Fecaloma is a mass of hardened faeces difficult to pass through the intestinal lumen. Due to various reasons, a bolus of fecal matter stagnates, accumulates and inspisates in the intestinal lumen, until it becomes big enough to cause intestinal obstruction. Though there are various predisposing factors for the formation of fecaloma but the actual cause of fecaloma formation is still unknown. Small gut fecalomas are very rare. Herein we are presenting a case of 50 year male who underwent laparotomy three times for small gut obstruction caused due to recurrent formation of small gut fecaloma. To our knowledge it is the first case report of recurrent small gut fecaloma obstruction to be reported.

A 60-year-old woman was presented in emergency department with abdominal pain and vomiting for 1 day. She was known to have seropositive rheumatoid arthritis. Importantly, she was treated surgically for complex jejunal atresia and duplication surgery, when she was 6 days old. CT scan showed intestinal obstruction secondary to intussception. Patient had lapartomy and operative findings revealed side-to-side anastomosis with gut duplication 10 cm distal to duodenal jejunum junction (due to her previous surgery during infancy). Redundant part of the loop dilated up to 300 mL with large hard stone (4×3.5 cm) was excised. Patient recovered well postoperatively and was discharged to go home. To our knowledge, this is the first case report to show formation of large stone 60 years after surgical treatment of complex jejunal atresia and duplication surgery in infancy.

AbstractCoagulase-negative Staphylococcus warneri is a rare cause of human disease, which as a rule manifests as an insidious and protracted infection of various prostheses and endovascular catheters. A case of Staphylococcus warneri sepsis with multiple abscesses in an immunocompetent patient free from the usual predisposing factors for coagulase-negative staphylococcal infection is presented. To our knowledge, this is the first case of sepsis with multiple abscesess caused by this bacterium reported in the literature.

Ileal Atresia is noted to be the commonest cause of intestinal obstruction in neonates worldwide but still under diagnosed in Sub-Saharan countries with scarce data reported on its occurrence. It is likely under diagnosed due to low index of suspicion. Advancement in foetal ultrasound during prenatal period will increase index of suspicion and hence early diagnosis and correction. This is the first case report in our setup highlighting this condition. We present a case of a 10-day-old male baby referred to us due to vomiting since birth. Clinically was in distress with a palpable supraumbilical mass that was firm and non-tender. Abdominal ultrasound scan showed poor peristalsis and minimal dilation of bowels suggestive of partial intestinal obstruction. Abdominal X-ray showed dilated bowels with multiple air-fluid levels, empty rectum with features suggestive of intestinal obstruction. Patient was successfully operated but unfortunately on day 3 post-surgery succumbed.

This is the first case report from Scandinavia of a pyogenic liver abscess caused by a Klebsiella pneumoniae isolate belonging to the international hyper virulent clone ST23. The patient, an 85-year old Caucasian, had no history of foreign travel or any classical predisposing factors for infection. The isolate was hypermucoviscous of capsular serotype K1 and carried the virulence factors aerobactin, allS, kfu and rmpA.

Cryptorchidism as a cause of bowel obstruction is very unsuspecting. Bowel obstructions in cases of cryptorchidism, though reported were mostly as a complication of malignant transformation. Only two case reports of cryptorchidism document a non-malignant aetiology for bowel obstruction. This case is of a 16 year old boy referred for pain abdomen and obstipation for last 3 days. He had signs of peritonitis on examination and so, urgent exploratory laparotomy was planned. A loop of jejunum of approx. 30 cm length at a distance of 35 cm from duodeno-jejunal junction was found gangrenous with gubernacular bands around the loop emerging from a left cryptorchid testis. Resection– anastomosis was done along with left orchidectomy. Histopathology revealed an atrophied testis with no malignant changes. This is the first case report of a non-malignant cryptorchid testis as a cause of strangulated intestinal obstruction in literature.
