Approximately 200 million people worldwide use injectable therapies as part of diabetes management. There appears to be a significant gap between insulin injection technique recommendations and injection practice for many. We aimed to develop and validate a novel, brief, self-administered injection technique assessment questionnaire.

Diabetes (diabetes mellitus) is a metabolic endocrine disease that is considered a «silent killer» because of its rapid growth and increased mortality due to dangerous complications of the disease. Target: Description of self-insulin injection practice of type 2 diabetes outpatient at the National Hospital of Endocrinology, 2020. Factors related to insulin self-injection practice of outpatients at the National Hospital of Endocrinology. Objects and methods: Cross-sectional descriptive studies, 250 Type 2 Diabetes Outpatient Treatment in On-Demand Clinic, National Hospital of Endocrinology from January 2020 -06/2020. Process: ITAS-scale interviews, questions about treatment costs, awareness of insulin self-injection practices. Results: 62.8% male, 68% urban area. Psychological factors group with mean age 61.45 ± 10.43, HbA1C 7.29 ± 1.55, durations of diabetes 11.72 ± 6.47, 184 patients (73.6%) had psychological insulin resistance (PIR). The rate of patients afraid of hypoglycemia 83.2%, seeing treatment failure 80%, patients situation getting worse 69.6%, difficult to determine the correct number and timing of injection 58%. Medical expenses covered by health insurance 91.2%. Age and duration of disease in the group of patients having PIR higher than in the group of patients don’t have PIR, HbA1C in the group with PIR lower than in the group without PIR. Over 190 patients self-injected insulin with injection pen: 100% preserved correctly; 97.6% determined the correct injection site; 97.6% had the correct injection time; 98% know how to change injection sites. Elderly age and prolonged illness reduce knowledge about dose compliance and post-use needle disposal. The group with psychological factors has higher understanding of needle flow and needle reuse. Conclude: Psychology and practice of self-insulin injection is a major factor affecting the effectiveness and compliance of insulin injection in patients with type 2 diabetes, which should be considered for counseling.. Keywords: Type 2 diabetes, insulin injection, psychological factors when using insulin, ITAS score.

Abstract Background Intraarticular injections (IAI) were first reported in adult rheumatology in the 1950s and subsequently gained acceptance as a safe and efficacious treatment in Juvenile idiopathic arthritis (JIA). IAIs are now widely performed and recommended as the initial or only treatment of oligoarticular JIA and ancillary treatment of actively inflamed joints in other varieties of JIA. However, the performance of the procedure is currently not guided by standardized recommendations, and several practice variations are observed. Methods This worldwide survey of pediatric rheumatologists (with 48.5% response from Pediatric Rheumatology International Trials Organization [PRINTO and Pediatric Rheumatology Collaborative Study Group [PRCSG] members) captures the differences in pre-procedural, procedural and post-procedural protocols and practices observed across the globe and asks the necessity of developing consensus in this area of Pediatric Rheumatology. Results This worldwide survey of Pediatric Rheumatologists had a response rate of just under 50% and the views of about 42% who routinely performed the procedure. It captured the differences in IAI protocols and practices observed across the globe. Significant variations in practice were noted in use of Local anesthesia, choice, and dose of therapeutic agent for the intraarticular injection and use of ultrasound to guide injections. While some practice variations may be explained by institutional protocols in different parts of the world, the clinical implications of these are largely unknown and beg the need for further studies. Conclusions Given these practice variations, the authors recommend further studies to explore the cost and clinical implications and subsequently work towards developing consensus plans to ensure uniformity in this widely used procedure in Pediatric Rheumatology.

Purpose: Intravitreal injection therapy (IVT) is the most performed procedure in ophthalmology. This study was conducted to determine current trends in IVT delivery. Methods: An online, 31-question, multiple-choice survey was sent to 1677 retina specialists. The survey consisted of 3 sections: general questions, procedure technique, and postprocedure technique. Results: A total of 264 (16%) retina specialists completed the survey. The use of povidone-iodine (100%) and small-gauge needles (97%) was common, whereas ocular anesthesia was split among lidocaine gel (31%), lidocaine drops (25%), subconjunctival lidocaine (28%), and lidocaine-soaked pledgets (15%). More than 85% indicated povidone-iodine contributes to post-IVT corneal toxicity, and 12% reported that a needlestick injury to physician or staff occurred during IVT. Conclusions: Key areas for IVT improvement include optimized ocular anesthesia, development of a guarded needle for ocular drug delivery, and formulation of a less toxic ocular antiseptic.

In nursing process, injection practice is part of nursing intervention, but injection practices was risky for the health care workers. Needle stick injury was happened to the nurses, 18% due disposal of the needle, and 15% while draw the blood for laboratory study. Adherence to the work procedure influence by few behavior factors. Aim of research of the research were to finding out of the factors influence of adherence to safe injection practice among the nurses at healthcare center X Qatar. Type of the research was observational analytic quantities of the analytic description, using cross sectional approach. 9 variable latent with 56 variable indicator. Size of sample were 114 nurses. Data analytic with using PLS-SEM. T-statistic value on variable culture 2.94; environment 1.99; work procedure 3.81 and supervisory 3.10. T-statistic of Individual characteristic, work facility, attitude, and knowledge bellow 1.96. R2 variable adherence were showed 96.05% with Q2 87.07%. Culture, environment, work procedure and supervisory influenced of the adherence safe injection practice among the nurses. (96.05%) variable independent influence of the adherence safe injection practice among the nurses, within 87.07% showed good predicted.
