Breast carcinomas are an uncommon neoplastic condition in men, whichaccounts for only 1% of all breast cancers, and not more than 1% of all malignancies in men.A 55 years old man presented with a ulcerated mass in the left breast with a history of pain, discharge and fever. On examination, there was an ulcerating growth above the left nipple withnumerous maggots. The fine needle aspiration cytology confirmed the ductal cell carcinomawhich was further confirmed by ulcer edge biopsy.

Breast cancer has been considered a female dominated disease. Carcinoma of male breast is a rare disease representing 1% of all breast cancers and less than 1 % of all cancers in men. The mean age at presentation is mainly in sixties. We here present a case of male breast cancer presented at very young age of 29 years, diagnosed on fine needle aspiration which was confirmed later on histopathological examination.

Aim: Breast cancer is the most common neoplasm worldwide. Fine needle aspiration cytology is a minimally invasive, highly sensitive and cost effective investigation for breast lesions. The aim of our study was to evaluate the spectrum of fine needle aspiration cytology findings of breast lesions. Material and Methods: It was a prospective study conducted in the Department of Pathology during a 2 year period from Jun 2016- May 2018. A total of 284 patients with breast lesions were subjected to ultrasound guided fine needle aspiration cytology. Result: The age of presentation ranged from 13 years to 81 years with a mean age of 34.9±13.5 years. Most common age group for breast lesions was in 21-30 age groups (39.1%). Fibro adenoma was the most common benign pathology (26.8%) followed by fibrocystic changes (26.4%). Malignancy was noted in 16.2% with majority in the 41-50 age groups. Parasitic infection was noted in 2 cases. Conclusion: Screening of breast lesions is warranted resulting in significant reduction of morbidity and mortality related to breast cancers. Fine needle cytology plays a pivotal role in the overall management of breast lesions.

Mullerian tumors are extremely rare malignancies in the retroperitoneum. We report a case of a 46-year old woman who presented with an eight year history of lower abdominal mass. Ultrasonography (US) and computed tomography (CT) demonstrated a 15×10 cm cystic mass in the left lower retroperitoneum. As serial percutaneous needle aspiration cytology was negative for malignancy, she was observed for seven years. Eleven months ago, the mass was excised. The histopathology was reported as mucinous adenocarcinoma of the retroperitoneum. Six cycles of intraperitoneal (IP) chemotherapy was administered during the last six months after diagnosis of recurrence by aspiration cytology and high serum tumor markers (CEA, CA19-9). A few days ago, positron emission tomographic (PET) scanning showed evidence of local recurrence and single vertebral metastasis, so she was admitted again for systemic chemotherapy. Meticulous revision of additional sections of the tumor revealed papillary, serous, mucinous, and endometrioid subtypes of the mullerian adenocarcinoma. To our knowledge, there has been no similar case described in the literature.

ABSTRACT Schwannoma is a benign, encapsulated tumor that is derived from schwann's cells. The most common site is parapharyngeal space of the neck; oropharyngeal occurrence is extremely rare. Fourteen year male boy presented with a history of difficulty in swallowing, more for solids for the last 8 months. The patient also had a change of voice for the last 5 months and was muffled in character. Examination showed single lobulated mass having smooth surface, of the size of approximately 5 × 4 cm and was arising from the tonsillolingual sulcus. The fine needle aspiration cytology from the lesion, reported to be schwannoma. The mass was excised completely under general anesthesia with the help of bipolar cautery. We report first case of schwannoma of tonsillolingual sulcus and also review the literature of the rare entity.

We describe the case of pulmonary hyalinizing granuloma in a 34-year-old asymptomatic man who presented with a pulmonary nodule apparent by chest radiography and computed tomography (CT). He had a history of previous treatment for tuberculosis. His laboratory data were normal. Bronchoscopy and CT-guided percutaneous transthoracic fine needle aspiration cytology were inconclusive. The diagnosis was revealed after the histopathological examination of an open lung biopsy.

Abstract Background: Breast cancer is a malignant disease with a heterogeneous prognosis. It is the most common cancer in the urban Indian population. Women over 30 years of age are especially prone to develop this type of cancer. There are various prognostic factors that have been studied in invasive ductal carcinomas to predict the tumour behaviour. As fine needle aspiration cytology is an important investigation for diagnosis and grading of the tumour, preoperative evaluation of the tumour behaviour can be studied. Aim: The aim of the study was to diagnose and grade the tumours cytologically by fine needle aspiration cytology (FNAC), to study the tumour behaviour by ultrasonography, by histo pathological examination and also to study the her2neu oncogene overexpression of the malignant tumours. Materials & methods: The present study was carried out in Gauhati Medical College, Guwahati, Assam from May, 2010 to July, 2012. The patients presenting with breast lumps were selected for the study. FNAC smears were stained by Papaniculaou and May Grunwald Giemsa stain. Ultrasonography of the breast lumps were done. Histopathological examination was done using the H & E stain. Immunohistochemical study was done to know the her2neu oncogene overexpression by the tumours. Results & observations: The age of patients with malignant tumours were found in the pre and perimenopausal age group. 48% of the malignant tumours were infiltrating duct carcinoma and of high grade. Her2neu overexpression was seen in 35% of the malignant tumours. Conclusion: A good correlation exists between the cyto-histopathologic grade of the tumours (85.3%), BIRADS 5 category on ultra sonography with histopathologic grade (98.5%) and her2neu oncogene overexpression along with histopathologic grade (93.3%). Therefore, cytological grading indicates an important prognostic marker to know the behaviour of breast carcinomas.

We report a case of metastatic endometrial carcinoma of the neck. A patient with a past medical history of squamous cell carcinoma of the larynx, breast carcinoma and endometrial carcinoma presented with a neck mass. Fine needle aspiration cytology (FNAC) showed this to be a poorly differentiated carcinoma with squamoid features and thus a potentially curative neck dissection was performed. Histology of the mass showed a clear cell endometrial carcinoma. Metastatic gynacecological malignancies to the head and neck are rare and this is the first reported case of metastatic endometrial carcinoma in the neck.

Phylloides tumor constitutes less than 1% of all breast tumors and 2 - 3% of fibroepithelial breast tumors. Several histological parameters should be evaluated, including stromal cellularity, atypia, mitoses, stromal overgrowth, infiltrative borders, and presence or absence of necrosis. Here we report a case of a 60 years- old female who presented with left breast lump. Fine needle aspiration cytology was done which suggested epithelial hyperplasia with fibrocystic changes. Biopsy was performed which showed predominance of stromal hypercellularity with proliferation of spindle cells (no atypia, mitosis and stromal overgrowth were noticed). However, a focus showed proliferation of discohesive tumor cells arranged singly and in single file. A diagnosis of benign phylloides tumor with foci of invasive lobular carcinoma was made. The diagnosis was confirmed with IHC which showed intense 80%positivity for estrogen and progesterone receptor and spindle cells showing positivity for bcl-2. In situ lobular carcinoma component was not observed. DOI: http://dx.doi.org/10.3126/jpn.v4i7.10321   Journal of Pathology of Nepal (2014) Vol. 4, 597-599
