Fetal distress is an alteration of the fetus maternal exchange with disorder in the metabolism due to complications of high-risk pregnancies that affect the well-being of the fetus, Evaluation by Doppler echography and amniotic fluid index can help us to diagnose fetal distress; the Objective of the research was to analyze the Brain placenta index and amniotic fluid index in high-risk pregnancies as a predictor of low Apgar; We investigated pregnant women who attended the hospital Alfredo G Paulson performing Doppler ultrasounds, the result when evaluating the pathologies of high Risk comparing ALI with Apgar was (p.208) and BPI with Apgar was (p.064) which indicates that there are differences so we came to the conclusion that they do not predict if the fetus at the time of birth would present Apgar bass.

Background: Fetal surveillance even in normal or low risk pregnancy is essential to ensure safe parturition with minimum intervention. Cardiotocography (CTG) and clinical estimation of amniotic fluid volume (AFV) measured as amniotic fluid index (AFI) are two tests that are easily available in the labor room and can be used to identify fetal well-being. Our study aimed to evaluate role of admission cardiotocography (CTG) and amniotic fluid index (AFI) on perinatal outcome in low risk pregnancy at term.Methods: The study was conducted as a prospective observational study. All low risk pregnant women at term admitted to the labor ward in early or established labour between September 2018 and August 2020 were included in the study. They underwent admission CTG and AFI assessment using ultrasonography. All parameters including CTG changes, mode of delivery, AFI, presence of meconium, APGAR score at 1 and 5 mins, need for admission in neonatal ICU and perinatal mortality were recorded. Quantitative data was compared using chi square test.Results: A total of 180 patients were included in the study. Majority of the women belonged to the age group of 30-35 years. Abnormal CTG showing fetal distress was seen in 105 (58.33%) cases. Non-reactive CTG was significantly associated with meconium stained liqour, requirement for LSCS, still birth, fetal distress, APGAR <7 at 1 and 5min and NICU admission (p<0.001). The association of low AFI with non-reactive CTG had statistically significant impact on perinatal outcomes like low birth weight, requirement for LSCS, fetal distress, APGAR <7 at 1 and 5 mins and NICU admissions.Conclusions: Admission CTG is a simple non-invasive test that can serve as a screening tool in low risk obstetric population to detect fetal distress already present or likely to develop and prevent unnecessary delay in intervention. Thus, it may help in preventing fetal morbidity and mortality.

Background: High risk pregnant women have increased risk of maternal and neonatal morbidity and mortality. Antepartum surveillance is important and should be effective in such conditions. Modified biophysical profile is the method of antepartum surveillance which comprises of cardiotocography and amniotic fluid index. Methods: A cross-sectional study was carried out in Paropakar Maternity and Women’s Hospital from February 2019 to January 2020 to determine the effectiveness of modified biophysical profile. Cardiotocography was interpreted as reactive, equivocal and non-reactive. AFI was considered normal if it was 5 to 24 cm. In the study 172 high risk cases at term and not in labor were included. Each case was subjected to cardiotocography then amniotic fluid index was obtained using real time sonography where it was measured from all four quadrants. Modified biophysical results were obtained and then were divided into 2 arms as normal modified biophysical profile and abnormal modified biophysical profile then analysis was done. Results: Of 172 cases, there were 97 (56.4%) cases in normal modified biophysical profile and remaining 75 (43.6%) in abnormal modified biophysical profile group. The rate of cesarean section increased when there was abnormal modified biophysical profile.  Neonatal resuscitation and admission was increased in abnormal modified biophysical profile. Conclusions: Normal modified biophysical profile in high risk pregnancy had more cases of vaginal delivery and less adverse fetal outcome like low APGAR score, neonatal resuscitation and neonatal intensive care admission. Background & Objectives: The measurement of amniotic fluid volume (AFV) has been an important component of antenatal evaluation of fetal well being. The most commonly used methods are by amniotic fluid index technique (AFI) and single deepest pool method (SDVP). Both the methods employ ultrasound for AFV measurement. The objective of this study is to compare the usefulness of AFI and SDVP method in assessing amniotic fluid volume for predicting adverse perinatal outcome.Materials & Methods: This is a hospital based prospective comparative study. The patient whose amniotic fluid was measured by SDVP technique was study group and that by AFI was comparison group. Any case with high risk factor was excluded from the study. First case was decided by lottery and then alternately one case was kept in study group and the other in comparative group. If the women did not deliver within 1 week the measurements was retaken and the final values was used for analysis.AFI was estimated as described by Phelan and colleagues and SDVP as described by Manning. Data collection was started after the approval of Institution review board. This study was conducted at Patan Academy of Health Sciences. Duration of data collection was 3 months, Asadh –Bhadra, 2071.Results: A total of one hundred and fifty four cases which met the inclusion criteria were taken. Incidence of oligohydramnios by SDVP method was 10.4 % by SDVP technique and by AFI method it was 18.2 %. There was no statistical significant difference between the two groups in terms of rate of induction, mode of delivery, meconium staining of liquor, fetal heart rate tracings, APGAR score at 5 mins and admission to special care baby unit.Conclusion: In non-high risk pregnancy AFI technique detects slightly more number of oligohydramnios as compared to SDVP technique without apparently any difference in perinatal outcome.

Abstract Introduction The pregnancy-puerperal cycle is defined as a moment that involves physical, psychological and social changes. There are pregnancies that require specialized care and attention due to the presence of risk factors that may be prior to pregnancy and / or that may be associated with the pregnancy condition itself, which characterize them as high-risk pregnancies. Currently, prenatal care with risk stratification is the main care strategy for pregnant women, once the risk classification is identified, it is possible to promote interventions according to the health needs of each pregnant woman. For this, the risk assessment is carried out at each prenatal consultation, so that, depending on the course of pregnancy, hospitalization becomes necessary. The diagnosis of high-risk pregnancies accompanied by hospitalization has impacts on the woman's life, such as loss of autonomy, a sense of failure and the incidence of greater care and interventions by the health team and the family that can configure practices to control their bodies. Objective This work aims to identify the hospitalization process during high-risk pregnancy as a space for the production of projects, resistance and protagonism through the protocols and regulations that configure the hospital context. Methodology This is an exploratory analysis of a qualitative approach in public health. A reference maternity hospital in high-risk pregnancy located in a teaching hospital in Greater Vitória was chosen. It will be used to define sampling for convenience. The data will be collected through semi-structured interviews and on-site observations by the researcher. The data analysis methodology used will be content analysis. Expected Results It is expected to understand the various practices of protagonism and resistance that permeate the care of hospitalized pregnant women. Key messages This work has an impact on the improvement of the maternal and child care network of the public health system in Brazil. This work allows to evaluate the quality of the assistance provided in the public maternity.
