Objective: To evaluate the diagnostic significance of reticulocyte maturity indices in iron deficiency anaemia. Study Design: Comparative cross-sectional study. Place and Duration of Study: Department of Hematology, Armed Forces Institute of Pathology, Rawalpindi, from Sep 2019 to Jun 2020. Methodology: A total of 340 children, ages between 1 and 5 years of either gender, were divided into two groups based on haemoglobin and ferritin level. Group A comprised of 203 children with haemoglobin >11 g/dL andferritin level >7 ng/ mL. Group B comprised of 137 children with haemoglobin <11g/dL and ferritin <7 ng/mL. Red blood cell and reticulocyte parameters were compared. Results: Iron deficiency anaemia was found to be 37.6% in the study population. Comparison revealed that the children in groups B had significantly higher mean red cell distribution width (17.1 ± 1.3% vs. 13.3 ± 1.3%) and significantly lower hemoglobin (9.5 ± 1.0 g/dL vs. 12.7 ± 1.0 g/dL), mean corpuscular hemoglobin (21.8 ± 2.5 pg vs. 28.6 ± 2.3 pg), mean corpuscular volume (69.4 ± 3.5 fL vs. 80.4 ± 3.3 fL), mean corpuscular hemoglobin concentration (31.5 ± 1.8 g/dL vs. 32.7 ± 1.6 g/dL) and serum ferritin level (4.5 ± 2.6 mg/mL vs. 55.9 ± 32.1 ng/mL). No significant differences in the mean total red blood cell count, hematocrit and reticulocytes between two groups were noted (p>0.05) and significant differences were noted in terms of low, medium & high fluorescence ratios in both groups (p<0.05). Background: Fetal iron stores are affected by maternal diabetes and it is lower at birth in infants of diabetic mothers (IDMs). Risks for developing iron deficiency and neurocognitive impairment are reported in IDMs. This study was done to assess serum ferritin and red cell indices in IDMs and to compare the values with infants born to mothers without diabetes mellitus. Methods: This cross-sectional study was carried out at BIRDEM General Hospital from March to October, 2018. Total 102 full term neonates were included in this study. Among them 70 neonates were IDMs and 32 were infants born to mother without diabetes mellitus. Serum ferritin and red cell indices like hemoglobin (Hb), packed cell volume (PCV), mean corpuscular volume (MCV), mean corpuscular hemoglobin (MCH), mean corpuscular hemoglobin concentration (MCHC), red cell distribution width (RDW) were measured within 48 hours of birth. Comparison of red cell indices and serum ferritin level were done between IDMs and infants of non-diabetes mothers. Statistical analysis was performed by using Epi info, and p value of <0.05 was considered statistically significant. Results: IDMs had significantly higher value of Hb% (19.00 vs 17.47 g/dl), PCV (57.60 vs 52.67 %) and RDW (20.09 vs17.77 %) than infant of non-diabetic mother (p <0.05). But there was no significant difference regarding the values of MCV, MCH and MCHC between IDMs and infants of non-diabetic mothers (p >0.05). Serum ferritin level was found significantly low in IDMs (94.51 vs 307.50 ng/ml, p <0.001). Conclusion: Iron stores of IDMs were found significantly lower at birth despite higher hemoglobin content, as indicated by lower serum ferritin level. Further studies and long-term follow up are needed to determine whether these infants are at risk for developing iron deficiency anemia or iron-deficient neurocognitive disorder Birdem Med J 2020; 10(3): 182-186

