Background: Animal studies show that ovarian hormone deficiency is associated with development of insulin resistance. In women, the menopause transition marks the cessation of ovarian function and insulin sensitivity would be reduced in postmenopausal women.Aim and Objectives: This study aimed to evaluate the insulin sensitivity in Myanmar postmenopausal women compared to premenopausal womenMaterial and Methods: A cross sectional comparative study was undertaken in non obese, normal glucose tolerant post menopausal women (n=42, BMI (mean± SD) 21.3±2.8 kg/m2, age 52.6±4.4 years) and premenopausal women (n=33, BMI(mean± SD)  19.7±2.2 kg/m2, age32.7±4.9 years). Insulin sensitivity was assessed by homeostasis model assessment method of insulin resistance (HOMA-IR) based on fasting blood glucose and fasting serum insulin level. Blood glucose was determined by glucose oxidase method. Serum insulin was estimated by enzyme linked immunosorbent assay (ELISA).Results:No differences in fasting blood glucose level (Mean ±SD 4.87±0.51 vs. 4.76±0.63mmol/L), fasting serum insulin level [median and interquartile range (IQR) 8.1 (6.7-11.8) vs. 8.1(6.5-9.9)    µ IU/ml] and HOMA-IR [median and IQR 1.7 (1.4-2.8) vs. 1.7 (1.2-2.2)] were found between premenopausal and postmenopausal women.Conclusion: Postmenopausal status has no effect on insulin sensitivity in postmenopausal women.Asian Journal of Medical Sciences Vol.7(5) 2016 36-41

Background: Infertility has become a global health problem in the world wide affecting 8-10% of couple. It is also a matter of social injustice and inequality. Increase level of insulin has been implicated as a cause of infertility. Objective: To find out the association of fasting serum insulin level with gonadotropins in infertile women. Methods: This cross sectional study was conducted in the department of Physiology, Dhaka Medical College, Dhaka from July 2010 to June 2011. A total number of 150 female age ranged from 20 – 40 years were included in this study. Out of them100 infertile women were selected as study group (group B). Group B was subdivided into group B1 and B2. Group B1 consisted of 50 primary infertile women and group B2 consisted of 50 secondary infertile women. Rest 50 age matched apparently healthy parous women were considered as base line control group A. All the study subjects were selected from out patient department of infertility unit, BSMMU, Dhaka. The control subjects were selected by personal contact. Serum fasting insulin was measured by enzyme-link-immunosorbend assay. Fasting blood glucose and blood glucose two hours after breakfast were measured by glucose oxidase method. The Data were collected in a prescribed data sheet after taking written consent. Statistical analyses were done by unpaired students “t” tests by SPSS program version 12. The level of significance was calculated and p value <0.05 was accepted as level of significance. Results: In this study, the mean fasting serum insulin level were significantly higher in infertile women than those of fertile women (p<0.001). Within the study group serum fasting insulin was higher in primary infertile women than that of secondary infertile women both were statistically not significant. Again, serum FSH and LH levels were significantly lower (P<.0001) in infertile women than those of fertile women. But serum FSH level was lower and LH level was higher in primary infertile women than that of secondary infertile women. In addition, fasting blood glucose level was almost similar but within normal limits in all groups. Blood glucose 2HABF was significantly higher in secondary infertile women than that of fertile women but within in normal limit. Moreover, fasting serum insulin level was negatively correlated with serum FSH and LH in primary infertility but negatively correlated with serum FSH and positive correlation with serum LH in secondary infertility. Conclusion: From the above study it may be concluded that fasting serum insulin level was higher in infertile women than those of healthy fertile women. An association between hypertension and diabetes mellitus has been identified.Objectives: The study was carried out to observe the association of hypertension with diabetes mellitus in adult female subjects.Methods : This cross sectional study was conducted in the Department of Physiology, Dhaka Medical College, Dhaka, during the period of January 2011 to December 2011. Systolic blood pressure and diastolic blood pressure of all participants were recorded by standard method. Data was analyzed by Unpaired Student’s- test and Pearson’s correlation co-efficient (r) test as applicable.Results: The value of fasting serum insulin level was significantly higher in diabetic subjects than those of non-diabetic subjects. Fasting blood glucose level was significantly higher diabetic patient in comparison to those of non-diabetic subjects. In diabetic subjects fasting serum insulin and fasting blood glucose levels were higher and showed positive correlation with systolic and diastolic blood pressure.Conclusion: Present study revealed that systolic and diastolic blood pressure has positive relationship with fasting serum insulin and fasting blood glucose levels.Bangladesh J Medicine Jan 2018; 29(1) : 7-12

