Pulmonary fibrosis in a patient with history of ruptured silicone breast implants may present a therapeutic challenge to diagnose and treat. In this case report, we aim to discuss our experience in diagnosing a patient with chronic silicone embolism syndrome masquerading as refractory multifocal pneumonia that presented with respiratory failure. A young woman with no significant past exposure having recurrent admissions to the hospital due to fever and shortness of breath was found to have chronic silicone embolism with pneumonitis. This case report emphasis the prompt diagnosis and treatment of silicone induced fibrosis and approach to the most common side effects of breast implants.

Although described as far back as 1898, the Ganser state is still a subject of controversy and causes discussions regarding its existence as a clinical entity, its psychopathology and its incidence. Lately, Goldin and Macdonald (1955) endeavoured to clarify the meaning of the Ganser state after a critical review of the literature. They provided an illustrative case report with some hitherto unreported features and psychological test results. Although their paper presents a wealth of valuable material, there are still some points which need further discussion and elucidation.

Cancer is a result of genetic changes precipitated by heredity, viruses, industrial agents, drugs, radiation, diet, and many other unknown agencies. At least 2 and perhaps as many as 10 or more chromosomal abnormalities are necessary. Certain genetic aberrations cause the malignancy to become more aggressive and metastasize. At this time, there is little evidence that silicone can cause cancer in the human except, perhaps, under the theory of solid-state carcinogenesis. Information on all types of cancers in patients with silicone breast implants has to be gathered. Prospective studies must be started and retrospective analyses and clinical cases should be reported. This article adds six new cases to the record and offers a review of the literature.
