This study aimed to assess the ability of dental students and recent graduates to detect and recognize mucosal changes that are suggestive of oral cancer and potentially malignant disorders. In this cross-sectional study, a questionnaire was administered to dental students and recent graduates of Ajman University (n = 132). Completed questionnaires were received from 84 (63.6%) females and 48 (36.4%) males which included fifth-year students (n = 80), interns (n = 39), and dental practitioners (n = 13). This questionnaire was designed to assess the respondent’s ability to detect and recognize different types of oral lesions, as well as their knowledge of oral cancer appearance and malignancy potential. The overall accuracy of diagnosis was 46%. The participants correctly identified normal variations, benign tumors, malignant tumors, and premalignant lesions at rates of 60.3%, 31.0%, 55.7%, and 33.4%, respectively. There was no significant difference between the two genders in their ability to recognize and detect correct answers (females, 48.3%; males, 47.2%). According to education level, interns provided the highest percentage of correct answers (52.5%), followed by newly dental practitioners (51.9%) and fifth-year students (44.1%). Conclusion: The respondents of this survey did not exhibit a satisfactory diagnostic capability in recognizing mucosal changes consistent with the clinical presentation of oral cancer. Thus, a need exists for improved and updated educational methods for undergraduate students regarding oral cancer and potentially malignant disorders. Meanwhile, practitioners should look for oral abnormalities to provide better diagnosis and management. Practitioners should also stay up to date on the oral malignancy topic by attending workshops and clinicopathological conferences.

The importance of detecting oral malignant disease is paramount. Recent government strategies have placed an emphasis on the early detection of cancer. This paper presents a brief account of the current debate around the role of oral cancer screening and provides a summary of the results of three practice-based studies that were supported by the British Society for General Dental Surgery and the Faculty of General Dental Practice (UK). Overall, there appears to be a lack of rigour among some general dental practitioners when screening for potentially malignant disorders and their incidence in dental practice is perceived to be low. Barriers remain to the implementation of smoking-cessation schemes and many general dental practitioners feel out of their depth in tackling the abuse of alcohol, despite recognising its importance as a risk factor. Patients continue to present to their general medical practitioner (GMP) when they are concerned about something in their mouth that is not related to their teeth. GMPs’ ability to manage early oral cancer is discussed. A prospective randomised controlled trial (RCT), or a demonstration study, is recommended to investigate the numbers of cases of oral cancer diagnosed in primary dental care and primary medical care and to develop standardised referral criteria for early cases of this disease to reduce under- and over-referral. The RCT could also examine the potential of using auxiliary healthcare workers and examine the impact of such a programme upon secondary care services.

Oral cancer has been recognized as a significant challenge to healthcare. In Malaysia, numerous patients frequently present with later stages of cancers to the highly subsidized public healthcare facilities. Such a trend contributes to a substantial social and economic burden. This study aims to determine the cost of treating oral potentially malignant disorders (OPMD) and oral cancer from a public healthcare provider’s perspective. Medical records from two tertiary public hospitals were systematically abstracted to identify events and resources consumed retrospectively from August 2019 to January 2020. The cost accrued was used to estimate annual initial and maintenance costs via two different methods- inverse probability weighting (IPW) and unweighted average. A total of 86 OPMD and 148 oral cancer cases were included. The initial phase mean unadjusted cost was USD 2,861 (SD = 2,548) in OPMD and USD 38,762 (SD = 12,770) for the treatment of cancer. Further annual estimate of initial phase cost based on IPW method for OPMD, early and late-stage cancer was USD 3,561 (SD = 4,154), USD 32,530 (SD = 12,658) and USD 44,304 (SD = 16,240) respectively. Overall cost of late-stage cancer was significantly higher than early-stage by USD 11,740; 95% CI [6,853 to 16,695]; p< 0.001. Higher surgical care and personnel cost predominantly contributed to the larger expenditure. In contrast, no significant difference was identified between both cancer stages in the maintenance phase, USD 700; 95% CI [-1,142 to 2,541]; p = 0.457. A crude comparison of IPW estimate with unweighted average displayed a significant difference in the initial phase, with the latter being continuously higher across all groups. IPW method was shown to be able to use data more efficiently by adjusting cost according to survival and follow-up. While cost is not a primary consideration in treatment recommendations, our analysis demonstrates the potential economic benefit of investing in preventive medicine and early detection.

