Penerapan Terapi Psikologis dalam Menurunkan Tingkat Depresi pada Pasien Hemodialisis: Literature Review

The prevalency of hemodialysis patient with depression was high but only a few of patients who experienced the symptoms of the depression were diagnosed and treated. Therefore, the study and treatment of depression is one of the highest priorities in health care in the hemodialysis unit. Although psychological therapy has been widely implemented in hemodialysis patients who were depressed, the effectiveness from psychological therapy particularly applied to hemodialysis patients in Indonesia has not been certainty. This article was intended to assess the effectiveness of psychological therapy on a decline in depression levels in hemodialysis patients. The study of the article was a literature study obtained through data retrieval on four online databases of Pubmed, Ebsco, Cocharane and Scopus with the inclusion criteria have randomized control trial (RCT) designs, full text, published in 2015-2020, and in English. In the search, there were 219 articles found later in the review on abstract according to the research purpose that followed 8 selected articles after quality assessment using Critical Appraisal Skills Programme guide. The 4 types of psychological therapy were cognitive behavioral therapy, psychoeducation, hemodialysis self-management and hope therapy are potentially applied in Indonesia. These modifications and combinations of interventions could be suggested as to further research to improve th effectivities of phisicological therapy on hemodyalisis patients who were depressed. Oleh karena itu, kajian dan penanganan terhadap depresi merupakan salah satu prioritas utama dalam pelayanan kesehatan di unit hemodialisis. Meskipun terapi psikologis telah banyak digunakan pada pasien hemodialisis yang mengalami depresi, tetapi belum ada kepastian efektifitas dari terapi psikologis terutama untuk bisa diterapkan pada pasien hemodialisis di Indonesia. Artikel ini bertujuan untuk menilai efektifitas terapi psikologis terhadap penurunan tingkat depresi pada pasien hemodialisis. Penelitian artikel ini mengunakan kajian literatur yang diperoleh melalui pencarian pada 4 database online yaitu Pubmed, EBSCO, Cocharane dan Scopus dengan kriteria inklusi memiliki desain Randomized Control Trial (RCT), full text, dipublikasi tahun 2015-2020, dan berbahasa Inggris. Dalam pencarian ditemukan ada 219 artikel, kemudian di telaah yang sesuai dengan tujuan penelitian dan selanjutnya diperoleh 8 artikel terpilih setelah dilakukan penilain kualitas dengan menggunakan panduan Critical Appraisal Skills Programme (CASP). 4 jenis terapi psikologis yaitu cognitive behavioral therapy, psychoeducation, hemodialysis self- management dan hope therapy berpotensi untuk diterapkan di Indonesia. Modifikasi dan kombinasi dari berbagai intervensi ini disarankan untuk dilakukan pada penelitian selanjutnya untuk lebih meningkatkan efektivitas terapi psikologis pada pasien hemodialisis yang mengalami depresi. Adolescence is a critical time for physical development and maturation, and with these important physiological changes comes greater awareness of body image and appearance, which, for a proportion of young people can become excessive, signaling the onset of body dysmorphic disorder (BDD). BDD in adolescence is associated with significant impairment and suicidality, is poorly understood, and currently there is limited evidence for the effectiveness of psychological therapy. Cognitive behavioral therapy (CBT) is currently the most promising and best available psychological therapy for BDD in youth. The aim of this article is to provide clinicians with information on CBT treatment for BDD in young people and to provide guidance based on clinical experience of working with this complex population group. The article will include discussion of strategies including maximizing psychoeducation with parental involvement, the use of cognitive therapy techniques, exposure with response prevention, and perceptual visual training techniques, including attention training and mirror retraining.

