Dental ceramics have an excellent ability to reproduce natural teeth appearance, but once the substrate has an altered coloration, it is necessary to mask this intercurrence, considering the characteristics of the ceramic and cement selected. This article aims to present a clinical case of rehabilitation with ceramic laminates to mask a darkened central incisor by planning a more opaque porcelain structure under a translucent contact lens. A female patient sought dental care complaining of dissatisfaction with smile aesthetics. It was clinically identified the presence of a Class IV darkened restoration in composite resin in element 11. In treatment planning harmony in the shape and color of elements 11 and 21 was aimed at through lithium disilicate lenses. An opaque intermediate structure was used to cover the darkening in tooth 11. The double molding technique was performed to obtain the working model. Prior to cementing, the surfaces were treated. In ceramics, conditioning with 10% hydrofluoric acid and 37% phosphoric acid, silane application and universal adhesive; in dental substrate, conditioning with 37% phosphoric acid and application of universal adhesive were performed. In laminates definitive cementation Allcem Venner cement was chosen. Patient reported satisfaction with treatment final aesthetics. Thus, cementation technique of a translucent laminate over an opaque facet is a good option for masking substrates with darkened coloration.

The present report follows the case of a young boy with solitary median maxillary central incisor (SMMCI) syndrome between the ages of 4 and 7 years. This condition is characterized by the presence of one single maxillary central incisor in the midline instead of two central incisors. No other developmental abnormalities involving growth or brain function were noted at, or subsequent, to birth. This report includes a discussion of the aetiology of SMMCI syndrome and its association with birth defects such as holoprosencephaly (HPE), CHARGE and VACTERL, as well as a discussion of the long-term prognosis and associated dental and medical issues for this particular patient

The latest guidelines from the Enhanced Recovery After Surgery (ERAS®) Society stated that early drain removal after pancreatoduodenectomy (PD) is beneficial in decreasing complications including postoperative pancreatic fistulas (POPFs). This study aimed to ascertain the actual benefits of early drain removal after PD. The data of 450 patients who underwent PD between 2018 and 2020 were retrospectively reviewed. The surgical outcomes were compared between patients whose drains were removed within 3 postoperative days (early removal group) and after 5 days (late removal group). Logistic regression analysis was performed to identify the risk factors for clinically relevant POPFs (CR-POPFs). Among the patients with drain fluid amylase < 5000 IU on the first postoperative day, the early removal group had fewer complications and shorter hospital stays than the late removal group (30.9% vs. 54.5%, p < 0.001; 9.8 vs. 12.5 days, p = 0.030, respectively). The incidences of specific complications including CR-POPFs were comparable between the two groups. Risk factor analysis showed that early drain removal did not increase CR-POPFs (p = 0.163). Although early drain removal has not been identified as apparently beneficial, this study showed that it may contribute to an early return to normal life without increasing complications.

Background and Objectives: This study was conducted to determine the effective method to torque the incisor with thermoplastic aligner using a three-dimensional (3D) finite element method. Materials and Methods: Three finite element models of maxilla and maxillary dentition were developed. In the first model, thermoplastic aligner without any auxiliaries was used. In the second and third models, thermoplastic aligner with horizontal ellipsoid composite attachment and power ridge were used, respectively. The software used for the study was ANSYS 14.5 FE. A force of 100 g was applied to torque the upper right central incisor. The resultant force transfer, stress distribution, and tooth displacement were evaluated. Results: The overall tooth displacement and stress distribution appeared high in the model with power ridge, whereas the root movement was more in the horizontal ellipsoid composite attachment model. The model without any auxillaries produced least root movement and stress distribution. Conclusion: Horizontal ellipsoid composite attachment achieved better torque of central incisor than the model with power ridge and model without any auxillaries.
