The primary purpose of this study was to examine the effects of an exercise program on physical fitness, psychosocial factors, and symptoms in two men with fibromyalgia syndrome (FMS). An interventional case study research design was used to collect data on physical fitness, psychosocial factors, and symptoms at intervals throughout a 14-month period. Raw scores and raw score averages, devoid of statistical manipulation, are presented to display real individual results. The secondary purpose of the study was to discuss how these two men tolerated the program in comparison to women in a similar program. This is important because data on men with FMS appear to be unavailable, because the greater majority of participants in FMS programs are women. Results support that these two men benefited from the program and that differences between men and women with FMS should be considered relative to the conduct of exercise programs for these groups.

Rationale: A novel COVID-19 with a severe acute respiratory syndrome or pneumonia had arisen in Wuhan, China in December 2019. Emerging atrial fibrillation in COVID-19 patients is highly significant in cardiovascular medicine. A newly coronary artery spasm in the presentation of COVID-19 infection has certainly a risk impact on both morbidity and mortality of COVID-19 patients. Wavy triple an electrocardiographic sign (Yasser Sign) is an innovated sign of hypocalcaemia linked to tachypnea and acute respiratory distress. Patient concerns: An elderly male COVID-19 patient presented to physician outpatient clinic with bilateral pneumonia, atrial fibrillation, evidence of coronary artery spasm, and Wavy triple an electrocardiographic sign (Yasser Sign). Diagnosis: COVID-19 pneumonia with coronary artery spasm and the Wavy triple an electrocardiographic sign (Yasser Sign). Interventions: Chest CT scan, electrocardiography, oxygenation, and echocardiography. Outcomes: Gradual dramatic clinical, electrocardiographic, and radiological improvement had happened. Lessons: The reversal of electrocardiographic ST-segment depressions in a COVID-19 patient after adding oral nitroglycerine is an indicator for the presence of coronary artery spasm. It signifies the role of the anti-infective drugs, anticoagulants, antiplatelet, and steroids in COVID-19 patients with bilateral pneumonia, AF, coronary artery spasm are effective therapies. The disappearance of AF after initial therapy may a guide for a good prognosis in this case study. The evanescence of Wavy triple ECG sign as a hallmark for the existence of the Movable-weaning phenomenon of hypocalcaemia is recommended for further wide-study.

Considering the perceived benefit of early recruitment and the time and resources spent developing youth players, individuals released from talent development programmes are often re-recruited by rival academies. However, due to the contractual nature of many talent development programmes, limited empirical data exists on players deselected from (or reselected to) youth soccer academies. Adopting a novel case study approach, differences in skill, psychological, and physical attributes associated with reselection following closure of a junior-elite soccer academy were explored. Overall subjective coach ratings for skill, psychological, and physical abilities; subjective coach ratings for skill and psychological attributes; and physical fitness test performance of 79 junior-elite soccer players (U11–U17) were assessed as part of regular scheduled testing and monitoring practices prior to the academy closure. Reselection status was monitored and recorded for all players in the 6 months following the academy closure and was classified as a persistence/progression (“Reselected”) or attrition (“Deselected”) in playing level. Of the 79 released players, a total of 60 players (76%) were re-signed to a junior-elite academy within 6 months. Differences were observed for overall ratings of skill, psychological, and physical abilities in favor of the “Reselected” player group. “Reselected” players were also rated higher by coaches for all attributes categorized as skill and psychological, as well as performing better at all physical fitness tests. However, “Reselected” players were lesser in stature and body mass and less mature than “Deselected” players. Our findings suggest that reselection is not a product of anthropometric criteria and, therefore, a pathway for selection remains open for later maturing players. We also inform upon desirable qualities associated with player reselection and provide a case study approach of a unique, yet highly relevant, scenario for talent identification and development in youth soccer.

Purpose:To describe the physical fitness of a top-level lower limb amputee (LLA) cyclist and paracycling time-trial (TT) race demands.Methods:The 40-y-old male unilateral transfemoral amputee TT World Champion was tested in a laboratory for peak oxygen uptake (VO2peak), ventilatory threshold (VT2), power output (PO), and hemoglobin mass (Hb-mass). Moreover, several measures (eg, PO, heart rate [HR], cadence) were collected during 4 international TT competitions in the same season. The races’ intensity was evaluated as time spent below, at, or above VT2.Results:The cyclist (1.73 m, 55.0 kg) had a VO2peak of 3.372 L/min (61.3 mL · kg−1 · min−1). The laboratory peak PO was 315 W (5.7 W/kg). The maximal HR was 208 beats/min, and his Hb-mass was 744 g (13.5 g/kg). The TTs were meanly 18 ± 4.5 km in length, and the mean PO was 248 ± 8 W with a cadence of 92 ± 1 rpm. During the TTs, the cyclist spent 23% ± 9% of total time at VT2, 59% ± 10% below, and 18% ± 5% above this intensity.Conclusions:The subject’s relative VO2peak is higher than previously published data on LLA, and surprisingly it is even higher than “good” ACSM normative data for nondisabled people. The intensity of the races was found to be similar to cycling TTs of the same duration in elite female cyclists. These results might be useful to develop specific training schedules and enhance performance of LLA cyclists.
