Axenfeld–Rieger syndrome (ARS) is a rare autosomal dominant disorder that has both systemic and ocular anterior segment dysgenesis. The ocular manifestations include posterior embryotoxon, iris and anterior angle abnomalies with a high risk of glaucoma and blindness. The systemic manifestations can include craniofacial abnomalies such as maxillary hypoplasia, hypodontia, oligodontia and microdont.

Axenfeld Rieger Syndrome is anterior segment dysgenesis characterized by posterior embryotoxon and dysgenesis of the anterior chamber. Developmental abnormalities of the anterior angle cause increased resistance to outflow. Ocular hypertension is a complication in almost 50% of the cases. It is an autosomal dominant disease and its prevalence is between 50,000 and 100,000 newborns per year. The main associated affected genes are FOXC1 and PITX2, occurring in 40% of the cases. Axenfeld Rieger Syndrome can be associated with systemic manifestations such as dental anomalies (hypodontia and microdontia), facial anomalies (maxillary hypoplasia, telecanthus and hypertelorism), redundant paraumbilical skin, hypospadia, auditory and cardiac alterations. Within this syndrome, we distinguish different phenotypes, being the Rieger anomaly the one with least systemic affectation. We present three cases in different stages of the disease, which allow us to understand the development and management of this disorder.

Monogenic syndromic disorders frequently feature ocular manifestations, one of which is glaucoma. In many cases, glaucoma in children may go undetected, especially in those that have other severe systemic conditions that affect other parts of the eye and the body. Similarly, glaucoma may be the first presenting sign of a systemic syndrome. Awareness of syndromes associated with glaucoma is thus critical both for medical geneticists and ophthalmologists. In this review, we highlight six categories of disorders that feature glaucoma and other ocular or systemic manifestations: anterior segment dysgenesis syndromes, aniridia, metabolic disorders, collagen/vascular disorders, immunogenetic disorders, and nanophthalmos. The genetics, ocular and systemic features, and current and future treatment strategies are discussed. Findings from rare diseases also uncover important genes and pathways that may be involved in more common forms of glaucoma, and potential novel therapeutic strategies to target these pathways.

Familial hypocalciuric hypercalcaemia (FHH) is a rare genetic autosomal dominant disorder, with 3 variants described. An inactivating mutation in the calcium sensor receptor (CASR) gene causes the subtype 1, which represents 65% of the cases. Inactivation of Ca-sensing receptors (CaSR) can also lead to hypercalcemia associated with increased parathyroid hormone (PTH) secretion.[1] It is characterised by causes mild asymptomatic hypercalcemia[2] and hypocalciuria with normal or elevated PTH. FHH is generally asymptomatic and treatment is not needed. Differential diagnosis with primary hyperparathyroidism (PHPT) is crucial and based on calcium-creatinine clearance ratio (CCCR), which, when under 0.02 points to the diagnosis of FHH.[3] Genetic test is necessary for confirmation.[4]

Neurofibromatosis type 1 (NF1) is an autosomal dominant disorder with multiple systemic manifestations. Pulmonary involvement has been reported in the form of interstitial fibrosis, emphysema, pulmonary hypertension and thoracic neoplasm. We report a case of desquamative interstitial pneumonia in a non-smoker with NF1.

Introduction. Piebaldism is an autosomal dominant disorder characterized by the congenital absence of melanocytes in the affected areas of skin and hair due to mutations of the KIT protooncogene, which affects the differentiation and migration of melanoblasts. Case report. A 3 ? month old male infant was admitted to hospital due to depigmentation of skin in the area of forehead, trunk and extremities. On admission, he had multiple, irregularly shaped areas of leucoderma present at the forehead, abdomen, lower legs and left forearm. Based on the characteristic skin features and family history, we diagnosed the boy?s leucoderma as piebaldism. Conclusion. Vitiligo differs from piebaldism by the presence of unstable hypopigmented lesions that are acquired later in life. Albinism presents with widespread skin involvement and lacks the characteristic hyperpigmented macules within hypopigmented areas.

Axenfeld-Rieger syndrome is a rare autosomal dominant disorder characterized by various ocular and extraocular malformations. Dental abnormalities are considered as definitive features for the diagnosis and differentiation of Rieger syndrome from other anterior chamber of the eye malformations. A case of Rieger syndrome with distinct dental and craniofacial anomalies is described. Significant cranio-dento-facial findings that have been observed are, teeth with short and dilacerated roots, hyperplastic frenums and underdeveloped maxilla. There was an anterior crossbite, bilateral posterior openbite and moderate to severe anterior crowding.