AbstractObjectivesTo compare the effect of Iberet Folic® and Zincofer® on haemoglobin (Hb) and serum ferritin level; and its adverse effect.MethodsThis randomised controlled trial conducted from January 2018 until December 2018. Pregnant women below 34 weeks of gestation, with Hb concentration less than 11 g/dL and serum ferritin level less than 12 ug/L were randomised to receive either one tablet Zincofer® or one tablet Iberet Folate® daily for four weeks. Both groups were compared in terms of effect on Hb level, serum ferritin level, and other haematological indices adverse effect related to treatment, and treatment cost.ResultsHundred and thirty patients were recruited in this study with 68 patients in Iberet Folic group and 62 patients in Zincofer group. The change in the Hb and serum ferritin level from baseline to day 30 did not differ significantly between treatment groups. The mean (±SD) change from baseline to day 30 was 2.15 (±0.59) g/dL in the Iberet Folic group, and 1.98 (±0.49) in the Zincofer (p value = 0.08). Mean serum ferritin at day 30 was 17.2 (±3.68) ug/L and 16.7 (±4.28) ug/L with 8.44 (±3.41) and 8.55 (±3.50) difference, respectively (p = 0.86). Adverse events were comparable in between groups, with p value >0.05. GI intolerance and constipation were among the common side effects, occurred in 34.6 and 29.2% cases, respectively.ConclusionsZincofer® offers equivalent efficacy and side effect profile in comparison with Iberet Folic® for the treatment of iron deficiency anaemia (IDA) during pregnancy, but with lower cost.

A Cross sectional multicentre study was carried out to determine the prevalence of Iron deficiency (ID) and Iron deficiency anaemia (IDA) in children using Ferritin and full blood count. Full blood count was done with Mindray Bc-2800 and Ferritin test with Enzyme Linked Immuno-Sorbent Assay. Authorization was granted by Cameroonian national Ethical Committee. Data were analysed using Statistical Package of Social Sciences 21.0 significant results were considered for P< 0.05. ID varied from 4.2% to 9.6% for cut-offs of 30 and 50μg/L. IDA ranged from 4.2 to 8.5% at these 2 cut-offs. Considering red cells indices, Mean Corpuscular Volume and Mean Corpuscular Haemoglobin blood count currently used should be interpreted cautiously as only Mean Corpuscular Volume and Mean Corpuscular Haemoglobin corroborate with a Ferritin level lower than 30 μg/L.

Background: Iron deficiency is the commonest cause of anaemia and at least half of anaemia cases will have causes other than iron deficiency. The objective of this study was to determine the prevalence of iron deficiency anaemia among antenatal mothers based on haematological parameters and serum ferritin levels.Methods: This was a hospital based cross sectional study, conducted at Meenakshi Medical College and Research Institute, Kancheepuram from January 2017 to June 2018. Two hundred antenatal mothers who were diagnosed with anaemia in the third trimester of pregnancy were evaluated by haematological parameters and serum ferritin assay.Results: 38% of women had mild anaemia, 50% had moderate anaemia and 12% had severe anaemia. On peripheral smear, 60% had microcytic hypochromic anaemia indicating iron deficiency, and 12% had megaloblastic anaemia. In 41% of cases, the mean corpuscular volume was <82fL indicative of iron deficiency. Among the 120 women diagnosed with iron deficiency anaemia on smear, the mean ferritin level was 16.62±73.42 (less than 30µg/l). There was a statistically significant low levels of serum ferritin levels in those diagnosed with iron deficiency anaemia (P < 0.05). The serum ferritin level was >160µg/lit in 19% of cases.Conclusions: Based on the haematological parameters and serum ferritin estimation, in 60% of cases, anaemia was due to iron deficiency. Before initiating treatment, it is important to differentiate various types of anaemia, so that appropriate treatment can be given.

Aim: The aim of our study was to determine association between iron deficiency and simple febrile seizure in children aged 6 months to 5 years.Methods: Both case and control group were analysed for biochemical tests including complete blood count for measurement of Hb., Hct, MCV, MCH, MCHC and RDW. Peripheral smear used for confirmation of microcytic, hypochromic picture of RBCs and confirmatory test for iron deficiency anaemia by serum ferritin level also included.Results: 22 out of 50 (44%) cases had iron deficiency anaemia whereas, 10 out of 50 (20%) controls were found to have iron deficiency anaemia. Odds ratio derived from these results was 3.14 (CI 1.29- 7.65, P-0.01).Conclusions: Screening for iron deficiency should be done in children presenting with simple febrile seizure.
