We described clinical cases of scleral lenses fitting in patients with induced ametropia and have shown the efficiency of this optical correction. High functional results and comfortable use make scleral lenses the main optical correction method for patients with irregular corneas after refractive surgery procedures.

We described two clinical cases of ortho-k lenses fitting in patients with “non-typical” corneal curvature/diameter ratio. The main goal was to acknowledge effective and safe use of this myopia correction method in patients with corneal diameter greater than 11 mm. Individual approach to each patient is the key to a successful and safe ortho-k lenses fitting.

Modern corneal refractive surgery provides high efficiency, safety, predictability and stability of the different ametropia kinds correction’s clinically-functional results. However, in overwhelming percentage of cases, having spent some time after surgery patients complain about discomfort, eyes dryness, burning, feel gritty, redness, blurred and vision instability. Complaints mentioned above are caused by the appearance of transitory dry eye syndrome. Main etiology and pathogenesis factors of this disease are presented in this review. The results of dry eye syndrome classical diagnosis methods have been analyzed, as well as modern techniques, characterized by high specificity and sensitivity, which allow to increase the dry eye’s diagnostics accuracy. The study of the dry syndrome after corneal refractive surgery is devoted to a huge number of works of domestic and foreign authors. However, at present time there is no unique algorithm for assessing the damage of the ocular surface before and after corneal refractive surgery, which would include a set of high-precision and specific techniques for quickly and reliably evaluate the severity of dry eye syndrome, allowing to develop preventive measures and pathogenetically oriented treatment and, thereby, accelerate rehabilitation of patients after surgery. Research continuation is needed in this direction.

AbstractMalunion or congenital disorders of the forearm might lead to biomechanical alterations and change the pronosupination motion. Occasionally, an osteotomy should be performed to correct this malalignment. Several techniques have been reported, especially 3D modeling, which is not yet widely used due to its cost. The traditional method is still mostly performed, but sometimes it may be difficult to do the preplanning. In this paper, we explain an easy method, called “giro point” technique, to perform the calculation of osteotomies using commercially available software. We present six clinical cases with good functional results, in which the calculation was performed using this method. The method is easy to follow, with adequate surgical-radiological correlation.

Documented experience of treatment of digital nerve lesions with the termino-lateral (end-to-side) nerve suture is limited. Our clinical experience of this technique is detailed here alongside a systematic review of the previous literature. We performed, from 2002 to 2008, seven termino-lateral sutures with epineural window opening for digital nerve lesions. Functional outcome was analysed using the two-point discrimination test and the Semmes–Weinstein monofilament test. The results showed a sensory recovery of S3+ in six cases and S3 in one case. The mean distance found in the two-point discrimination test was 12.7 mm (range 8–18 mm). After a review of the literature, we were able to obtain homogeneous data from 17 additional patients operated by termino-lateral coaptation. The overall number of cases included in our review was 24. A sensory recovery was observed in 23 out of 24 patients. The functional results were S0 in one case, S3 in one case, S3+ in twenty cases and S4 in two cases. Excluding the one unfavourable case, the mean distance in the two-point discrimination test was 9.7 mm (range 3–18 mm). It can thus be concluded that the treatment of digital nerve lesions with termino-lateral suture showed encouraging results. Based on the results obtained in this current study we believe that in case of loss of substance, end-to-side nerve coaptation may be an alternative to biological and synthetic tubulisation when a digital nerve reconstruction by means of nerve autograft is declined by the patient.

ABSTRACT Introduction: Considering the large number of fixed functional appliances, choosing the best device for your patient is not an easy task. Objective: To describe the development of fixed functional appliances as well as our 20-year experience working with them. Methods: Fixed functional appliances are grouped into flexible, rigid and hybrid. They are different appliances, whose action is described here. Four clinical cases will be reported with a view to illustrating the different appliances. Conclusions: Rigid fixed functional appliances provide better skeletal results than flexible and hybrid ones. Flexible and hybrid appliances have similar effects to those produced by Class II elastics. They ultimately correct Class II with dentoalveolar changes. From a biomechanical standpoint, fixed functional appliances are more recommended to treat Class II in dolichofacial patients, in comparison to Class II elastics.

Background: Refractive errors are a major cause of defective vision in all age groups. Although contact lens and refractive surgery are popular, still spectacles are the traditional and most widely used method of optical correction. The main objective of the study was to determine the knowledge and preference for refractive surgery and preference for refractive correction. Materials and Methods: Questionnaire based study was conducted among 252 participants aged between 22-40 years with significant refractive errors (>±1.00D) for a period of three months between June to August 2017. Participants who had prior refractive surgery and not willing to participate were excluded from the study. Results: One hundred and forty (55.6%) were males and majority (76.6%) of the participants were in the age range 22-30 years. Myopia (60%) was more prevalent followed by astigmatism (38%) and hypermetropia (2%) with refractive power between 1-3 D in 72.5% of participants. Only 61(24%) of the participants had knowledge about refractive surgery.  Forty three percentages of them knew from their friends, internet was source of information for 36% and only 19% of them knew from eye care professionals. Majority (87.3%) preferred spectacles 4.8% preferred contact lens and only 8% preferred refractive surgery as methods of refractive correction. Only 44% showed willingness for refractive surgery when available at Bharatpur Eye Hospital. Participants with prior knowledge (p=0.002), higher refractive error and longer duration of glass wear (P=0.005) had more preference for refractive surgery. Conclusion: The knowledge, preference and willingness for refractive surgery were less with spectacle being the most preferred methods of refractive correction.

Podiatric physicians encounter many conditions, especially in sports medicine, that involve pain in the vicinity of the rearfoot or lower leg. These conditions are often associated with ankle equinus and may affect either child or adult sports participants. A review of the literature and clinical experience identify posterior night stretch splinting as an effective adjunct in the treatment of persistent symptomatic plantar fasciitis, negating the need for corticosteroid injections, further protracted pain, or surgery. This article reviews clinical cases in which night stretch splinting was used for a variety of diagnoses. Further research is needed into its efficacy for conditions other than plantar fasciitis.
