Chronic Patients’ Satisfaction with the Medical Aid

Population’s satisfaction with medical care has recently become an important structural component of the comprehensive assessment of the healthcare activities. The satisfaction is subjectively determined which makes it possible to assess the level of the population’s satisfaction with medical care. The sociological survey is recognised as the most informative method of study. The article provides the results of study of satisfaction with medical care of the respondents suffering from chronic diseases and followed up at the medical institutions of Saratov that has been conducted by the authors.

Insufficient availability of emergency medical services to the rural population is noted. The dynamics of the growth of calls to emergency medical services testifies to the fact that emergency medical institutions have taken over part of inpatient services for the provision of emergency care to patients with chronic diseases and acute colds.

Abstract Background: The French healthcare system is evolving to meet the challenges of an aging population, the growing prevalence of chronic diseases, the development of new technologies and the increasing involvement of patients in the management of their disease. The aim of this study is to assess the satisfaction and priorities of chronic patients regarding medical care, information and services and their quality of life.Methods: A cross-sectional study was conducted from February to March 2018 via the French Carenity platform. Adult patients enrolled in type 1 diabetes, heart failure or obesity communities were invited to answer an online questionnaire. A numeric scale from 0 (meaning not satisfied) to 5 was used to evaluate patients’ satisfaction. Patients’ priorities were assessed using a ranking question. Patients’ satisfaction and priorities have been combined in a matrix to identify patients’ expectations.Results: 67 respondents of each condition answered a questionnaire. The most important and least satisfactory items about medical care are availability and active listening from healthcare providers, as well as access to coordinated and multidisciplinary care. Regarding information and services, respondents mostly expect connected medical devices, in addition to lifestyle and dietary measures. As for the quality of life, respondents fear that their chronic condition will keep impacting their daily mood and ability to do physical activities.Conclusions: This study shows that chronic patients want to be more actively involved in their care pathway. Patient training and therapeutic patient education programs could help them manage their chronic conditions within a patient-centred healthcare system.

The aim of the study is to analyze medical aid given to the workers of ferrous metallurgy enterprises in the first half of the 1950s. Primary medical unit provided medical aid for workers of the ferrous metallurgical enterprises. Because of the lack of medical unit at the metallurgical plant or because there was no medical institutions in the organizational structure of the medical unit, local medical institutions provided the workers with medical care. During the first half of the 1950s, was the problem of the lack of medical staff in the medical institutions caused by insufficient material and household support. It did not allow implementing the shop principle of medical service of metallurgists. At the enterprises, individual shops did not have a factory’s sectorial doctor, or the number of workers assigned to a factory’s sectorial doctor exceeded the norm. Because the local hospitals, which controlled the health post at the ferrous metallurgy enterprises of the Zaporizhzhya region, were placed at a considerable distance, ‘the shop principle’ was not implemented there at all. The personnel problem also affected the formation of queues, which were a characteristic feature of the medical care provided to the workers. Work overload of physicians led to a quick examination of the patients, resulting in mistakes in the diagnosis. Contrary to accepted laws aimed at improving the quality of diagnosis, the periodic medical examinations of workers were not always carried out qualitatively, and the medical examination was carried out in a number of plants partly. In the first half of the 1950s the enterprises of the ferrous metallurgy of Zaporizhzhya region had a problem that the medical institutions were on a far distance and the percentage of patients needed medical help reduced. Lack of hospital beds had caused the untimely hospitalization of patients that prevented recovery of their. Many of the medical institutions were located in the old or non-specialized buildings with limited space, stove heating and primitive food units, without household premises and sewage. Characteristic of the medical services of metallurgists were depreciation of the material fund of medical institutions, lack of medicines, medical equipment. Not all medical institutions were provided with ambulances. There were cases of refusal to leave for a challenge or untimely provision of emergency care. It was concluded that the level of medical services given to the metallurgists in the first half of the 1950s was insufficient, despite the fact that they were workers of one of the leading branches of the Soviet economy.

The initiation of the common register of all the patients who suffers from chronic diseases and is also observed by specialized clinics for all the city out-patient clinics and the emergency stations will significantly optimize the performance of both the emergency service and the network of out-patient clinics. The initiation of the common register will allow to hand the part of emergency calls from patients observed by a specialized clinic over to the emergency medical aid services of local policlinics. Owing to the information concerning patients presented in the register both the diagnostics quality during the pre-hospital period carried out by the emergency specialists and the performance of the adequate qualified medical care of the patients observed by a specialized clinic.