Background: Metabolic syndrome is an asymptomatic disorder and an important risk factor for cardiovascular disease and type 2 diabetes. Lifestyle modification and self-management of health-promoting behaviors are the most important actions to control metabolic syndrome. This study was done to investigate the effectiveness of self-management interventions based on group Cognitive-Behavioral Therapy (CBT) on lifestyle among adults with metabolic syndrome. Methods: This research was a randomized clinical trial study. First, 80 adults with metabolic syndrome were selected using the convenience sampling method, and then, using the randomized blockchain method, they were divided into two groups of 40 people (intervention and control groups). Data collection using the Health Promotion Lifestyle Profile II Questionnaire (HPLP II) was performed in two stages before and three months after the intervention. Teaching self-management interventions based on cognitive-behavioral therapy was done twice a week for eight 90-minute sessions. Data were analyzed using the Chi-square test, independent t-tests, and Analysis of Covariance (ANCOVA). A P-value less than 0.05 was considered significant. Results: the results of ANCOVA showed that self-management intervention based on group CBT had a significant effect on the dimensions of lifestyle, including exercise and physical activity with an effect size of 0.51, nutrition with 0.49, health responsibility with 0.39, and stress management with 0.32 (P<0.05), but it did not significantly affect the two dimensions of self-actualization and spiritual growth (effect size= 0.003, P=0.669) and interpersonal relationships (effect size= 0.029, P=0.151). Conclusion: Self-management interventions based on group CBT can improve lifestyle and some dimensions in adults with metabolic syndrome. Identifying people with metabolic syndrome and performing effective interventions with the active participation of the individual through self-management based on cognitive-behavioral therapy seem necessary.

BACKGROUND Sickle cell disease is an inherited blood disorder that affects over 100,000 Americans. Sickle cell disease–related complications lead to significant morbidity and early death. Evidence supporting the feasibility, acceptability, and efficacy of self-management electronic health (eHealth) interventions in chronic diseases is growing; however, the evidence is unclear in sickle cell disease. OBJECTIVE We systematically evaluated the most recent evidence in the literature to (1) review the different types of technological tools used for self-management of sickle cell disease, (2) discover and describe what self-management activities these tools were used for, and (3) assess the efficacy of these technologies in self-management. METHODS We reviewed literature published between 1995 and 2016 with no language limits. We searched MEDLINE, EMBASE, CINAHL, PsycINFO, and other sources. We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. Two independent reviewers screened titles and abstracts, assessed full-text articles, and extracted data from articles that met inclusion criteria. Eligible studies were original research articles that included texting, mobile phone–based apps, or other eHealth interventions designed to improve self-management in pediatric and adult patients with sickle cell disease. RESULTS Of 1680 citations, 16 articles met all predefined criteria with a total of 747 study participants. Interventions were text messaging (4/16, 25%), native mobile apps (3/16, 19%), Web-based apps (5/16, 31%), mobile directly observed therapy (2/16, 13%), internet-delivered cognitive behavioral therapy (2/16, 13%), electronic pill bottle (1/16, 6%), or interactive gamification (2/16, 13%). Interventions targeted monitoring or improvement of medication adherence (5/16, 31%); self-management, pain reporting, and symptom reporting (7/16, 44%); stress, coping, sleep, and daily activities reporting (4/16, 25%); cognitive training for memory (1/16, 6%); sickle cell disease and reproductive health knowledge (5/16, 31%); cognitive behavioral therapy (2/16, 13%); and guided relaxation interventions (1/16, 6%). Most studies (11/16, 69%) included older children or adolescents (mean or median age 10-17 years; 11/16, 69%) and 5 included young adults (≥18 years old) (5/16, 31%). Sample size ranged from 11 to 236, with a median of 21 per study: <20 in 6 (38%), ≥20 to <50 in 6 (38%), and >50 participants in 4 studies (25%). Most reported improvement in self-management–related outcomes (15/16, 94%), as well as high satisfaction and acceptability of different study interventions (10/16, 63%). CONCLUSIONS Our systematic review identified eHealth interventions measuring a variety of outcomes, which showed improvement in multiple components of self-management of sickle cell disease. Despite the promising feasibility and acceptability of eHealth interventions in improving self-management of sickle cell disease, the evidence overall is modest. Future eHealth intervention studies are needed to evaluate their efficacy, effectiveness, and cost effectiveness in promoting self-management in patients with sickle cell disease using rigorous methods and theoretical frameworks with clearly defined clinical outcomes.
