We present the case of a 41 years old patient with poor social status and previous facial trauma with a sharp object (wood branch) 2 years ago. Although it was performed the extraction of the branch in a local ENT department, the patient developed a fistula in the left genian region with the presence of puss. Two months ago the patient undergoes another head trauma leading to neurosurgery. CT scan raises the suspicion of a foreign body in the left maxillary sinus. We performed left maxillary sinus surgery through Caldwell-Luc approach with extraction of the foreign body and closure of the fistula. Surgical healing was optimal within 10 days. We analyze the medico-legal implications of such cases.

Significance statement: A 53-year-old man with left facial pain was referred to our hospital. Nasal endoscopy revealed a purulent discharge at the left middle meatus and bulging of the uncinate process. Computed tomography demonstrated that the dental implant was blocking the left maxillary ostium. Functional endoscopic sinus surgery was performed under general anesthesia. After removal of the uncinate process, a yellowish purulent discharge in the left maxillary sinus discharged from the maxillary sinus. The presence of the dental implant in the infundibulum shows the direction of mucociliary clearance from the nasal sinus. This case indicates how our sinus clears a foreign body, and the direction in which the foreign body is removed.

Ameloblastoma (AM) is a slow growing and aggressive benign tumor with an odontogenic epithelial origin arising from the mandible or maxilla. The odontogenic neoplasm invades local tissues asymptomatically and accounts for 1% of oral tumors and over 10% of odontogenic tumors. A 64-year-old man with a history of allergic fungal rhinosinusitis (AFRS) undergoing a revision image-guided endoscopic sinus surgery was found to have a fibrous mass suspicious of malignancy projecting inferolaterally and attached to the floor of the left maxillary sinus. Diagnostic biopsies were taken, and additional surgery was required to successfully resect the tumor via a transnasal endoscopic dissection. Multiple permanent pathology samples concluded the diagnosis of an AM. Endoscopic investigations led to the incidental discovery and ultimate complete endoscopic resection of the AM. The utilization of an endoscopic resection compared to the traditional maxillectomy with reconstruction results in significant less short and long-term morbidity for the patient.

Introduction: The variation in the course of infraorbital canal and protrusion of the infraorbital nerve through it to the maxillary sinus may lead to its accidental injury during reconstructive or endoscopic sinus surgery. Preoperative identification of this variant will prevent unintended injuries. Methods: A retrospective study of 307 patients who underwent CT scan study of the paranasal sinuses at Chitwan Medical College, Nepal was conducted. The protrusion of infraorbital nerve to the maxillary sinus was identified and the length of the bony septum along with the infraorbital nerve was measured. It was further classified as Class I to III according to the length of the septum.Results: The prevalence of protrusion of inferior orbital nerve in our study was 11.40 % and bilateral protrusion was 5.8 %.  The median length of the protruding component along with the septum was 4.9 mm. Conclusion: Preoperative identification of the normal protrusion of infraorbital nerve to the maxillary sinus will prevent accidental injuries during sinus surgery. CT scan of the paranasal sinus would be the modality of choice for identification of this variant.