Introduction. The purpose of our work was to justify the need to improve the legislative, regulatory and methodological framework and preventative measures in relation to the spread of parasitic infections in the provision of medical care. There is a wide range of pathogens of parasitic infestations that are transmitted to humans through various medical manipulations and interventions carried out in various medical institutions. Contaminated care items and furnishings, medical instruments and equipment, solutions for infusion therapy, medical personnel’s clothing and hands, reusable medical products, drinking water, bedding, suture and dressing materials can serve as a major factor in the spread of parasitic infections in the provision of medical care. Purpose of research is the study of the structure and SMP of parasitic origin, circulating on the objects of the production environment in multi-profile medical and preventive institutions of stationary type in order to prevent the occurrence of their spread within medical institutions. Material and methods. The material for the study was flushes taken from the production environment in 3 multi-profile treatment and prevention institutions of inpatient type: a multi-specialty hospital, a maternity hospital and a hospital specializing in the treatment of patients with intestinal diseases for the eggs of worms and cysts of pathogenic protozoa. Results. During the 2-year monitoring of medical preventive institutions, a landscape of parasitic contamination was found to be obtained from the flushes taken from the production environment objects in the premises surveyed as part of the research work. Discussions. In the course of research, the risk of developing ISMP of parasitic origin was found to be determined by the degree of epidemiological safety of the hospital environment, the number and invasiveness of treatment and diagnostic manipulations and various medical technologies. Conclusion. It is necessary to conduct an expert assessment of regulatory and methodological documents in the field of epidemiological surveillance and sanitary and hygienic measures for the prevention of medical aid related infections of parasitic origin, to optimize the regulatory and methodological base, to develop a number of preventive measures aimed at stopping the spread of parasitic infections in the medical network.

Development of health care of Siberia in the 18<sup>th</sup>&nbsp;— first half of the 19<sup>th</sup> century was closely connected with economic and political development of the Russian state. Before the medical personnel emergence in Siberia its population received medication only in the form of folk remedies. Development of its territory, insanitary conditions of life of Siberians, constant contingent of exiled to the indigenous people, and severe climate contributed to the spread of such diseases as smallpox, syphilis, and others.<br> The first medical institutions of Siberia were military hospital and infirmaries. In the days of reign of the Empress Anna Ioannovna, the city medical care started developing, and thanks to an initiative of the industrial enterprises owners&nbsp;— medical care for miners. After the establishment of the Public Charity Orders in Siberia, hospitals under their jurisdiction began to function in Tobolsk, Irkutsk, and Tomsk. A worthy contribution to the development of local medical institutions was made by donators Tolstopyatov, Bednyagin, Chupalov, and others. However, deficiency of financing, hospitals, and the medical personnel couldn’t adequately satisfy the needs of Siberian inhabitants in medical care.<br> The purpose of the article is to analyse the condition of public health, causes of morbidity and mortality of the population, as well as the process of medical care development in Siberia the turn of the 18<sup>th</sup> century by means of a historical and genetic method.

Effectively improving the medication adherence of patients is crucial. Past studies focused on treatment-related factors, but little attention has been paid to factors concerning human beliefs such as trust or self-efficacy. The purpose of this study is to explore the following aspects of patients with chronic diseases: (1) The relationship between emotional support, informational support, self-efficacy, and trust; (2) the relationship between self-efficacy, trust, and medication adherence; and, (3) whether chronic patients’ participation in different types of online communities brings about significant statistical differences in the relationships between the abovementioned variables. A questionnaire survey was conducted in this study, with 452 valid questionnaires collected from chronic patients previously participating in online community activities. Partial Least Squares-Structural Equation Modeling analysis showed that emotional support and informational support positively predict self-efficacy and trust, respectively, and consequently, self-efficacy and trust positively predict medication adherence. In addition, three relationships including the influence of emotional support on trust, the influence of trust on medication adherence, and the influence of self-efficacy on medication adherence, the types of online communities result in significant statistical differences. Based on the findings, this research suggests healthcare professionals can enhance patients’ self-efficacy in self-care by providing necessary health information via face-to-face or online communities, and assuring patients of demonstrable support. As such, patients’ levels of trust in healthcare professionals can be established, which in turn improves their medication adherence.
