The goal of this work is to deepen the relationship between hyperactivity and tonic dialogue, by considering the body as a common element of communication even if their psychomotor manifestations and meanings are different during development. While tonic dialogue is vital for the child in the constitution of the attachment bond, psychomotor instability is the expression of a difficulty in relating to reality. In the clinic it is difficult to place instability in international diagnostic classifications, due to the multi-problematic nature and variability of expressions of it. Research confirms the consequences of a disorder of tonic dialogue, constituting a model on which future behaviors will be organized. The authors hypothesize that each stage of the body communication, if lived with difficulty, constitutes a matrix on which diversified expressions of instability will be structured. The intent, therefore, is to collect theoretical-clinical material in order to subsequently make a diagnostic classification of psychomotor instability. El objetivo de este trabajo es profundizar en la relación entre hiperactividad y diálogo tónico al considerar el cuerpo como elemento común de comunicación, aunque sus manifestaciones psicomotoras y significados sean diferentes durante el desarrollo. Mientras que el diálogo tónico es vital para que el niño establezca el vínculo de apego, la inestabilidad psicomotora es la expresión de una dificultad para relacionarse con la realidad. La complejidad y variabilidad de los cuadros clínicos dificulta su ubicación en las clasificaciones diagnósticas internacionales. La investigación luego confirma las consecuencias del trastorno del diálogo tónico, constituyendo un modelo sobre el que se organizarán los comportamientos futuros. Los autores plantean la hipótesis de que cada etapa del diálogo tónico, si se vive con dificultad, constituye una matriz sobre la que se estructurarán diversas expresiones de inestabilidad. La intención es recopilar material teórico-clínico para posteriormente realizar una clasificación diagnóstica de la inestabilidad psicomotora.

With growing attention being paid to perinatal mood and anxiety disorders (PMADs) in both medical and mental health settings, there is a need for further elaboration on meaningful and impactful treatments with this population. This article outlines some of the unique stressors and psychological states that come with pregnancy and parenting a newborn and infant. The concepts and experiences discussed include: primary maternal preoccupation (Winnicott, 1956), parental ambivalence, major changes to the physical body, and reorganisation of attachment representations and current family dynamics. Clinical material from therapy sessions with patients at a specialised perinatal centre is included in the discussion of using an approach informed by attachment theory (Bowlby, 1988).

Drawing upon Donald Winnicott’s classic paper, “Delinquency as a Sign of Hope”, the author has underscored the importance of recognising the role of emotional loss in the aetiology and treatment of adolescent forensic patients, and she has elucidated how this particular psychoanalytical lens can assist psychotherapists to work more effectively with deprivational trauma. The author has also described her experience as both a field social worker and as a forensic psychotherapist, and has shared moving clinical material with great clarity and generosity, demonstrating the value of incorporating Winnicott’s insights into the forensic consulting room. In particular, the author has examined how the patient can use the psychotherapist as an understanding object who can help to counteract earlier deprivations.

The proposed surgical procedure was studied on six unfixed male human cadavers. The removal of the end colostomy and paracolostomy hernia with subsequent intra-abdominal implantation of a previously prepared hernioprosthesis from the xenopericardium were modeled on the corpses. The use of the proposed method for the repair of paracolostomy hernias, including the use of a xenopericardial plate as an endoprosthesis according to an original technique, has shown its consistency and prospects for use on clinical material. Further studies associated with the use of the proposed technique of paracolostomy hernia repair in clinical practice seem promising. Acharya sushruta- the father of Indian surgery has logically characterized in a foundational way an abundance of clinical material and the standards of the board for vidradhi, which are legitimate even today. "Sheegra vidhahivat'' meaning of vidradhi itself recommends the destructiveness of the disease. Vidradhi word is advanced from vidra, i.e, a painfull condition like pricking, stabbing or cutting sensation in the skin. The infection Vidradhi (abscess) is a typical infirmity disturbing mankind and debilitate the victim for his standard work. It presents as a limited expanding with torment, red discoloration,local rise of temperature, delicacy and confined capacity of impacted part. it is normal in India with second most noteworthy frequency because of helpless disinfection, stuffing and deficient sustenance. Around the world, roughly 40-50 million individuals are contaminated every year with amoebic abscesses.  The current exploration article is intended for relative investigation of Vidradhi and Abscess as far as Samprapti (Pathophysiology), Lakshanas (Clinical elements) and Chikitsa (Treatment) affirms that Vidradhi and Abscess can be comparable disease entities.

BACKGROUND: Despite numerous studies of the vaginal microbiota, there is still a lack of knowledge regarding its restoring dynamics in the early postpartum period. The condition of the vaginal microflora during pregnancy plays a key role in maintaining the physiological microbiocenosis of the birth canal and creating conditions for the normal course of pregnancy, the establishment of an infants intestinal microbiota, and the further development of the child. AIM: The aim of this study was to estimate the role of certain types of lactobacilli in restoring the vaginal microbiota in women in the early postpartum period, depending on the method of delivery. MATERIALS AND METHODS: We examined 150 women at 38-41 weeks of gestation. The clinical material for the study was vaginal discharge before and after delivery. To determine the species of lactobacilli and other microorganisms in the clinical material, we used quantitative real-time PCR. RESULTS: Before delivery, lactobacilli were found in vaginal discharge in 144 out of 150 women (96.0%), their number in the majority being more than 106 GE. In the postpartum period, lactobacilli were found in 66/128 (51.5%) cases, while prevailing in women after vaginal delivery in 50/65 (76.9%) cases (4.61.6, p = 0.000000). Among the dominant species of lactobacilli was L. crispatus, found in vaginal discharge before delivery, which most often affects the recovery of the vaginal microbiota in the postpartum period (29 out of 61 women, 47.5%). L. iners detected in the lochia predisposes to the violation of uterine involution in the early postpartum period (p = 0.03). CONCLUSIONS: Normal vaginal microbiota in the postpartum period is restored more quickly in women after vaginal delivery. Our study confirms that L. crispatus and L. iners play a major role in restoring the vaginal microbiota in the postpartum period.

Freud, in 1937, put forward the idea that the repudiation of femininity and the death instinct were factors contributing to the resistance to recovery in psychoanalysis, stimulating much contentious debate. I will illustrate how these ideas have been modified and expanded as both the repudiation of femininity and the death instinct are linked by their envy of the receptive dependence on the good object which lead to a feeling of humiliation and shame in both males and females. When the death instinct is formulated as an anti-life instinct the relationship with envy becomes clearer, the two may indeed turn out to be different aspects of the same thing. In this article I will use clinical material to try to show how the process of negotiating this receptive attitude by repudiating femininity in favour of phallic omnipotence is enacted with perverse behaviour and violence and how, with the help of analysis, the patient may be able to modify their destructive behaviour.
