most widely distributed techniques. These nuclear medicine techniques may be indicated in selected cases to answer differential diagnostic problems. They can be helpful to search for presence and localization of receptor positive tissue. Furthermore they can detect metastasis in the rare cases of a pituitary carcinoma. Scintigraphy with Gallium-67 is suitable for further diagnostic evaluation in suspected hypophysitis. Other SPECT radiopharmaca do not have relevant clinical significance. F-18-FDG as PET radiopharmacon is not ideal because obvious pituitary adenomas could not be visualized. Other PET radiopharmaca including C-11-methionine, C-11-tyrosine, F-18-fluoroethylspiperone, C-11-methylspiperone, and C-11-raclopride are available in specialized centers only. Overall indications for nuclear medicine in studies for the assessment of endocrine disorders of the hypothalamic-pituitary-axis are rare. Original studies often report only about a small number of patients. According to the authors’ opinion the relevance of nuclear medicine in studies of clinically important endocrinologic fields, e. g. localization of small ACTH-producing pituitary adenomas, tumor localization in ectopic ACTH syndrome, localization of recurrent pituitary tissue, assessment of small incidentalomas, can not be definitely given yet.

Hyperprolactinemia is a common endocrine disorder of hypothalamic-pituitary axis. It affect about 4-17% of women in reproductive age and about 3-10% of patients with polycystic ovaries. Vitamin B6 is an effective prolactin inhibitor that is extremely cheap and safe.it exerts hypothalamic dopaminergic effect which causes a significant reduction in prolactin level. The aim of the study is To evaluate the effectiveness of vitamin B6 in reducing serum prolactin in Hyperprolactinemic patient. Compare this effect to that of cabergoline.

<b><i>Introduction:</i></b> Nasal chondromesenchymal tumors (NCMT) are rare benign neoplasms that usually present in children &#x3c;1 year of age. They can display rapid growth and significant local bony remodeling that can mimic a malignant process. Of the ∼50 published cases to date, few have documented the need for neurosurgical intervention. We herein report a NCMT in an infant treated with a staged cranial and transnasal approach, as well as summarize the available literature on this pathology. <b><i>Case Report:</i></b> A newborn male with a compromised airway was noted to have a large sinonasal lesion. After stabilization, MRI demonstrated a 4-cm enhancing mass with diffuse sinus involvement and significant extension into the anterior cranial fossa, with displacement of the optic apparatus and hypothalamic pituitary axis. After an initial biopsy, the patient underwent a bifrontal craniofacial approach at 2 months of age, followed by a second-stage transnasal endoscopic approach at 15 months which resulted in a complete resection. There were no neurosurgical complications. Pathology was consistent with a NCMT. <b><i>Discussion:</i></b> Although rare, neurosurgical involvement is critical for the treatment of NCMTs with intracranial extension. Staged cranial and endonasal endoscopic approaches may be needed for complete resection of such lesions.
