Exfoliative erythroderma is rare but serious condition, which requires close supervision. We report a rare case of 28-year-old man with kissing lesions of Paederus dermatitis at his right side of neck. The rash caused by Paederus beetle was improved after treatments. However, the patient developed generalized erythema with desquamation and scaling. The patient was successfully treated topically with moisturizing liquid soap and topical moisturizer with emollients and humectants, and triamcinolone lotion was applied on the bright red lesion. The patient was also treated with oral replacement solution and tropical azelaic cream was applied on the hyperpigmented kissing lesion. This case report shows the importance of a diagnostic practice with follow-up examination.

Thermostable polymerases play a significant role in molecular biology and diagnostic practice. The most famous and demanded is Polymerase I from the thermophilic bacterium Thermus aquaticus (Taq-pol). This polymerase at one time made a kind of revolution in the polymerase chain reaction. In this work, we attempted to modify this polymerase by attaching an additional Sso7d protein from Sulfolobus solfataricus to Taq-pol, which provides additional binding to the double-stranded DNA of the template. Sso7d-Taq fusion gene was expressed in BL21(DE3) cells. Optimal conditions were selected for maximum production of modified Sso7d-Taq polymerase. The optimal conditions for the intracellular accumulation of Sso7d-Taq polymerase: activation of the T7 promoter when the optical density of the culture reaches OD600 = 0.8-1.0 by adding IPTG at a concentration of 0.2 mM, followed by incubation of the culture at 37°C for 20-24 hours. Recombinant Sso7d-Taq polymerase has been purified and tested by PCR for thermal stability and elongation time. It was found that the Sso7d-Taq enzyme withstands 5 hour incubation at 95°C and 75 minute incubation at 98°C. Comparative analysis with unmodified Taq DNA polymerase showed that the Sso7d-Taq enzyme reduces the elongation rate by several times - up to 15-13 seconds per 1 kbp. The results obtained indicate the prospects of using Sso7d-Taq DNA polymerase in scientific research and diagnostic practice.

Abstract BackgroundCulture and social context affect the expression and interpretation of symptoms of distress, raising challenges for transcultural psychiatric diagnostics. This increases the risk that mental disorders among migrants and ethnic minorities are undetected, diagnosed late or misdiagnosed. We investigated whether adding a culturally sensitive tool, the DSM-5 core Cultural Formulation Interview (CFI), to routine diagnostic procedures impacts the psychiatric diagnostic process. MethodWe compared the outcome of a diagnostic procedure that included the CFI with routine diagnostic procedures used at Swedish psychiatric clinics. New patients (n=256) admitted to a psychiatric outpatient clinic were randomized to a control (n=122) or CFI-enhanced diagnostic procedure (n=134) group. An intention-to-treat analysis was conducted and the prevalence ratio and corresponding 95% confidence intervals (CI) were calculated across arms for depressive and anxiety disorder diagnoses, multiple diagnoses, and delayed diagnosis. ResultsThe prevalence ratio (PR) of a depressive disorder diagnosis across arms was 1.21 (95% CI=0.83-1.75), 33.6% of intervention-arm participants vs. 27.9% of controls. The prevalence ratio was higher among patients whose native language was not Swedish (PR =1.61, 95% CI=0.91-2.86). The prevalence ratio of receiving multiple diagnoses was higher for the CFI group among non-native speaking patients, and lower to a statistically significant degree among native Swedish speakers (PR=.39, 95% CI=0.18-0.82). ConclusionsThe results suggest that the implementation of the DSM-5 CFI in routine psychiatric diagnostic practice may facilitate identification of symptoms of certain psychiatric disorders among non-native speaking patients in a migration context. Medical practice—from testing methods to diagnostic reasoning, treatment protocols, and prognostic evaluations—is often both complex and uncertain. Via an examination of three case studies, this chapter demonstrates that effective management of diagnostic uncertainty must begin with an ability to recognize and acknowledge it in routine cases in the clinical setting. However, even when diagnostic uncertainty is recognized, this alone is not enough for effective diagnostic practice: Once it is recognized, it must then also be clearly communicated to the patient. If these steps are taken in routine cases, then it will become automatic to do so in even the most complex situations, and diagnostic practice will be the better for it.

The ICF is an internationally recognized classification of health constituents and health-related factors. ICF allows the osteopathic physician to formulate the goals of the patient′s treatment, to determine the terms of treatment. A clinical example reveals the possibilities of using ICF in osteopathy.

This paper first conducts knowledge point association analysis on a large amount of data collected in practical applications. Data mining includes data collection, data preprocessing, actual mining, and result analysis, establishes knowledge point association rules table, and develops college English diagnostic practice system. Then, starting from the existing paper composition mode of the system, the knowledge point association rule table is introduced, and the knowledge point association relationship mining model is constructed using the association rule algorithm to explore the potential influence relationship between different knowledge points that affect the improvement of learning quality. Finally, the data collected when the system is used is preprocessed, and the three dimensions of learning status evaluation, question-type association analysis, and college English score prediction are, respectively, modeled. Finally, after combining these submodels, a relatively complete and reliable diagnosis is obtained by evaluation model and related verification.

English reading ability is an important indicator to measure learners’ English ability. However, because reading ability cannot be directly observed, people usually take tests to judge the reading ability of learners. Therefore, it is very necessary to design a reasonable diagnostic practice sentence repetition recognition system to analyze and test, inform learners of the advantages and disadvantages in reading, and give corresponding countermeasures. In order to properly solve the problem of repeated recognition of English diagnostic sentences, we have developed a new recognition system combined with matching tree and edge computing technology. First, the matching tree algorithm is used for the repetitive diagnosis of English sentences. The algorithm has achieved good results in the repetitive diagnosis and matching. Secondly, an English diagnostic practice sentence repetition recognition system architecture is built through edge computing algorithms, which improves the efficiency of the English diagnostic system. Finally, through the simulation test of the English diagnostic practice system, the applicability of the established repeated recognition model is verified.
