Abstract Aims A mesenteric cyst, rare intra-abdominal pathology, poses a clinical challenge due to lack of pathognomonic symptoms, variability in presentation and location. We present an interesting case with a significant diagnostic dilemma; a suspected benign ovarian cyst invading the mesentery on repeat imaging and laparoscopy is questioned for mesenteric in origin. Methods A 77-year-old female presented with a 6-year history of abdominal mass. She had multiple radiological scans which reported it as a simple cyst, but the origin could not be defined. She had repeated aspirations with benign cytology. They cyst developed septations, filled-up rapidly, became an abdominal cyst from the pelvis. A careful decision was taken for surgery. The procedure undertook with general surgeons and gynaecologists with a possibility of the mesenteric cyst. During laparotomy, the cyst was found extending from pelvis and base of mesentery up to the diaphragm densely adhered to sigmoid colon. The cyst was completely excised with hysterectomy and oophorectomy. Results The patient recovered well from surgery. Post-operatively, histopathology confirmed Figo IC2 Grade 2 mucinous adenocarcinoma of the ovary with an expansive growth pattern. She is awaiting further treatment with chemotherapy after discussion in the Gynaecology MDT. Conclusion This case emphasises the importance of having an insight into the question of established diagnosis in the face of a changing clinical picture. Also, it highlights the need for understanding the mesenteric cyst. Early exploration can prevent diagnostic delays and further treatment.

Abstract Background: Ovarian malignancy is rare in pregnancy and constitutes 3 – 6% of all ovarian tumours associated with pregnancy. It is usually asymptomatic, detected during routine antenatal ultrasound scan. Treatment is mainly surgical. The aim of this report is to share our experience in the management of a referred case of papillary serous cystadenocarcinoma of the ovary disguising as degenerating uterine fibroid in pregnancy. The diagnostic challenges and treatment are highlighted with a view to improving care in our setting.Case presentation: A 29 year old gravida 2 para 1 with one living child was referred to our unit at 25 weeks gestation with complaint of progressive painful abdominal distension with dyspnoea noticed in the index pregnancy. Ultrasound scans were suggestive of uterine fibroid in pregnancy with degenerative changes for which she had conservative management. Due to worsening symptoms, she had laparotomy and a huge ovarian mass was removed alongside the affected ovary. Histology confirmed Serous Papillary Cyst Adenocarcinoma of the ovary. She is doing well on adjuvant chemotherapy. Conclusion: A high index of suspicion for ovarian malignancies in pregnancy, coupled with complementing MRI (when available) with ultrasound to differentiate degenerating uterine fibroids is suggested/recommended.

Abstract Objectives: Clear cell adenocarcinoma of the ovary (CCAO) and mixed cell adenocarcinoma of the ovary (MACO) were one of the gynecological malignancies.Methods: Univariate and multivariate cox regression analysis were used to determine prognostic factors. Drawing nomograms, the receiver operating characteristic (ROC) curve and the calibration curve was applied to evaluate the agreement of the nomogram. The survival analysis was constructed to the high-risk factors.Results: The nomogram was constructed and had a better discrimination. The calibration curves indicated that the nomograms had good calibration capabilities.4 or more regional lymph nodes removed by surgery was beneficial to the patient's prognosis. Conclusions: Our study analyzed the prognosis of CCAO or MACO patients, and constructed a predictive nomogram with good accuracy.