Background: An association between essential hypertension and defective insulin secretion has been identified. Objective: To estimate fasting serum insulin level in adult male with essential hypertension to observe its relationship to hypertension. Methods: This cross sectional study was conducted in the Department of Physiology, Dhaka Medical College, Dhaka from July 2012 to June 2013. A total number of one hundred fifty male subjects were selected with age ranging from 25 to 45 years. Seventy five male essential hypertensive were enrolled from Out- patient Department of Medicine, Dhaka Medical College Hospital, Dhaka. Age matched seventy five apparently healthy males were studied as control. Fasting serum insulin level was measured by ELISA method and fasting blood glucose by glucose oxidase method. For statistical analyses, unpaired Student’s ‘t’ and Pearson’s correlation coefficient (r) tests were performed. Results: Fasting serum insulin level was significantly (P<0.001) higher in essential hypertensive male patients than normotensive subjects. Fasting serum insulin level shows significant positive correlation with systolic blood pressure in hypertensive patients. Conclusion: This study reveals that essential hypertension has positive and significant relationship with fasting serum insulin level. Background: Exercise can ameliorate type II diabetes mellitus (T2DM) by regulating intestinal flora metabolites. However, the detailed mechanism needs to be further explored. Methods: A T2DM model using mice was established by feeding them a high-fat diet and giving them subsequent streptozocin injections. Fasting blood glucose and serum insulin were determined by blood glucose meter and radioimmunoassay, respectively. Intestinal flora was measured by 16sRNA sequencing. SCFA content was measured by gas chromatography (GC) or enzyme-linked immunosorbent assay (ELISA). A fluorescently labeled 2-deoxyglucose (2-NBDG) kit was employed to detect glucose uptake capacity, and western blot was utilized to explore the signaling pathway of insulin resistance and cell autophagy. Results: In the T2DM model, along with a reduction in insulin resistance (IR), exercise reversed the decline of intestinal Bacteroidetes and the increase of Firmicutes. For metabolites of Bacteroides, exercise restored the decline in total intestinal and plasma short-chain fatty acids (SCFAs) in T2DM mice. However, the administration of GLPG0974—the inhibitor of G protein-coupled receptor 43 (GPR43), which is the receptor of SCFAs—abolished exercise-mediated alleviation in IR in vivo and acetate-mediated reduction of skeletal muscle IR (SMIR) in vitro. Mechanistically, exercise induced skeletal muscle cell autophagy, thereby ameliorating SMIR, which was neutralized by GLPG0974 exposure. Conclusions: Exercise-mediated SCFAs-upregulation may ameliorate insulin resistance (IR) through increasing autophagy of skeletal muscle cells by binding to GPR43. This study provides a theoretical basis for targeting gut bacterial metabolites to prevent T2DM.

Hyperproinsulinemia is commonly present in subjects with impaired glucose tolerance. The present study was undertaken to investigate the proinsulin level in Bangladeshi IGT subjects and to explore its association with insulin resistance. This observational study was conducted under a case-control design with IGT subjects (n=50) and controls (n=44). IGT was diagnosed following the WHO Study Group Criteria. Serum glucose was measured by glucose-oxidase method, serum lipid profile by enzymatic method and serum insulin and serum proinsulin were measured by ELISA method. Insulin secretory capacity (HOMA%B) and insulin sensitivity (HOMA%S) were calculated from fasting serum glucose and fasting serum insulin by homeostasis model assessment. The study subjects were age- and BMI- matched. Mean (±SD) age (yrs) of the control and IGT subjects were 40±6 and 40±5 respectively (p=0.853). Mean (±SD) BMI of the control and IGT subjects were 23±3 and 22±2 respectively (p=0.123). Fasting glucose was not significantly higher in IGT subjects, but serum glucose 2 hours after 75 gm glucose load was significantly higher in IGT subjects. Median (Range) value of fasting serum glucose (mmol/l) of control and IGT subjects were 5.3 (3.8-6) and 5.2 (4-12) respectively; (p=0.297). Median (Range) value of serum glucose (mmol/l) 2 hours after 75 gm glucose load of control and IGT subjects were 6.1 (3-7.8) and 7.9 (5- 21) respectively; (p=0.001). Fasting TG was significantly higher in IGT subjects and LDL-c was significantly lower in IGT subjects. Serum Total cholesterol and HDL-c were not significantly different between the IGT and control subjects. Median (Range) value of fasting serum TG (mg/dl) of control and IGT subjects were 119 (51-474) and 178 (82-540) respectively; (p=0.001). Median (Range) value of fasting serum T chol (mg/dl) of control and IGT