The most common vaginal neoplasias in old dogs are leiomyoma and fibroma. Although surgical excision is the indicated treatment, it does not eliminate potential complications that may lead to death or poor quality of life. This paper reports the case of a 9 year-old female Doberman dog with vaginal leiomyoma attended by copious and recidivating colporrhagia homeopathically treated between March and December 2005. Homeopathic approach was the one designed at Homeopathic Unit of the Veterinary Hospital, Rural Federal University of Rio de Janeiro, Brazil. The progress of disease was assessed through clinical evaluation, laboratory exams and ultrasonography. Hemorrhage decreased while the general clinical state of the animal improved. After 7 months of homeopathic treatment, the tumor was spontaneously eliminated through violent abdominal contractions, which was followed by recovery of the animal’s state of health. Keywords:  Homeopathy; Veterinary medicine; Tumors; Leiomyoma; Vaginal; Dogs.   Tratamento homeopático de leiomioma vaginal em cães: relato de caso Resumo As neoplasias vaginais mais comuns em cadelas velhas são o leiomioma e o fibroma. Embora a excisão cirúrgica é o tratamento indicado, não elimina complicações potenciais que podem levar ao óbito ou baixa qualidade de vida. O presente artigo relata o caso de uma cadela Doberman de 9 anos de idade, com leiomioma vaginal, apresentando colporragia copiosa e recidivante, tratada entre março e dezembro de 2005. A abordagem homeopática utilizada foi a desenvolvida na Unidade Homeopática do Hospital Veterinário da Universidade Federal Rural do Rio de Janeiro, Brasil. A evolução da doença foi acompanhada através de avaliação clínica, testes de laboratório e ultrassonografia. A hemorragia diminuiu e o estado geral melhorou. Depois de 7 meses de tratamento homeopático, o tumor foi espontaneamente eliminado através de violentas contrações abdominais, seguido de recuperação do estado de saúde do animal. Este artículo relata el caso de un canino Doberman femenino de 9 años de edad, con leiomioma vaginal acompañado de colporragia abundante y recidivante, tratado entre marzo y diciembre de 2005. El abordaje homeopático utilizado fue el desarrollado en la Unidad Homeopática del Hospital Veterinario de la Universidad Federal Rural de Río de Janeiro, Brasil. La evolución de la enfermedad fue estimada mediante evaluación clínica, exámenes de laboratorio y ecografía. Las hemorragias disminuyeron, con mejoría del estado general. Después de 7 meses de tratamiento homeopático, el tumor fue espontáneamente eliminado mediante violentas contracciones abdominales, seguido de recuperación del estado de salud del animal. Paraurethral vaginal leiomyoma is the infrequent case to be described. Approximately 300 cases have been described so far. Imaging modalities aid in identifying the morphological, structural characteristics of the mass and its relationship to the surrounding structures. Thirty-six years old married women presented with a vulvar mass of (3x5) cm2. Her associated complaints were left shift of the urinary stream and dyspareunia. Ultrasonography and cystography revealed a mass with no relationship with bladder or uterine structure. Surgical excision was performed. The histopathological report confirmed the diagnosis of paraurethral vaginal leiomyoma. Surgical excision is the treatment of choice and diagnosis is confirmed by histopathological examination.

Leiomyomas are common benign tumors of the uterus, affecting 20-30% of women of reproductive age group. But vaginal leiomyomas remain an uncommon entity with only about 300 reported cases. The first case was described in 1733. Only a few cases have been reported in Nepal to date. Tumors are thought to arise from Mullerian smooth muscle cells in the sub-epithelium of the vagina. Vaginal leiomyomas are usually situated in the anterior vaginal wall. Here, we report a case of a 48-year old multipara who presented the outpatient department with the ultrasonographic report showing multiple uterine fibroids but was asymptomatic. A physical examination showed a mass in the right vaginal wall. Pervaginal removal of the tumor was performed and subsequent histopathology revealed a vaginal leiomyoma. Removal of the tumor by the vaginal route, wherever possible, with the subsequent histopathological examination, appears to be the optimum management plan.

Leiomyomas are common benign smooth muscle tumours of the uterus. Leiomyoma of the vagina are very rare with only about 300 reported cases. Correct diagnosis and differentiation from urethral lesions are important but is usually difficult on clinical examination alone. A 40-year P3L3, with a previous history of abdominal hysterectomy 5 years back, for multiple fibroids presented with complaints of a mass descending per-vagina since the last 2 years. Since the last 6 months she had dyspareunia, dysuria and perineal discomfort. On external appearance the mass looked like a vault prolapse. However, on vaginal examination, it appeared as a single well circumscribed mass arising from the anterior vaginal wall and the vault. Transvaginal sonography showed a hypoechoic mass arising from the anterior vaginal wall of dimension 50×54×63 mm pressing on the urethra. Contrast CT revealed a rounded homogeneous well-defined soft tissue density mass lesion measuring approximately 63×52 mm in the region of the vaginal vault interposed between the bladder base and the rectum. Bilateral ovaries and adnexa appeared normal. Surgical excision of the tumour through the vagina was done. Histopathology confirmed leiomyoma.  The diagnosis of vaginal leiomyoma is based on careful examination and preoperative imaging both by ultrasonography and a CT scan/MRI for ascertaining its location, extent, its relationship to adjacent structures and characterization before attempting surgical excision. The correct clinical diagnosis of a vaginal fibroid is frequently overlooked in favour of much more common urethral diverticulum.