One of the global health issues is Oral cancer, which has an increased death rate. Awareness of oral cancer screening is important for oral health care professionals for early detection and improving the patients' survival rate. Attitude towards oral cancer awareness and knowledge of oral cancer screening are the key factors that impact oral cancer awareness success. Materials and Methods: To assess the knowledge, attitude, and practice of dental practitioners and dental students towards screening for oral pre-cancer and cancer. This study included dental practitioners working in various academic institutions and private practitioners in Chennai. Participants were asked to fill in age, gender, designation, and experience. A structured questionnaire consisting of 10 questions, each having four options, was administered to the participants. The data collected was 100 responses analyzed by statistical package for the social science (spss inc., version 16 for, Chicago, IL, USA). Simple descriptive statistics were used to describe the distribution of data collected. T-test and ANOVA were employed to assess the statistically significant difference in the KAP scores between different groups. Results: The response rate for the present study was 100%. 37% of general dental practitioners reported that they routinely do a thorough oral examination for all the patients,52% agree that they are adequately trained to examine patients for oral pre-cancer and cancer screening,8% of them reported that they never do toluidine blue staining for patients with oral lesion. Majority(57) of them agree that they would recommend biopsy for suspected cases of oral cancer. Only 9% of the participants were aware of the time required for intraoral screening examination. 31% of them were aware of the risk of malignant transformation in a white lesion. 73% of the participants were aware of the occurrence of oral cancer in non - tobacco users was rare. 53% of agree that thorough intraoral examination for oral cancer will reduce oral cancer mortality and morbidity.

ABSTRACT Oral squamous cell carcinoma is the most common malignancy of the oral cavity, which is usually preceded by a myriad of oral potentially malignant disorders (OPMDs). In the classification of OPMDs, inherited cancer syndromes (ICSs) were proposed as one of the categories. Inherited cancer syndromes are genetic disorders in which inherited genetic mutation in one or more genes predispose the affected individuals to the development of cancer and may also cause its early onset. Many of these syndromes are caused by mutations in tumor suppressor genes, oncogenes, and genes involved in angiogenesis. General dental practitioners frequently come across OPMDs in their day-to-day practice. It becomes of paramount importance to have knowledge about these rare but prognostically important OPMDs. With this view in mind, in this article, efforts have been made to comprehensively discuss about various ICSs that have higher potential of transformation into oral cancer. The ICSs discussed in this article are xeroderma pigmentosum (XP), ataxia telangiectasia (AT), Bloom syndrome (BS), Fanconi's anemia (FA), and Li–Fraumeni syndrome (LFS), with special emphasis on signs, symptoms, and genetic considerations. World Health Organization (WHO) defined the terminology ‘Oral Potentially malignant disorders’ (OPMD) as the presence of risk of malignancy in a lesion or condition either during the time of initial diagnosis or at a future date with the commonly accepted prevalence of 1–5%. All OPMDs may not transform into malignancy, many factors have been explored which effectively assess the risk of malignant transformation in OPMDs including many clinical, pathological and molecular factors. Qualitative & experimental factors of different cells in cytological preparations and/or biopsy specimens are reliable parameters for pathologists. The analysis and assessment of histological units can be enhanced by image analysis assisted by a computer that can be used for statistical comparisons. Fractal geometry is considered to be an ideal method of image analysis in quantitative microscopy & histopathology. Fractal dimension analysis is not only limited to determining cell and tumour types but can also be used for determining cellular behaviours in vitro such as cell migration, apoptosis and cellular differentiation, which can be a useful characterization of oral cancer lines and further help in the treatment planning. Background: Oral Cancer (OCA) is a common malignancy in South East Asia and the burden of the disease is increasing. OCA is usually preceded by Potentially Malignant Oral Disorder (PMOD). The early detection of these diseases may limit the high mortality and morbidity. The objectives of this study were to assess the awareness of burden, associated risk fac­tors, early detection and prognosis of OCA and OPMD among final year medical and dental students.. Methods: This cross sectional study was conducted among final year Medical and Dental undergraduate students of Chitwan Medical College, Nepal in April 2017 using a pre tested self-administered questionnaires. All the final year students present at their respective classes (Dental= 50 and Medical=75 students) responded to the questionnaires after giving informed consent. Data entered in SPPS 20 software was analyzed using Pearson Chi Square test and Fisher exact test. Results: Dental students had better awareness concerning PMOD. Yet, the dental students did not differ significantly from their medical coun­terpart in most of the responses related to OCA. Surprisingly, more Dental students had misconception that OCA is not preventable and early detec­tion is difficult. Few of both groups were confident in having sufficient knowledge in OCA prevention and detection. Only 18.6% of Medical stu­dents had examined PMOD and few students of both groups had exam­ined OCA. Conclusions: This study revealed the deficient aspect in the knowledge concerning OCA and OPMD among Medical and Dental students indicat­ing the need to emphasize on these aspects of medical education.