subjects were 180 (65-272) and 186 (140-400) respectively; (p=0.191). Median (Range) value of fasting serum HDL-C (mg/dl) of control and IGT subjects were 29 (19-45) and 31 (15-78) respectively; (p=0.914). Median (Range) value of fasting serum LDL-C (mg/dl) of control and IGT subjects were 117(29-201) and 111(41- 320) respectively; (p=0.001). Fasting serum proinsulin was significantly higher in IGT subjects. Median (Range) value of fasting serum proinsulin (pmol/l) of control and IGT subjects were 9.2(1.8-156) and 17(3-51) respectively; (p=0.001). Insulin secretory capacity (HOMA%B) was higher but insulin sensitivity (HOMA%S) was significantly lower in case of IGT subjects. Median (Range) value of HOMA%B of control and IGT subjects were 97(46-498) and 164(17-300) respectively; (p=0.001). Median (Range) value of HOMA%S of control and IGT subjects were 68(19-270) and 39(15-110) respectively (p=0.001). In multiple regression analysis a significant negative association was found between fasting proinsulin and insulin sensitivity (p=0.037). The data led to the following conclusions: a) Insulin resistance is the predominant defect in Bangladeshi IGT subjects. b) Basal proinsulin level is significantly increased in IGT subjects. c) Insulin resistance is negatively associated with serum proinsulin in IGT subjects. Background Adipose tissue is not merely a site for energy storage, but is also the largest endocrine organ, secreting various adipocytokines. Plasma visfatin, an adipocytokine predominantly secreted from visceral adipose tissue, has insulin-mimetic effects, and has been closely linked to insulin resistance.Objective To compare plasma visfatin levels between obese and non-obese adolescents, as well as between obese adolecents with and without insulin resistance.Methods This cross-sectional study was conducted in students who attended three senior high schools in Padang. Subjects comprised 28 obese and 28 non-obese adolescents. The age of the subjects ranged from 14-18 years. Obesity criteria were based on body mass index (BMI) measurements. Fasting serum glucose level was measured by glucose hexokinase photometry and serum insulin was measured by chemiluminesence immunoassay. Plasma visfatin was measured by enzyme-linked immunosorbent assay (ELISA). The insulin resistance index was estimated from fasting serum insulin and glucose levels using the homeostatic model assessment for insulin resistance (HOMA-IR). Differences in the variables were tested using independent T-test and Mann-Whitney test, depending on the distribution of the variables.Results The mean plasma visfatin level was significantly higher in the obese than in the control group [2.55 (SD 1.54) vs. 1.61 (SD 0.64) ng/mL, respectively; (P=0.005)]. The insulin resistant group had significantly higher mean plasma visfatin level than the non-resistant group [3.61 (SD 1.59) vs. 1.96 (SD 1.18) ng/mL, respectively; (P=0.004)].Conclusion Obese adolescents with insulin resistance have signifcantly higher plasma visfatin levels compared to those without insulin resistance.

Many traditional treatments have been recommended in the alternative system of medicine for diabetes mellitus. However, the mode of action of most of the herbals used has not been defined. It has been reported that sex hormones are important regulators of insulin-mediated events in skeletal muscles. In view of this, a novel herbal preparation containing antidiabetic and aphrodisiac plants was used in the present study. Adult male albino rats were divided into following groups after induction of diabetes. Rats were given an intraperitoneal (i.p.) injection of streptozotocin (STZ), at a dose of 65 mg/kg body weight after overnight fasting, to induce diabetic state with blood glucose levels >250 mg/dL. Group 1—Control rats treated with single i.p. injection of vehicle, Group 2—Rats treated with polyherbal preparation (PHP; 500 mg/kg body weight by oral intubation, morning and evening for 30 days), Group 3—STZ-diabetic rats treated orally with equal volumes of vehicle (water) alone and Group 4—STZ-diabetic rats treated with PHP after 10 days of diabetic induction. STZ-diabetes decreased the body weight, serum insulin level and glucose oxidation in liver and skeletal muscles but increased the fasting blood glucose level. After polyherbal treatment, body weight and glucose oxidation were completely restored to control level while serum insulin level was restored partially and the glucose tolerance was significantly improved. There was a significant decrease in total haemoglobin (Hb) level of diabetic rats when compared to control but polyherbal treatment significantly improved the same. However, the other parameters studied (red blood cell [RBC], white blood corpuscle [WBC], packed cell volume [PCV], mean corpuscular volume [MCV] and mean corpuscular haemoglobin [MCH]) were unaltered. In conclusion, the anti-diabetic properties of PHP appear to be mediated through pancreatic β-cell regeneration, resulting in maintenance of optimal blood glucose and its oxidation in liver and skeletal muscles.
