Esthetic dermatology till recently was considered a domain of only the metro cities and for those of the celluloid world. Setting up an esthetic dermatology practice is a difficult task in Northeast India. Our set up is located in Shillong, capital of Meghalaya, also known as “Scotland of the East,” just adjacent to Malki Forest in neighborhood, next to Lady Hydari Park, Barik, Shillong. Since patients had to travel long distances, we tried to provide all facilities under one roof which included an in-house cafeteria, laboratory, pharmacy, counseling, minor OT and laser rooms, recovery room, and reception totaling up to approximately 12 rooms. We are two dermatologists handling an average of fifty patients daily along with procedures. Over the years, we have found the following lasers and lights to be helpful to our patients QS pigment laser, diode hair removal laser, excimer lamp, CO2 laser, intense pulse light, iclearxl and NBUVB also procedures as microdermabrasion, microneedling, microblading, micro needling radiofrequency, monopolar radiofrequency, regenera active, platelet rich plasma, chemical peels, cryotherapy, injectables as neurotoxins, dermal fillers, skin boosters, lipolytics and thread lift in no particular order. Esthetic dermatology makes up about 35% of my practice.

Aims: To evaluate the efficacy of PDO thread lifting in lower face (marionette line) rejuvenation. Study Design:  Quasi experimental study. Place and Duration of Study: Dermatology and Venereology Department, Tanta University Hospital, between January 2018 and April 2019. Methodology: The study included 10 patients asking for lower face rejuvenation who were treated by PDO absorbable threads (2 threads in each side of marionette line). Results: The mean validated grading scale for marionette line score before treatment was of 3.30 ± 0.82 while the mean score immediately after treatment was 1.70 ± 0.82. There was statistically significant improvement according to validated grading scale of marionette line after compared to before. These results were preserved after 1, 3 and 6months. Conclusion: PDO threads is a simple, rapid, immediately effective and office technique procedure.

Abstract Embedding thread lift rhytidectomy, also known as “thread lifting” in China, with the natures of simple operation, less trauma and quick recovery, is progressively used in clinical practice as a new technology of face lifting. Herewith, a brief introduction of the previous advances of thread lifting techniques and materials in the facial beauty industry, combined with the discussion on various types of sutures, common complications, and the site of actions were provided. The main limitations of present thread lifting material include: (1) the use of non-absorbable sutures is liable to cause allergies and a series of complications; (2) the absorbable sutures are easily degradation, and people need to reshape in a relatively short period. Therefore, the high biocompatible spider silk was proposed as a novel material of thread lifting suture and related devices, the advantages and preliminary achievements on spider silk were also addressed. Among the procedures for facial rejuvenation, thread lift are currently widespread among professionals and highly sought after by patients. This demand can be justified because they are procedures, without incisions, shorter inactivy time, low operating time, and for the risk of postoperative complications when compared to rhytidoplasty. These are procedures capable of promoting collagen stimulation, elevating ptosis tissues, and adding volume to the application areas contributing to the restructuring of the face. Thread lift have evolved considerably over the years and can be now meet on absorbable devices based on polydioxanone and polylactic acid. In this article is proposed the use of Fiobloc® thread lift for the purpose of assessing their affectiveness in the replacement of ptosis tissues.
