Many non-invasive imaging techniques are available for the evaluation of patients with known or suspected coronary heart disease. Among these, computed-tomography-based techniques allow the quantification of coronary atherosclerotic calcium and non-invasive imaging of coronary arteries, whereas nuclear cardiology is the most widely used non-invasive approach for the assessment of myocardial perfusion. The available single-photon-emission computed tomography flow agents are characterised by a cardiac uptake proportional to myocardial blood flow. In addition, different positron emission tomography tracers may be used for the quantitative measurement of myocardial blood flow and coronary flow reserve. Extensive research is being performed in the development of non-invasive coronary angiography and myocardial perfusion imaging using cardiac magnetic resonance. Finally, new multimodality imaging systems have recently been developed bringing together anatomical and functional information. This article provides a description of the available non-invasive imaging techniques in the assessment of coronary anatomy and myocardial perfusion in patients with known or suspected coronary heart disease.

: Menopause is associated with changes consistent with cardiovascular aging. The effects on cardiac disease is multifaceted affecting endothelial function, coronary artery physiology and metabolic dysfunction leading to structural changes in the coronary anatomy. A systematic review of literature from 1986 to 2019 was conducted using PubMed and Google Scholar. The search was directed to retrieve papers that addressed the changes in cardiovascular physiology in menopause and the current therapies available to treat cardiovascular manifestations of menopause. The metabolic and clinical factors secondary to menopause such as dyslipidemia, insulin resistance, fat redistribution and systemic hypertension contribute to the accelerated risk for cardiovascular aging and disease. Atherosclerosis appears to be the end result of the interaction between cardiovascular risk factors and their accentuation during the perimenopausal period. Additionally, complex interactions between oxidative stress and levels of L-arginine and ADMA may also influence endothelial dysfunction in menopause. The increased cardiovascular risk in menopause stems from the exaggerated effects of changing physiology on the cardiovascular system affecting peripheral, cardiac and cerebrovascular beds. The differential effects of menopause on cardiovascular disease at the subclinical, biochemical and molecular levels form the highlights of this review.

We report the case of a 71-year-old patient with many risk factors for coronary atherosclerosis, who underwent computed coronary angiography (CTA), in accordance with the guidelines, for recent onset atypical chest pain. CTA revealed critical (>50% lumen diameter narrowing) stenosis of the proximal anterior descending coronary, and the patient was scheduled for invasive coronary angiography (ICA). Before ICA he underwent enhanced transthoracic echo-Doppler (E-Doppler TTE) for coronary flow detection by color-guided pulsed-wave Doppler recording of the left main (LMCA) and whole left anterior descending coronary artery (LAD,) along with coronary flow reserve (CFR) in the distal LAD calculated as the ratio, of peak flow velocity during i.v. adenosine (140 mcg/Kg/m) to resting flow velocity. E-Doppler TTE mapping revealed only mild stenosis (28% area narrowing) of the mid LAD and a CFR of 3.20, in perfect agreement with the color mapping showing no flow limiting stenosis in the LMCA and LAD. ICA revealed only a very mild stenosis in the mid LAD and mild atherosclerosis in the other coronaries (intimal irregularities). Thus, coronary stenosis was better predicted by E-Doppler TTE than by CTA. Coronary flow and reserve as assessed by E-Doppler TTE trumps coronary anatomy as assessed by CTA, without exposing the patient to harmful radiation and iodinated contrast medium.

RésuméDes observations nouvelles et quelques échouages complètent le premier bilan faunistique établi en 1989 pour les îles Kerguelen. Aux neuf espèces identifiées précédemment ( Eubalaena glacialis , Balaenoptera acutorostrata , Megaptera novaeangliae , Physeter macrocephalus , Mesoplodon layardii , Cephalorhynchus commersonii , Lagenorhynchus cruciger , Orcinus orca , et Globicephala melas ) s'ajoutent, grâce à des échouages, deux nouvelles espèces: Ziphius cavirostris et Hyperoodon planifrons . En outre, la présence occasionnelle de Phocoena dioptrica est confirmée. L'événement le plus marquant est cependant le grand nombre d'observations (n=1649) de cachalots ( Physeter macrocephalus ) faites depuis 1989 au delà du plateau des îles, dans une zone peu fréquentée autrefois par les bateaux de pêche. Pour le dauphin de Commerson ( Cephalorhynchus commersonii ), espèce la plus commune, des données biologiques et écologiques s'ajoutent aux données faunistiques. Nous faisons également état, pour trois espèces ( Physeter macrocephalus , Orcinus orca , et Globicephala melas ), d'interactions avec la pêcherie à la palangre de la légine australe ( Dissostichus eleginoides ).
