We followed the fate of 166 patients with limb arterial embolism in the late stage of the disease after conservative and surgical treatment from 6 months to 10 years. Of 166 patients, 44 (26.5%) died after hospital discharge. Thirty of them were older than 61 years. The most frequent causes of death in the long-term period were acute cerebrovascular disease (11), myocardial infarction (7), progressive cardiovascular insufficiency due to cardiac defects (6), repeated peripheral artery embolism causing limb gangrene (7). In 7 patients the cause of death was not determined.

AbstractBackground:In patients with tetralogy of Fallot with the diminutive pulmonary arteries, we sometimes have to give up the complete intra-cardiac repair due to insufficient growth of the pulmonary arteries. We have carried out palliative intra-cardiac repair using a fenestrated patch.Methods:Of all 202 patients with tetralogy of Fallot in our centre since 1996, five patients (2.5%) with the diminutive pulmonary arteries underwent palliative intra-cardiac repair using a fenestrated patch. Mean operative age was 1.8 years. Previous operation was Blalock–Taussig shunt in 4. At operation, the ventricular septal defect was closed using a fenestrated patch and the right ventricular outflow tract was enlarged. Follow-up period was 9.8 ± 2.6 years.Results:There were no operative and late deaths. Fenestration closed spontaneously on its own in four patients 2.7 ± 2.1 years after the intra-cardiac repair with a stable haemodynamics; however, the last patient with the smallest pulmonary artery index had supra-systemic pressure of the right ventricle post-operatively. The fenestration was emergently enlarged. Systemic arterial oxygen saturation was significantly and dramatically increased from 83.5 to 94% after the palliative intra-cardiac repair, and to 98% at the long term. A ratio of systolic pressure of the right ventricle to the left was significantly decreased to 0.76 ± 0.12 at the long term. Now all five patients were Ross classification class I.Conclusion:Although frequent catheter and surgical interventions were needed after the palliative intra-cardiac repair, this repair might be a choice improving quality of life with good results in patients with tetralogy of Fallot associated with the diminutive pulmonary arteries.

Over the course of time, new developments associated with embryogenesis of the murine heart have served to clarify the developmental processes observed in the human heart. This evidence allows for creation of a developmental framework for many congenital cardiac defects. Here, we aim to solidify the framework related to the categorization of both solitary and multiple ventricular septal defects. Mice having genetic perturbation of the Furin enzyme have demonstrated perimembranous and juxta-arterial ventricular septal defects, permitting the inference to be made that these defects can co-exist with defects occurring within the apical muscular septum. Based on developmental evidence, furthermore, all interventricular communications can be placed into one of three groups, namely, those which are perimembranous, juxta-arterial, and muscular. All of the defects are described based on their borders as seen from the morphologically right ventricle. Our focus here will be on those defects within the muscular ventricular septum, recognizing that such defects can co-exist with those that are perimembranous. We discuss the differentiation of multiple discrete defects from those referred to as the ‘Swiss cheese’ variant. As we show, appropriate surgical management requires understanding of the specific terminology, as the surgical approach may differ depending on the combination of the individual defects. Data from the Society for Thoracic Surgeons revealed that both mortality and morbidity were increased in the setting of multiple as opposed to solitary ventricular septal defects.

Objectives: In this study we observed that the effect of post-Norwood 3.5 mm modified BTS(mBTS and 5 mm right ventricle to pulmonary (Sano modification) shunt models on clinical outcomes. Patients and Methods: 54 patients who were operated in our hospital between 2011and 2020 years, were included in our study and this is a retrospective, observational, single-center case series study. Results: The effect of shunt type on mortality was statistically significant, and the rate of death was lower in patients who used SANO type shunts. (p <0.05) The effect of VISC score on early mortality was significant, and VISC score was higher in the group with exitus. (p <0.05). Conclusion: Although Sano shunt seems more preferred in Norwood, the experience of the surgeon and the center's experience may have led to variation in results at HLHS. Nevertheless, it will be clinically important for studies to evaluate the long-term results between the two shunt types.

Introduction. Cardiac hemangiomas of the right ventricle are very rare and mostly asymptomatic benign tumors. The surgical excision is the first line treatment. Case report. We report a case of 69-year-old woman with an asymptomatic arteriovenous hemangioma of the right ventricle. The complete surgical excision was performed with the use of cardiopulmonary bypass and the patient was discharged on the postoperative day 6 after the uneventral postoperative course. There was no relapse during the six-month followup. Literature review revealed totally 35 cases of this tumors including our case Conclusion. Described procedure can be performed safely with the excellent long-term results.
