Development of an original method for the repair of paracolostomy hernias

The proposed surgical procedure was studied on six unfixed male human cadavers. The removal of the end colostomy and paracolostomy hernia with subsequent intra-abdominal implantation of a previously prepared hernioprosthesis from the xenopericardium were modeled on the corpses. The use of the proposed method for the repair of paracolostomy hernias, including the use of a xenopericardial plate as an endoprosthesis according to an original technique, has shown its consistency and prospects for use on clinical material. Further studies associated with the use of the proposed technique of paracolostomy hernia repair in clinical practice seem promising. In Uruguay, Lacanian ideas arrived in the 1960s, into a context of Kleinian hegemony. Adopting a discursive approach, this study researched the initial reception of these ideas and its effects on clinical practices. We gathered a corpus of discursive data from clinical cases and theoretical-doctrinal articles (from the 1960s, 1970s and 1980s). In order to examine the effects of Lacanian ideas, we analysed the difference in the way of interpreting the clinical material before and after Lacan's reception. The results of this research illuminate some epistemological problems of psychoanalysis, especially the relationship between theory and clinical practice.

Bacground: Variceal bleeding is a severe complication of portal hypertension due to cirrhosis with high rate of motality, hence, predicting early rebleeding and mortality in cirrhotic patients with acute variceal bleeding is vital in clinical practice. Objectives: To evaluate the prognostic value of the combination of AIMS65 and MELD scores in predicting first 5 days in-hospital rebleeding and mortality in cirrhotic patients with acute variceal bleeding. Materials and Methods: 44 cirrhotic patients with acute variceal bleeding hospitalized at Hue Central Hospital. MELD and AIMS65 scores were calculated within the first 24 hours and monitoring rebleeding and mortality in the first 5 days in these patients. Results: AIMS65, MELD scores can predict first 5 days rebleeding and mortality with AUROC are 0.81, 0.69 and 0.92, 0.95, respectively. Combination of AIMS65 and MELD scores can predict first 5 days in hospital rebleeding with AUROC is 0.84, sensitivity 83.3%, specificity 81.6% (p<0.001) and mortality with AUROC is 0.96, sensitivity 100%, specificity 92.7% (p<0.001). Conclusions: The combination of AIMS65 and MELD scores increased the sensitivity, specificity and prognostic value in predicting first 5 days in-hospital rebleeding and mortality in cirrhotic patients with acute variceal bleeding in compare to each single scores. - Some problems of the relationship between psychotherapy and scientific research are examined. The following aspects are discussed: the theory of demarcation between science and non-science, the problem of replicability, "hard" and "soft" sciences, complexity and chaos theory, the levels of probability and indeterminacy, the inductive-deductive circle, abduction, etc. Clinical material is presented in order to exemplify the issues under discussion. Some of the problems met by empirical research in psychotherapy (for example the manualization of psychotherapy techniques) are described, and the phases of the history of psychotherapy research movement are summarized. - The authors aim to deepen the concept of timing. They focus on the internal and external facts used by the analysts in the choice of when to interpret. Following an accurate historical examination of the concept of timing, some detailed clinical material is presented. A specific greed is articulated as an instrument used to deepen those aspects that have an influence on the analyst's choice of the timing of interpretation. Objetivo: Analizar el contenido de los videos testimoniales sobre pérdidas gestacionales en Youtube en español para poder concluir si las narrativas se enmarcan en la lógica biomédica moderna o posmoderna de vivir esta situación de acuerdo a los postulados de Frank y Bauman. Métodos: Búsqueda sistemática de los vídeos en español desde 2011 a mayo de 2019, que contuvieran en su título las palabras clave “aborto” o “perdida gestacional”, y que tuvieran más de 100.000 visualizaciones en Youtube desde su publicación, como criterios de inclusión. Visualización de los vídeos para su posterior análisis de contenido para detallar las características del canal, los motivos para contar la experiencia, el momento en que se cuentan, la interacción existente con las seguidoras, las etapas que se narran sobre el suceso y el lenguaje utilizado para relatarlo. Resultados: Se encontraron nueve youtubers que cumplieron los criterios de inclusión, y un total de diez vídeos que recogían relatos sobre pérdidas gestacionales. Sólo dos de los nueve canales estaban centrados exclusivamente en la maternidad. Los relatos se inscriben en una lógica biomédica posmoderna, por la necesidad manifiesta de explicar el suceso online, el reconocimiento de la fase del duelo, y la poca presencia del lenguaje de la supervivencia. Conclusiones: Este estudio muestra que la necesidad de explicar la experiencia online responde a la decisión de visibilizarla y de servir de apoyo a otras mujeres. Los resultados de este estudio pueden ser de utilidad para los/as profesionales de Enfermería que en su práctica clínica tienen contacto directo con las mujeres que han sufrido pérdidas gestacionales. Objective: To analyze the content of testimonial videos about pregnancy loss in Spanish on YouTube, so as to explore whether such narratives are framed in relation to the modern or postmodern biomedical logic of this experience as formulated by Frank and Bauman. Methods: Systematic search of videos in Spanish from 2011 to May 2019, using the following inclusion criteria: contain the key words miscarriage or pregnancy loss in their title and have had more than 100,000 views on YouTube since their publication. To enable further content analysis, the videos were viewed to detail the characteristics of the publication channel in each case and to catalog their contents in terms of reasons for talking about the experience, the time chosen to tell the story, interaction with followers, the stages in the narrative, and the language used. Results: We identified nine YouTubers that met the inclusion criteria, comprising a total of ten videos. Only two of the nine channels focused exclusively on motherhood. The stories are inscribed in a postmodern biomedical logic, due to the clear need to explain the traumatic event online, the recognition of the grief phase, and the limited use of survival language. Conclusions: This study shows that the need to talk about the experience online is motivated by a decision to make the experience visible and offer support to other women. The results of this study may be useful to nursing professionals whose clinical practice involves caring for women who have suffered pregnancy loss.

Abstract Knowledge of anatomical anomalies is significant for all specialists in clinical practice and may prevent serious complications following medical procedures. This report presents the rare crossed fused renal ectopia (CFRE) with atypical renal vasculature in cadaver of a 68-year-old man. The ectopic kidney was located on right side with four renal veins, three renal arteries, two ureters, where one of them is double. The embryological background, as well as the potential clinical significance of this morphological variation, is discussed. An interventional radiological and surgical procedure should be appropriately implemented to treat anomalies of vessels and CFRE.

Looking towards Ayurved, there are lot of Panchakarma Procedures Advocated for Shalakya (ENT & Ophthalmic) disorders. This review focuses on the specific characteristics of COVID-19 disease, which leads not only to respiratory impairments (bronchoalveolar epithelium does not retain oxygen, etc.), but also decreases the level of hemoglobin and its ability to transfer oxygen to the organs and tissues and increases the level of heme, resulting in anoxemia, hypoxia in all organs and tissues, and oxidative stress. Mexidol, a drug developed in Russia, is widely used in clinical practice, including the treatment of diseases accompanied by ischemia and hypoxia. Mexidol has antihypoxic and antioxidant effects, can treat mitochondrial respiratory dysfunction, thereby affecting the key processes in different cells of organs and tissues that develop due to hypoxia. Mexidol can be useful in the comprehensive therapy of patients with COVID-19.
