Double primary malignant tumor in a centenarian female: Case report and review of literature

A hundred and three year old female patient was evaluated for ulcer over the nasion area and a mass arising from the right upper eyelid, appearing two months apart. Histopathology revealed nasion ulcer as basal cell carcinoma while right eyelid mass was diagnosed as poorly differentiated squamous cell carcinoma. Based on these findings and the time gap between two lesions a diagnosis of synchronous double primary malignant tumor was made. Although the prevalence of multiple primary malignant tumor is highest in breast, colo-rectum and prostate cancer we report a rare case of double primary malignant tumor of the eyelid and nasion in a centenarian female.

Metastatic carcinomas in the nasopharynx are a rarity. We report a case of a 54-year-old male patient who presented with a history of recurrent epistaxis. On evaluation, a mass in the right Rosenmüller fossa was detected, which was biopsied and diagnosed as a poorly differentiated adenocarcinoma, immunoreactive for thyroid transcription factor-1, consistent with metastatic pulmonary adenocarcinoma. Fluorodeoxyglucose–positron emission tomography/computed tomography (FDG-PET/CT) scan confirmed the presence of a mass in the upper lobe of the left lung, proven pathologically to be a poorly differentiated pulmonary adenocarcinoma, with an immunoprofile similar to the nasopharyngeal tumor. The patient underwent palliative chemotherapy, and was then shifted to immunotherapy. He is currently alive and disease free 50 months after the initial diagnosis was made. The unusual presentation of a metastatic carcinoma in the nasopharynx will be discussed, along with a review of literature. The role of immunotherapy in cancer control and greater longevity will also be presented.

Renal cell carcinoma – the third on occurrence frequency tumor of urinogenital system and the most widespread renal tumor which makes about 2–3% of malignancies at adults. Doubling of inferior vena cava (IVC) is anomaly in case of which two inphrarenal segments of IVC are defined. Left IVC after lockin in it the left renal vein crosses an aorta in front, connects to the right renal vein and the right IVC. Combination of renal cell carcinoma and doubling IVC is rather seldom. The presented clinical case of a combination of renal cell carcinoma and doubling of IVC has the feature existence at the patient of metachronous multiple primary neoplasms that does necessary more careful inspection of the patient. Also a doubling of renal veins on both sides and a doubling of the left renal artery have been revealed. Possibilities of presurgical non-invasive diagnostics (ultrasonography and computer tomography) of retroperitoneal space vessels anomaly at the patient with transmural localization of kidney tumor allowing to plan and execute difficult surgery – an ex vivo resection of a left kidney in the conditions of pharmacological cold ischemia was shown. Performance of an ex vivo nephrectomy in the conditions of pharmacological cold ischemia allows to dilate indications to organ-preserving treatment of patients with the localized kidney cancer. However long cold ischemia and the subsequent vascular reconstruction demand dynamic observation over kidney’s functional conditions.

Small cell carcinoma of the ovary hypercalcemic type (SCCOHT) is an exceedingly rare aggressive malignancy. It typically affects young women. Herein we aimed to report a case of a 28-year-old woman treated for SCCOHT in Salah Azaiez Institute. The tumor was revealed by a delay of menses. The imaging found a large mass of the right ovary. The patient underwent a bilateral salpingo-oophorectomy, a total hysterectomy and a pelvic and a para-aortic lymphadenectomy. She subsequently received 6 cycles of adjuvant chemotherapy. Three years later, she presented a metastatic bone relapse.

Head and neck reconstruction serves two purposes, namely to restore function and aesthetics. The priority goals for lip reconstruction should be determined before surgery. In cases of large lip defects, maintaining the goal makes lip reconstruction difficult. This is a case report of a 66 year old man who came to ORL-HNS OPD, Dr. Soetomo Hospital, Surabaya with the main complaint of ulcers on the upper lip since one year ago accompanied by pain. A mass biopsy result on the right vestibule in Malang on October 11, 2016 showed adenosquamous cell carcinoma. The patient planned to have a wide excision and reconstruction of the lip defect. Wide excision and lip reconstruction was performed on 15 September 2016. Total resection of the upper lip was performed up to the vestibule of the nose. The results showed that squamous cell carcinoma was poorly differentiated. The evaluation after myocutaneous flap surgery was well combined, there were no signs of inflammation or infection. Patients can open their mouths, eat, drink and speak without complaint. The patient then received regular chemoradiotherapy two weeks after surgery.
