Clinical introductionA 24-year-old Filipino man attended the ED with a 1-month history of a discrete swelling over his upper anterior chest wall that was rapidly increasing in size and tenderness. He denied any other symptoms. His medical history was unremarkable.Examination revealed a tender, 7 cm × 6 cm mass over the upper part of the sternum (figure 1). The surface was smooth, it was immobile, non-compressible and the overlying skin was normothermic but mildly erythematous. Cervical lymphadenopathy was present. Abstract Background: Numerous publications have showed the biomechanical influence of thoracic spine on cervical spine. Recently, some authors have pointed out the role of other physiological systems in this relationship, what have been calle interdependence regional model. It has been studies that interventions applied over thoracic spine have a neurophysiological influence on cervical spine and respiratory system. Our study evaluates the influence of bilateral manipulative technique of first rib on pain and pulmonary function in subjects with chronic non-specific neck pain.Methods: The immediate changes produced by bilateral direct manipulation (HVLA) of first rib in subjects with non-specific chronic neck pain will be studied. The sample will be randomly divided into two groups, experimental and placebo. As dependent variables of the study, we will measure the peak expiratory flow and cervical rom. Findings will be analyzed statistically considering a 5% significance level (p ≤ 0.05)Discussion: Our study aims to provide knowledge about the influence of bilateral manipulative technique of first rib in pain and pulmonary capacity in subjects with non-specific chronic neck pain. The efficacy of this technique may entail an easy access and low cost strategy in patients with pain and a reduction in pulmonary function. An 18-year-old male with no previous medical history presented to hospital with sudden onset of acute epigastric pain radiating to the anterior chest wall and both shoulders. There was no history of recent trauma and he had not been vomiting.

We present the first case of a thoracic ranula which originated from the left submandibular area extending into the subcutaneous tissue planes of the anterior chest wall. The patient had a history of surgery for a previous benign left salivary gland cyst, and presented with an enlarging mass in the anterior chest wall. This was a recurrence of a ranula, with an extension into the anterior thoracic wall. The thoracic ranula was excised, together with ipsilateral sublingual and submandibular glands, via a transcervical approach. No recurrence was detected over a 3-year post-operative follow up.

Background: Calcified juxtafacet cysts in the cervical spine are extremely rate. Such symptomatic cysts commonly cause neck pain, radiculopathy, or even myelopathy. MR and CT studies typically document cord/ root compression. On occasion, some of these cysts will spontaneously regress, while many others may warrant surgical removal. Case Description: A 70-year-old male presented with a 2-year history of a progressive tetraparesis. The preoperative MR/CT studies showed a C1-C2 left extradural mass occupying more than half of the spinal canal. On MR, it was homogeneously hypointense on both T1- and T2-weighted images, while the CT showed a calcified cyst. Intraoperative and histopathological findings documented a calcified cervical juxtafacet cyst (i.e. ganglion subtype) that was fully excised without sequelae. Conclusion: C1-C2 juxtafacet cervical cyst should be considered when a patient presents with myelopathy due to a calcified MR/CT documented paraspinal lesion contributing to significant cervical cord/root compression.
