As the world advances, living standards are getting changed with inclination more toward sedentary life style and junk food, we are seeing a worldwide increased incidence of coronary artery disease. A 73 year of age gentleman with progressive shortness of breath NYHA-II/III since last 6 months and epigastric discomfort with effort angina CCS-III. Was having no other co-morbidities, except of 35 pack years of smoking. On evaluation his Coro-angiogram revealed Diffuses TVCAD with Lad diffusely diseased throughout its middle course >90%, LCX 80%, RCA mid-Course critical discrete lesion of 90%. His 2D, TTE showed normal valvular apparatus and ejection fraction of 55%. Literature review further furnishes the fact that LAD is most commonly being affected by atheroma formation hence making it the most common site for endartrectomy, though peri-operative MI is still a grave concern, but half dose protamine slow reversal of the heparin and keeping ACT at 150-160 sec greatly reduces the incidence of peri-operative MI.

Abstract Background The honeycomb-like structure (HLS) is a rare cause of myocardial ischaemia characterized by multiple communicating channels divided by thin septa. The aetiology of this specific structure remains speculative. Case summary A 55-year-old man was admitted due to worsening effort angina during the previous 2 months. Diagnostic coronary angiography revealed occlusion of the distal right coronary artery (RCA) with good collateral flow from the left coronary artery. We considered this lesion as a recent total occlusion, and planned a percutaneous coronary intervention (PCI). At the time of PCI, 7 days after admission, an angiogram showed a spontaneous recanalization of the occlusive RCA. Intravascular ultrasound (IVUS) depicted a HLS at the recanalized lesion, including atherosclerotic stenosis. We managed these lesions with drug-eluting stents. Discussion A recanalized thrombus may manifest as a HLS. In this case, the patient suffered from worsening effort angina during the previous 2 months, we confirmed a spontaneous recanalization of the occluded coronary lesion by serial angiographic images, and observed HLS adjacent to the atherosclerotic attenuated plaque by using high-resolution IVUS. Recanalized organizing thrombus is considered an entity of HLS. However, all previous studies on the HLS in vivo have detected the structure in an already recanalized state. Therefore, the aetiology remained speculative and evidence has been indirect. This present case demonstrates that recanalized atherosclerotic thrombosis might be one of the causes of HLS.

Aim. To assess five-year changes of somatic risk factors and comorbidities in patients with angina of effort.Material and methods. The study included 320 patients (143 men and 177 women aged 40-69 years) with coronary artery disease (CAD), class I-III angina of effort. Patients underwent examinations in 2012 in three medical centers of the Republic of Ingushetia (Russia), and in 2017 they were invited for a second complex examination with questionnaires, biochemical analysis, and instrumentation.Results. Over the five-year follow-up, there was an increase of men with class III effort angina up to 45%; a similar trend was observed in women. Both men and women experienced a three-fold increase in the incidence of type 2 diabetes. Initially and during the follow-up, chronic obstructive pulmonary disease was diagnosed 2 times more often in men than in women, which may be associated with smoking. A small increase in the total cholesterol level was recorded in the cohort; among women, these changes were significant. Target levels reach no more than 20% of patients. A significant increase of blood glucose levels over the 5-year period was observed in a cohort of men and women with effort angina.Conclusion. Over the 5-year follow-up, there was a clinical deterioration of the effort angina, which is associated with an increase in the incidence of some somatic diseases and the severity of the main behavioral and biological risk factors.

Abstract Aim To compare the inflammatory response of polymer-free versus permanent polymer drug-eluting stents in patients with stable coronary artery disease undergoing elective PCI, and the effect of this inflammatory response on the 6 month clinical outcomes. Methods This randomized prospective comparative study was conducted on 100 patients with stable coronary artery disease planned for elective PCI divided into 50 patients with deployed permanent polymer DES (PP-DES) groups and 50 patients with polymer free DES (PF-DES). Blood samples were taken to assess serum level of hsCRP before the procedure and the first day after the procedure. The target end points were the recurrence of effort angina, incidence of target lesion revascularization (TLR) over a period of 6 months and the inflammatory response by measuring the percentage of rise in hsCRP level. Results There was no statistically significant difference in the clinical outcome in the first six months between the patients who had PP-DES and the patients who had PF-DES regarding recurrence of effort angina (p-value = 0.822) and TLR (p-value = 0.727), with no patients developing myocardial infarction or stent thrombosis. There was a statistically significant inflammatory response after stent deployment in both groups represented by a significant rise in the serum level of hsCRP on the first day after the procedure in both groups (p-value < 0.001), but there was no statistically significant difference in the inflammatory response between PP-DES and PF-DES (p-value = 0.978). The number of patients having recurrence of anginal symptoms and TLR with hsCRP rise ≥ 11.36% was higher than those having the same with hsCRP < 11.36%. However, this difference was not statistically significant regarding recurrence of anginal symptoms (p = 0.057) and TLR (p = 0.092). Conclusion The PF-DES was proved to be non-inferior to PP-DES regarding the short-term clinical outcome and the early inflammatory response. It was concluded that the presence of polymeric coating in PP-DES did not add to the magnitude of the inflammatory response and it did not significantly increase the incidence of recurrence of anginal symptoms and TLR in this group of patients.
