This case report describes a male patient born in 1953 presenting an occasional increase in serum amylase and lipase forty years ago. The monitoring of enzymes was accompanied by radiological investigations, which did not reveal pancreatic pathology. In his family, including 10 siblings, half were carriers of this isolated anomaly; multiple cysts in pancreas, kidneys, and liver were present in some family members, in addition to a pancreatic neoplasia in a sister who did not carry the enzymatic abnormality. Our patient developed colon adenocarcinoma at the age of 67.Here we examine the characteristics of non-pathological chronic pancreatic hyperenzymemia defined as such by the main Italian pioneer Professor Gullo.

Hypoglycaemia in the post-operative period is mainly iatrogenic (related to anti-hyperglycaemic drugs), but can be explained by an endogenous hyperinsulinemic state. In the context of previous gastrointestinal surgery, a form of dumping syndrome can mask hypoglycaemia from an underlying cause, such as an insulinoma. The authors present a clinical case of a male patient who underwent oesophageal surgery for an oesophago-gastric junction adenocarcinoma and developed hypoglycaemic symptoms in the post-operative period, caused by an undiagnosed insulinoma. This case report portraits the diagnostic investigation of a hypoglycaemia state in the post-operative period, narrowing to the workup of an endogenous hyperinsulinemic hypoglycaemia and provides a summary of insulinoma’s treatment. An insulinoma should always be considered in a patient with endogenous hyperinsulinemic hypoglycaemia, even with a history of oesophago-gastric surgery.

The article analyzes the experience of the joint work of a speech therapist and a neuropsychologist using the DIRFloortime concept on a clinical case of a boy L., 3.5 years of age. The following tasks were solved during the sessions: the child’s advancement along the first stages of functional emotional development, taking into account his individual characteristics; work with the family: establishing a partnership, teaching the family members the basics of the DIRFloortime approach. As a result of the sessions, the child’s understanding of speech and implementation of instructions have significantly improved; he started using meaningful words and phrases for communication; his emotional and non-verbal repertoire expanded, the boy learned to play simple games with rules with an adult.

The lip brake is a fold in the oral mucosa, originating in the upper lip and insertion in the alveolar mucosa. At birth, the labial frenum may be inserted in the palatal papilla. As the individual grows, he can migrate to a more upper region, featuring a normal lip curl. However, when it remains inserted in the papilla, it is a case of hypertrophic lip curl. When pathological, this structure can promote the formation of interincisal diastema, periodontal pocket, gingival retraction, aesthetic problems, difficulties in phonation and brushing. The objective of this article is to report a clinical case of upper lip frenectomy using the double clamping technique or Archer technique, performed at the Oral Minor Surgery clinic of the Dentistry School of the Federal Fluminense University, in a male patient, leucoderma, 23 years old, indicated by his orthodontist after recurrence of interincisal diastema. From the procedure performed, it can be concluded that the surgical intervention and the applied technique were effective in solving the reported clinical case.

OBJECTIVE: Dental aesthetics is one of the areas of dentistry that has most advanced in terms of procedures, techniques and materials. Changes in color, shape, and texture are commonly cited as grounds for complaint, which are sometimes characterized by details, making the work challenging. The objective of this work is to report the planning and execution of a clinical case of smile harmonization with ceramic laminates for remodeling smile.CASE DESCRITION: A male patient consulted a specialist to refine the appearance of his smile that already had a satisfactory pattern of color and texture. The planning was started using digital smile planning, followed by diagnostic waxing of the elements involved. The planning approval took place through the Mock up. The dental grinding performed was minimal and followed a silicone wear guide. The pieces were made in lithium disilicate and installed under absolute isolation according to the cementing protocol of the material. The finishing and polishing completed the installation of the pieces, removing excesses and guaranteeing periodontal compatibility.CONCLUSION: We conclude that indirect aesthetic treatments currently have a minimally invasive character and are highly accepted due to their ability to remodel the smile with perfection and naturalness.

Avulsion fractures of the processes of the Antero Inferior Iliac Spine are rare in daily medical practice. The clinical case of a 14-year-old male patient was presented, a high-performance baseball athlete less than 15 years of age, who during a training race from home to first base felt strong pain in the left inguinal region that caused absolute functional impotence. He was treated in the orthopedic guard corps and an X-ray of the pelvis bone, ECOSOMA and CT was performed, diagnosing a fracture due to pulling of the Antero Inferior Iliac Spine. The injury was treated conservatively with rest, knee in semi-flexion and the administration of NSAIDs orally; consolidation of the fracture was achieved after two months. He underwent physiotherapy rehabilitation for another two months. Six months after the injury occurred, the patient achieved his incorporation back to daily sports practice without loss of sports performance.
