Study Objective: To study the prevalence of depressive syndromes in patients with chronic lumbosacral radiculopathy and pain syndrome, and to identify correlations between neuropathic pain and psychoemotional disturbances. Study Design: open perspective study. Materials and Methods. The study included 126 patients (45 (35.7%) males and 81 (64.3%) females) of 19 to 78 years old with verified unilateral lumbosacral radiculopathy of L4, L5, S1 radicules resulting from degenerative damage of the spine lasting for more than 12 weeks. For assessment, we used the Pain Detect Questionnaire, visual analogue scale for pain syndrome, Oswestry Disability Index, and Beck Depression Inventory. Study Results. Among patients with chronic lumbosacral radiculopathy, depressive syndromes were diagnosed in 74 (587%) cases as follows: 26 (20.6%) patients had mild (subclinical) depression, 33 (26.2%) cases were of moderate intensity, 12 (9.5%) patients had marked, and 3 (2.4%) had severe depression. We used Spearmen’s rank-order correlation to identify statistically significant correlations between Beck Depression Inventory score and neuropathic pain syndrome intensity (Pain Detect Questionnaire) (r = 0.861; p = 0.006). Pain Detect Questionnaire and Oswestry score correlated as well (r = 0.745; p = 0.001). Conclusion. Chronic pain syndrome in patients with lumbosacral radiculopathy resulting from degenerative damage of the spine is characterised by high comorbidity with depression. The results justify inclusion of psychoemotional assessment into a routine screening program for patients with lumbosacral radiculopathy. Purpose of the work. The article is devoted to the first experience of epidural stimulation in the Khanty-Mansiysk Autonomous Okrug at the budgetary institution Surgut Clinical Trauma Hospital. Clinical examples are presented for two main indications for the application of this technique (disease of the operated spine, a consequence of spinal cord injury in combination with chronic neuropathic pain syndrome). Research methods. An assessment of the intensity of pain syndrome was given according to a visual analogue scale, the Pain Detect questionnaire; indicators of anxiety, depression on the HADS scale; quality of life according to the Oswestry questionnaire for a follow-up period of 6-12 months in patients with chronic epidural stimulation. Results. A positive assessment of the action during test neurostimulation was 63.3% (38 patients). Of the established permanent systems, a good result was achieved and persisted for 12 months or more in 96% (24 patients). It was necessary to change the stimulation parameters in 13% (3 patients). Revision of permanent systems was performed in 20% (5 patients), due to the progression of the degenerative-dystrophic process of the spine, damage and migration of system elements. Conclusions. Chronic epidural spinal cord stimulation has established itself as a personalized, highly effective, minimally invasive and safe method of treating chronic neuropathic pain syndromes. Multicomponent corrective action is of scientific interest and requires further study.

The Aim of the Research. Based on the analysis of modern literature data and our own research to substantiate the principles of rational complex therapy with acute poisoning by thallium compounds (Tl+) at different stages of intoxication. Materials and Methods. The analysis of literature data and our own researches is carried out and the principles of rational complex therapy with acute poisoning by thallium compounds at different stages of intoxication development are substantiated and the algorithm of doctors’ actions at suspicion of acute Tl+ poisoning is defined. Results and Conclusions. The principles of complex therapy of acute Tl+ poisonings substantiated, which includes antidote therapy, methods of extracorporeal treatment (plasmapheresis, hemosorption, hemodialysis), treatment of toxic encephalopolyradiculoneuropathy, neuropathic pain syndrome and toxic liver damage. The sequence of appearance of the main symptoms of intoxication at low and high doses of Tl+ is determined, which allows to assume poisoning by this toxicant.