ABSTRACT Objective Chronic sinusitis affects more than 30 million people each year worldwide. Computed tomography (CT) has become a standard diagnostic tool in the evaluation of paranasal sinuses (PNS), but medical literature lacks studies correlating preoperative CT and intraoperative findings of functional endoscopic sinus surgery (FESS). The aim of our study was to evaluate ostiomeatal unit on multidetector CT (MDCT) and correlate findings of MDCT with intraoperative findings during FESS in patients with chronic sinusitis and hence check for its usefulness during surgery. Study design Prospective clinical study. Materials and methods Forty-nine patients with chronic sinusitis who visited the Lok Nayak Hospital, Delhi, India, between October 2010 and February 2012 were included in the study. All such patients underwent preoperative CT scan of PNS before surgery, i.e., FESS with or without septoplasty under general anesthesia. Results Maxillary sinus was found to be the most common sinus involved in chronic sinusitis. Preoperative CT findings correlated well with intraoperative findings for all sinuses except left maxillary sinus. Conclusion Multiplanar CT of nose and PNS helps to deli­neate the anatomy of nose and PNS, and drainage pathways of sinuses preoperatively. Therefore, the operating surgeon should be well versed with it. Progress of endoscopic sinus surgery can be partially attributed to revolution in imaging modalities. A case is presented in which a sphenoid sinus mucocele was indicated by rapidly progressive blindness in the left eye. Recognition and extension of the mucocele was made possible with a computerized tomographic (CT) scan and tomograms,1-5 which outlined its boundaries and the involvement of the maxillary sinus, the ethmoid, and the left orbit. Immediate decompression and marsupialization of the cyst through the left maxillary sinus, using the Caldwell-Luc approach, gave excellent results. Immediate improvement of vision was noted and almost complete return of vision occured within six weeks.

Instrument fracture during procedure is not uncommon for dental surgeons, especially in root canal surgeries, usually inside the root canals. In rare instances, high-speed rotary instruments can be fractured and can be dislodged in key anatomical areas of face. In our case report, a high-speed dental burr most probably penetrated the root and was seen in the left maxillary sinus during a likely routine dental procedure. The work-up and endoscopic surgical management of the case is described. Practitioners should be in great care during dental procedures and endodontic treatment to avoid unexpected complications by introducing foreign bodies into maxillary sinus. Any patient presenting with recurrent unilateral facial pain or unilateral sinus symptoms with/without previous history of sinusitis should raise the suspect of a foreign body in the paranasal sinus regardless of any previous history of dental procedure.

Rhinolithiasis is a rare condition in our current practice. We report the case of a 19 year old woman, presenting with a double symptom marked by rhinorrhea and chronic unilateral nasal obstruction evolving since the age of 7 years, without any known foreign body. Rhinoscopy and nasal endoscopy coupled with CT scan were decisive examinations for the positive diagnosis. The therapeutic management consisted in the extraction of the rhinolithiasis resulting in the vacuity of the nasal cavity. The evolution was favorable with simple postoperative courses. Foreign bodies in maxillary sinuses are unusual clinical conditions, and they can cause chronic sinusitis by mucosal irritation. Most cases of foreign bodies in maxillary sinus are related to iatrogenic dental manipulation and only a few cases with non-dental origin are reported. Oroantral fistulas secondary to dental procedures are the most common way of insertion. Treatment is surgical removal of the foreign body either endoscopically or with a combined approach, with Caldwell-Luc procedure if endoscopic approach is inadequate for visualisation. In this case, we present a 24-year-old male patient with unilateral chronic maxillary sinusitis due to a wooden toothpick in left maxillary sinus. The patient had a history of upper second premolar tooth extraction. CT scan revealed sinus opacification with presence of a foreign body in left maxillary sinus extending from the floor of the sinus to the orbital base. The foreign body, a wooden toothpick, was removed with Caldwell-Luc procedure since it was impossible to remove the toothpick endoscopically. There was no obvious oroantral fistula in the time of surgery, but the position of the toothpick made us to think that it was inserted through a previously healed fistula, willingly or accidentally.

Postoperative maxillary cyst is a quite rare delayed complication of surgical intervention associated with maxillary sinuses. It occurs many years after surgery. This paper describes a 54-year-old woman presenting with swelling of left cheek for seven-years duration. The orthopantomograph revealed a unilocular cystic radiolucency with well-defined margins in left maxillary sinus. In the computerized tomography, the cyst had a sclerotic wall with bony condensations. Aspiration cytology revealed many neutrophil leukocytes. Cyst was drained and enucleated. Histopathologically, it had a fibrous wall with inflammation and focal reactive bone formation and lined by a respiratory-type epithelium. In the clinical history, it is learned that she had a maxillary sinus surgery 8 years ago and the diagnosis was made considering the clinical and histopathological findings.
