Ileal Atresia is noted to be the commonest cause of intestinal obstruction in neonates worldwide but still under diagnosed in Sub-Saharan countries with scarce data reported on its occurrence. It is likely under diagnosed due to low index of suspicion. Advancement in foetal ultrasound during prenatal period will increase index of suspicion and hence early diagnosis and correction. This is the first case report in our setup highlighting this condition. We present a case of a 10-day-old male baby referred to us due to vomiting since birth. Clinically was in distress with a palpable supraumbilical mass that was firm and non-tender. Abdominal ultrasound scan showed poor peristalsis and minimal dilation of bowels suggestive of partial intestinal obstruction. Abdominal X-ray showed dilated bowels with multiple air-fluid levels, empty rectum with features suggestive of intestinal obstruction. Patient was successfully operated but unfortunately on day 3 post-surgery succumbed.

We report an individual with rapidly progressive motor neuron disease (MND), phenotypically compatible with amyotrophic lateral sclerosis (ALS). The patient described in this case report proved positive for human immunodeficiency virus (HIV) and was initiated on antiretroviral therapy (ART). Following ART he clinically stabilised over 10 years and deteriorated again due to noncompliance or ART resistance. HIV infection can give rise to an MND mimic, HIV-ALS. The improvement in response to ART supports the notion that HIV-ALS is a treatable entity also in Africa. This is the first case report of a patient with HIV-ALS and long term follow up in Sub-Saharan Africa. The report raises the suggestion that an additional (retro)virus can play a role in the aetiology of ALS.

Fecaloma is a mass of hardened faeces difficult to pass through the intestinal lumen. Due to various reasons, a bolus of fecal matter stagnates, accumulates and inspisates in the intestinal lumen, until it becomes big enough to cause intestinal obstruction. Though there are various predisposing factors for the formation of fecaloma but the actual cause of fecaloma formation is still unknown. Small gut fecalomas are very rare. Herein we are presenting a case of 50 year male who underwent laparotomy three times for small gut obstruction caused due to recurrent formation of small gut fecaloma. To our knowledge it is the first case report of recurrent small gut fecaloma obstruction to be reported.

We report the first case of isolated Cor triatriatum sinister in a 36-year-old male with metabolic syndrome in sub-Saharan Africa. It is a rare congenital heart disease that consists of a fibromuscular ring that divides the atrium into two chambers. It can mimic mitral valve stenosis.

Malignant melanoma of the oral cavity is an exceedingly rare tumor representing 0.2 to 8% of all melanomas. Mucosal melanomas are extremely rare and aggressive neoplasms. Patient reporting to the clinician with a pigmented lesion should raise suspicion in the first visit itself and should be further investigated so as to detect this dreaded malignancy at an earlier stage and thus managed appropriately. We presented two such rare cases who reported at our centre treated with different modalities and had different responses to treatment. In first case report 65 year old male patient diagnosed with malignant melanoma of left upper alveolus underwent multiple modalities of treatment like surgery, chemotherapy, radiation therapy but unfortunately as he defaulted post-surgery and also due to COVID-19 lockdown restrictions he was treated in various centres and finally the result was inoperable residual gigantic mass resistant to chemotherapy and radiation therapy. In second case report, 82 year old male patient reported with malignant melanoma hard palate having good response to initial chemotherapy. He was planned on hypo-fractionated radiotherapy in view of his old age but he refused radiation treatment and is on oral temozolamide and thalidomide with stable disease and good quality of life since past 6 months.

A thirteen-year-old, neutered, female Domestic Shorthair (DSH) was referred to the hospital with a suspicion of traumatic uroabdomen. Abdominal ultrasound revealed free peritoneal fluid and cellular infiltration of the urethra, kidneys, abdominal lymph nodes and cecal wall. Analysis of the free fluid confirmed the presence of uroabdomen. Cytology of the infiltrative lesions in the kidneys, urethra and lymph nodes confirmed the presence of multicentric malignant lymphoma. To the authors’ knowledge, this is the first case report of lymphomatous infiltration of the urethra in a cat.
