This dossier focuses on some characteristics of the health reform processes in three southern European countries —Greece, Italy and Spain— over the second half of the twentieth century. We are interested in the proposals, projects and debates that sought to reorganise the health systems of these three countries. The paper on Italy analyses some aspects of the discussion that led to a national health service in 1978–

AbstractAlford's theory of structural interests has been used as a framework within which to analyse health systems across the world. However, authors have often been uncritical in their acceptance of Alford's original analytic categories. In this article we use data from a detailed qualitative study of the introduction of Practice Based Commissioning in the UK NHS to interrogate Alford's work more critically. Disrupting Alford's original categories of ‘professional monopolisers’ as dominant interests, challenged by management ‘corporate rationalisers’, we suggest that the new structures established in the NHS since 2002 systematically privilege an interest that we call ‘corporate monopolisers’, and that this is under challenge from ‘professional rationalisers’.

Introduction: On September 2016, the Board of the College of Endocrinology and Nutrition of the Portuguese Medical Association carried out a national survey, about all Endocrinology, Diabetes and Metabolism Departments of the public hospitals included in the Portuguese National Health Service and a simplified version of this survey was sent to all endocrinologists working in Portugal and registered with the Portuguese Medical Association.Material and Methods: Data related to organizational and human resources were collected, reporting the situation by the end of year 2015. The census registered 107 individuals and 27 Departments.Results: The ratio of endocrinologists-population was 1.4, much lower than in the other European countries (varies between 2 to 4), resulting in alarming shortages of services in some areas of Portugal and in worse quality indicators.Discussion: These data suggest that actions should be taken to increase the number of endocrinologists and departments in the country.Conclusion: In recent years, the number of residents has significantly increased, which will make it possible to correct this situation.

Im Artikel werden persönliche Erfahrungen als Advanced Nurse Practitioner (ANP) in der Thoraxchirurgie im National Health Service (NHS) in Großbritannien geschildert. Die tägliche Routine wurde von sieben Kompetenzdomänen bestimmt, nämlich: Management des Gesundheits- und Krankheitszustandes des Patienten, Beziehungen zwischen Pflegeperson und Patient, Lehren und Unterrichten, professionelle Rolle, Leitung und Führung innerhalb der Patientenversorgung, Qualitätsmanagement sowie kulturelle und spirituelle Kompetenzen. Diese Elemente wurden durch die Zusatzqualifikation, selbstständig Medikamente verschreiben und verordnen zu dürfen, erweitert, was dazu beitrug, eine nahtlose Erbringung von Pflege- und Serviceleistungen zu gewähren. Die Position wurde zur zentralen Anlaufstelle im multi-professionellen Team und stellte eine kontinuierliche Weiterführung der Pflege von Patienten und ihren Familien sicher – auch über Krankenhausgrenzen hinweg.
