In Nepal, we started regular open-heart surgery since February 1997. Primary tumors of the heart are rareand the commonest is myxoma. Left atrium is the commonest site of occurrence. There is diagnostic difficultyin this most frequent benign tumor of the heart.We present 5 cases of left atrial myxomas who presented with different clinical features. Among them 4cases were diagnosed preoperatively by transthoracic echocardiography.The youngest was 32 years whereas the oldest was 58 years (mean 49.8 years) and 4 of them were female.Among them, one presented with right unilobar pulmonary venous hypertension; another mimicking mitralstenosis with large left atrial clot; with embolization to brain with motor the third aphasia and the remainingtwo had only constitutional symptoms. All of them were discharged with successful removal of tumor andare surviving at present with no features of recurrence.There are no pathognomic features of atrial myxoma. All suspected cases must be subjected forechocardiography. With the advent of open-heart surgery, extirpation of tumors with good outcome hasbeen a regular service in our hospital.Key Words: Atrial myxoma, Transthoracic Echocardiography, Embolization, Recurrence.

A 36-year-old male involved in a car accident was found to have an embolic stroke due to a left atrial myxoma. Open heart surgery was delayed 4 weeks to decrease the risk of neurologic complications from the anticoagulation required for cardiopulmonary bypass. After resection of the myxoma, intraoperative transesophageal echocardiography found severe mitral regurgitation, which was repaired.

Purpose: The value of coronary angiography in the diagnosis of thrombus in the left atrial appendage (LAA) was retrospectively analyzed. Material and Methods: The study covers 34 patients in whom coronary angiography showed coronary neovascularity in LAA with coronary artery-left atrial fistula indicating LAA thrombus. All 34 patients underwent transthoracic echocardiography within one week of coronary angiography. Open-heart surgery was undertaken 2–31 months after angiography in 28 patients. Results: Coronary neovascularity and coronary artery-left atrial fistula arose from the left circumflex artery in 28 patients, and from the left circumflex artery and the right coronary artery in the remaining 6 patients. By echocardiography, LAA thrombus was detected in only one of the 34 patients. In 18 of the 28 patients who underwent open-heart surgery, LAA thrombus was found at surgery to have resolved. Conclusion: Coronary angiography is useful in the diagnosis of LAA thrombus, and coronary neovascularity and fistula formation may indicate that the thrombus can spontaneously resolve.

Gradual Onset

Left atrial myxomas are benign, slow-growing primary cardiac tumors. They present with gradual onset of one or more of a triad of obstructive, embolic, or constitutional symptoms. Echocardiography aids in the detailed preoperative and intraoperative evaluation of the myxoma for surgical strategy planning. We describe a case of interstitial hemorrhage in a left atrial myxoma leading to rapid expansion of the tumor with features of acute, mitral valve obstruction. Echocardiography showed a cystic area in the left atrial tumor that corresponded to an area of recent hemorrhage confirmed on surgical removal. A 42 yrs old housewife presented with severe dyspnoea, cough and chest pain for 7 days, after detail evaluation she was diagnosed as a case of left atrial myxoma with heart failure. Subsequently she underwent emergency open heart surgery under cardiopulmonary bypass; removal of a huge LA myxoma was done, and recovered uneventfully.Cardiovasc. j. 2016; 8(2): 155-157

Objectives: Although echocardiography has traditionally been used to diagnose myxoma, invasive or non-invasive coronary angiography can be useful diagnostic tool before surgery. Methods: We present a case of an angiographically detected left atrial myxoma feeding from the left circumflex coronary artery. Results: The patient underwent open-heart surgery to remove the left atrial myxoma. After ligation of feeding artery, the mass was successfully excised Conclusion: Preoperative coronary angiography can offer additional valuable information moreover detecting coronary artery disease. Because, there is sudden death risk from embolization during invasive coronary angiography, preoperative cardiac computed tomography angiography should be considered to plan the surgery of myxoma.

Listeriosis has a variety of clinical manifestations from febrile gastroenteritis to bacteremia, sepsis, meningoencephalitis and infective endocarditis with valvular heart disease. In this report, we present a rare case of successful treatment of infective endocarditis of the mitral valve due to Listeria monocytogenes after previous open-heart surgery.