The leucine-rich repeat-containing G protein-coupled receptor 4 (LGR4, also called GPR48) plays a key role in multiple developmental processes, and mice lackingLgr4display anterior segment dysgenesis leading to early-onset glaucomatous retinal ganglion cell loss as well as defective eyelid formation. This paper will review Lgr4 signaling and its regulation of the Axenfeld-Rieger syndrome genePitx2, a crucial developmental transcription factor. In addition, Wnt signaling plays an important role in eye development, with Norrin functioning to activate the Wnt receptor Frizzled 4 required for proper retinal vascularization. Recent discoveries identifying Lgr4 as a receptor for Norrin highlight the potential for Lgr4 function in retinal vascularization. Finally, several unanswered questions impeding a full understanding of Lgr4 in glaucoma are considered as avenues for further research.

Abstract Case report - Introduction ANCA-associated vasculitis (AAV) encompasses the clinical entities of GPA, MPA, renal-limited vasculitis and eGPA. Even though well recognised and described in the medical literature, ocular manifestations in AAV are relatively uncommon (<20%) and may precede, present concomitantly with, or follow systemic manifestations. Our patient developed scleritis as the first manifestation of AAV and presented to the ophthalmology department. Within a few days, he developed systemic symptoms and subsequently severe and potentially life-threatening pulmonary haemorrhage. With collaborative working, he received appropriate treatments and made a good recovery. Case report - Case description Our patient is a 36-year-old Indian gentleman, who presented to Broomfield Hospital ophthalmology department in February 2021 with a 1-week history of pain and redness involving the left eye. Diagnosis of anterior scleritis was made and he received dexamethasone 0.1% eye drops and later switched to prednisolone 60mg/day. Investigations are shown below (Table 1). Diagnosis of AAV was made and he came under the care of the rheumatology. By this time, he noticed fleeting but severe arthralgia. He received three pulses of I.V. methyl prednisolone and received first dose of 1000mg of rituximab and one pulse of IV cyclophosphamide. His haemoglobin dropped with reduced oxygen saturation of 88% on air. Repeat CT chest showed extensive pulmonary haemorrhage and he was admitted to ITU. He did not need intubation and was transferred to University College London Hospital. Following five plasma exchanges and high-dose prednisolone, CRP fell from 113 to 6.6 and saturation improved to 98% on air. He completed four rituximab infusions. By June 2021 his chest X-ray returned to normal. 1. Table of Investigations Case report - Discussion This patient’s story highlights multiple clinical aspects of AAV. Published literature states that ocular manifestations as “initial” presentation of AAV are very uncommon (about 6%). The ophthalmologist requested the correct investigations including ANCA which helped to establish the diagnosis. Within a few weeks, the patient went on to develop other systemic manifestations which necessitated stepping-up the immune therapy. Rituximab was chosen for remission induction as it is now established as an alternative to cyclophosphamide. We discussed the case at virtual MDT of ENRAD (Eastern Network for Rare Autoimmune Disease) and got swift approval to use rituximab. Unfortunately, his clinical course was complicated with development of pulmonary haemorrhage which is potentially life-threatening. The PEXIVAS trial (Walsh et al NEJM 382; 622-31 (2020)) compared groups randomised to plasma exchange or no plasma exchange in addition to corticosteroids and either rituximab or cyclophosphamide. Outcomes were not different between the groups. However, Kronbichler et al (Nephrol Dial Transplantation 36; 227-31 [2021]) have argued that there was a trend towards better outcomes in a subgroup with alveolar haemorrhage and that plasma exchange may still have a role in such patients. Following five cycles of plasma exchange our patient made an excellent recovery from pulmonary haemorrhage which is very rewarding. Case report - Key learning points Scleritis is an uncommon presenting feature of AAV and should prompt the physician to look for systemic symptoms and check for ANCA serology. To recognise pulmonary haemorrhage as a potential life-threatening manifestation in a patient with AAV who drops haemoglobin. Despite lack of strong clinical trial evidence, plasma exchange can be a very useful therapeutic tool. Early recognition and initiation of immune therapy is crucial to induce remission. Collaborative working with clinicians from different medical specialities is the key for improved patient outcomes.
