The present article focuses on detecting the need for comprehensive care in out-of-hospital emergencies in Valencia. In addition, it aims to determine the need to carry out the social and health coordination essential for this purpose. It is a qualitative exploratory study of phenomenological type where a semi-structured ad-hoc interview answered by professionals of out-of-hospital emergencies has been used for data collection. The results show that comprehensive care in out-of-hospital emergency services does not yet exist in Valencia, despite the benefits demonstrated in other autonomous communities on the biopsychosocial well-being of the population and emergency service personnel. The personnel of out-of-hospital emergency services express the need for comprehensive out-of-hospital care in Valencia to be able to assist the population in a holistic manner, covering all the needs presented by individuals, families or groups, including the needs of emergency care personnel. It is essential to take political measures that make social and health care coordination possible through common protocols and procedures among health care, management and political personnel.

ABSTRACT Objectives: to analyze the municipal planning developed by nurses coordinating Primary Health Care, aimed at coping with Chronic Noncommunicable Diseases, from the perspective of the Situational Strategic Planning. Methods: a descriptive exploratory study with a qualitative approach developed in municipalities of a Health Region of Rio Grande do Sul State. Semi-structured interviews were carried out with the Primary Health Care coordination of the municipalities of the empirical area, between 2014 and 2015. Content analysis was the thematic and the theoretical perspective of strategic analysis. Results: in the development of municipal planning regarding care for chronic illness, the study highlighted fragilities, which are a challenge for management. The Regional Municipal Planning is a tool that contributes to the articulation between actors involved in the organization of the health system. Final considerations: strategic insertion of Nursing in the field of Health prompts the debate about the role of this professional in the management of SUS.

Gaps in knowledge and systematic tracking of the prevalence of intellectual and developmental disabilities (IDD) and characteristics that may affect the health of this disability group limits our ability to address the health disparities they experience in comparison to people without disability. The purpose of this study is to begin to fill one relevant critical gap in knowledge: understanding the demographics and health outcomes of adults with IDD who receive services under Medicare Fee-for-Service (FFS), many of who are also eligible for Medicaid. Using 2016 Medicare administrative claims, we examined the prevalence and characteristics of five diagnosis groups of IDD, in those under 65 and those 65 and over, as well as their health outcomes. We found that the IDD Medicare FFS group had high prevalence rates for chronic physical and mental health conditions, overuse of emergency departments, and high rate of 30-day readmission. These findings highlight the need for evidence-based health care coordination, improved and increased public health interventions, and continued surveillance.
