Objectives - to develop the main strategies of emergency medical care improvement for patients with acute coronary syndrome, in order to to reduce the morbidity and mortality in the population of a large industrial region. Material and methods. The study was conducted on the basis of the Moscow regional ambulance station. We used the content analysis, logical, system analysis, the method of mathematical statistics. Results. In the period 2017-2018, in the region's population, there is a tendency for decrease in the number of calls for ambulance teams caused by acute coronary syndrome. During the analyzed period, we noticed the reduction of the number of cases of pre-hospital thrombolytic therapy (by 17.5%). The response time to the patient's call not more than 20 minutes was observed in 92.9% of cases, the number of lethal outcomes in presence of the ambulance team decreased by 4. 6%. The results can be explained by the dynamic development of the infrastructure of cities and districts of the Moscow Region, by the improvement of road networks, by the complete renewal of the service's motor vehicle fleet, and by the opening of a number of new specialized medical institutions. A set of medical and organizational measures has been developed and put into practice in the ambulance service of the region, aimed at further improvement of the system of ambulance care for patients with acute coronary syndrome at the prehospital stage. Conclusion. The mortality reduction, preservation and improvement of public health can be achieved only through the comprehensive sustainable development of the region’s infrastructure, improvement of the organizational structure of the emergency medical service, mandatory use of algorithms and standards for the provision of medical care to patients at all stages.

The aim of our research was to study the structure of patients with acute coronary syndrome and determine the organizational components of medical care for patients with suspected acute coronary syndrome in Poltava region. Material and methods: For our study was on the one hand statistical reports of health care institutions for 2016-2018, and on the other – a survey of doctors and paramedics of emergency medical care teams on tactics management of patients with AMI. The methodological apparatus used to achieve the set goals was based on the determination of relative values and analysis of time series. Results: The number of patients hospitalized with acute coronary syndrome is growing every year: from 82 people in 2012 to 489 in 2018. According to the monitoring conducted in Poltava region, emergency medical care for patients with acute coronary syndrome with ST segment elevation in 2018 in the cities was provided in the amount of 717, while in rural areas – 288. Conclusions: Data from the analysis of emergency medical care for patients with acute coronary syndrome showed that among patients with ST-segment elevation, urban residents predominate; in 90.6% of cases, the time to the patient’s hospitalization was less than 120 minutes.

According to the current legislation medical institutions of any specialization and form of ownership are obliged to provide emergency medical care without delay and free of charge. Emergency medical care is not a profile one for many specialists and, within the framework of the current law, it is necessary to introduce an emergency medical care course into the CME system for all medical specialties at least once every five years, which will correspond to both the previously adopted system of postgraduate education and the existing standards of work of medical facilities.

In modern conditions, the burden on the staff of medical institutions has increased many times. This is due to the fact that for more than a year and a half, the planet has been overwhelmed by waves of the coronavirus pandemic. Having come from China, this virus spread like lightning across the planet, causing an economic crisis, violation of the usual foundations of human life, etc. However, despite the undulating incidence of COVID-19 and a significant number of cases, other diseases and medical emergencies also occur, and doctors need to provide patients with the necessary assistance. It should be said that, despite the high level of protective measures in medical institutions, the provision of emergency medical care to patients is associated with a certain risk for doctors, since assistance should be provided to such patients immediately, and it is not always possible to quickly assess whether they are carriers of the virus, unfortunately. For this reason, the risk of infection to medical workers providing emergency care to patients, especially surgical ones, is extremely high. During the epidemic, the treatment of burn patients who have not confirmed the absence or presence of a coronavirus infection, the processes of establishing intravenous access, endotracheal intubation or tracheostomy, wound treatment and surgical intervention are the points of risk of infection. The introduction of an effective, appropriate degree of protection and the development of practical medical procedures will increase the level of protection of the medical worker and reduce the risk of infection with COVID-19. The aim of the work is to investigate the features of providing antiviral protection for medical workers and to study the existing practical treatment protocols for providing emergency medical care to patients with thermal and chemical injuries during the spread of COVID-19.

The comparative effectiveness of enoxiparin sodium and non-fractional heparin (NPH) in the treatment of acute coronary syndrome (ACS) in young men was studied. 100 patients with ACS were examined on the basis of the Samarkand branch of the Republican Scientific Center for Emergency Medical Care from 2018 to 2020. The average age of the patients was 34±5 years. The patients were divided into 2 groups. The first group consisted of 54 men who received anticoagulant therapy with enoxiparin sodium at a dose of 1 mg / kg after 12 hours of 0.5 mg/kg. The second group consisted of 46 men who received NSF, with the first dose of 10,000 units administered intravenously by bolus, and then-5000 units 6 hours subcutaneously.

The result of the development of research in cardiology revealed the opportunity to implement the concept of the “golden hour” to restore coronary blood flow, which formed a new attitude to myocardial infarction with the recognition of its emergency, treatment of which requires emergency hospitalization. In recent years, a sufficient number of multicenter studies have been conducted, the data of which provided an opportunity to formulate the main strategic directions of care for patients with acute coronary syndrome and became the basis for the development of clinical guidelines by leading national and international organizations. The aim of our research was to conduct a comparative analysis of modern approaches to the treatment of acute coronary syndrome in Ukraine and abroad. The research used logical, graphical methods and the method of system analysis. The article covers modern issues of classification of acute coronary syndrome, methods of its diagnosis and treatment. Based on the results of clinical studies, global trends in the diagnosis and treatment of acute coronary syndrome are shown. The article also discusses the organization of medical care, and in particular the possibilities and problems of reperfusion therapy in the treatment of acute coronary syndrome. Analysis of literature sources and own data indicate the existence of some problems in the treatment of acute coronary syndrome, among which significant factors in high mortality are late treatment and late hospitalization. Organizational measures to improve patient awareness, early diagnosis, and early reperfusion treatment are important steps in improving the prognosis of acute coronary syndrome.
