Previous studies reported that 20–30% of COVID-19 patients will develop delirium during the hospitalization, achieving 70% in cases of severe illness. The risks factors and the consequences of delirium are well-documented in the literature; however, little is known about the personal experience of delirium. Delirium burden is common and tends to be distressing even after the delirium episode has resolved. Taking this in mind, the present work provides a first-person account of a doctor who acquired Covid-19 and developed bilateral pneumonia and had delirium and a complicate course of illness. During the course of his delirium, the patient recalled experiences of reality and unreality, complete disorientation, lack of control, strong emotions, and intense fear of dying which was significantly distressing. We anticipate that delirium burden will be common on these patients and family members and clinicians should be aware of this phenomenon in order to evaluate the neuropsychiatric consequences of this condition.

Wegner's analysis of the illusion of conscious will is close to my own account of how conscious experiences relate to brain processes. But our analyses differ somewhat on how conscious will is not an illusion. Wegner argues that once conscious will arises it enters causally into subsequent mental processing. I argue that while his causal story is accurate, it remains a first-person story. Conscious free will is not an illusion in the sense that this first-person story is compatible with and complementary to a third-person account of voluntary processing in the mind/brain.

Another novelist provides in some respects a point in between Chariton and Heliodorus. His elaborate expatiation on tears and the lover put rhythm at the service of an intricate treatment of the mind and body, and a shrewd depiction of amorous self-control and manipulation. The first-person narrative adds a further stratum of sophistication to this handling of the speaker’s rival and enemy. Achilles Tatius demonstrates further, in contrast with Chariton, the range of possibilities for the exploitation of rhythm seen already in the difference of Chariton and Plutarch. Comparison with Heliodorus brings out Achilles’ elegance.

Like a shining shadow, Louis Kahn's poetry is found in all his works, both built or just designed. In his conferences, Kahn formulates thoughts that often touch on notes of poetry, the sacred, or a mystic science of cosmos where the architect becomes a narrator of our origins. It is not, however, a question of simple cultural ornament, or a search for consensus, but a free inner necessity which produces expression, creation and design, and which translates into categories that literally materialise as architectural forms. ‘The Room', a 1971 sketch which shows the mind of this great ‘Philosopher by nature' like a stage, is an emblem of the original node that joins matter and spirit, which binds architecture and thought, architecture and archetype, well beyond the limits of Louis Kahn's personal experience.

As we doctors are beginning to understand more and more about dementia, the public has become increasingly aware of the condition and in turn this has been reflected in the arts. This article discusses four books whose main focus is the experience of dementia, each written from an entirely different perspective: a novel giving a first-person account of dementia by the Dutch writer J. Bernlef; a biography of the famous novelist Iris Murdoch by her husband John Bayley; Linda Grant's account of her mother's multi-infarct dementia (which also describes Jewish migration to the UK two generations ago); and Michael Igniateff's autobiographical novel Scar Tissue. Such accounts, offering insights into how patients and carers feel, cannot but help make us better doctors.

The main aim of this paper is to support a metaphilosophical and metascientific approach to the study of Consciousness.After a brief historical resume of the debate between the mind-brain-body relationship, we discuss how the apparently irreducible contention between a physicalist and an anti-physicalist interpretation of Consciousness can be overcome by a metaphilosophic and metascientific approach in the attempt to overcome ethnocentric cultural filters and constraints yielded by the Weltanschauung and the Zeitgeist one belongs to. IN fact, a metaphilosophical perspective can help to recognize key concepts and meanings common to different philosophies beyond their formal differences and different modes of theorization, where the common field of reflection is aimed to find the problem’s unity in the multiplicity of forms. Likewise, the metascientific approach, such as the anthropic principle adopted in astrophysics, helps overcoming the problems of indecidability of single axiomatic disciplines.A metaphilosophical and metascientific approach seems appropriate in the study of consciousness and subjective phenomena, since the first-person perspective and the meaning of the experience are the condition sine qua non for their proper understanding.

In lieu of an abstract, here is a brief excerpt of the content: THE TRAVELS OF Sir John Mandeville, the fourteenth-century "first-person" account of a fictional English knight's adventurous journey to Jerusalem and across the world, is difficult to teach.1 Popular with medieval European audiences, the book troubles today's students with its confusing descriptions of global geography, its treatment of non-Christian, non-European peoples, and its constant conflation of fact and fable. But, as those who have taught it can attest, it can serve as a valuable tool for challenging students' preconceptions of an isolated European Middle Ages. It introduces them to an unreliable narrator and to tensions between the doctrines of the institutional Roman church and individual faith. The author's global perspective shows students a world of diverse religions, ethnicities, races, diets, customs, and sexualities. And the Travels does this while being relatively short and entertaining, pulling the reader through the map via its engaging narrative of landscaped vignettes.
