There have been considerable recent technological developments for implant overdenture attachments. This study presents an overview of the biomechanical and biomolecular aspects of various attachments for implant overdenture. Available articles on attachments for implant overdenture were reviewed from January 1980 to August 2021 in the ScienceDirect, MEDLINE/PubMed, and Web of Science resources, and relevant studies were included in this study. We focused on the following topics: attachment systems, retention of various attachments, stress distribution with different attachments, the design and fabrication of attachments, digital techniques in overdenture attachments, and the effects of attachments in peri-implant health. We found that plastic resin is commonly used for ball and bar attachments, whereas nylon resin is commonly used in locator attachments. The locator system offers a valuable attachment option for implant-retained overdenture. Attachment retention reduces while lateral force increases with implant inclination in overdenture. The higher the retention of an overdenture attachment, the higher the transferred stresses. Additionally, clip loading produces more stress in implants and precision elements than bar-retained dentures. As such, we conclude that the ball and locator systems the best overdenture systems due to their superior tissue response, survival rate, and patient satisfaction.

Aim: Aim of this study was the evaluate the retention and biting force of conventional complete denture and after placement of a single implant in the mandible for an implant retained over-denture Material and methods Eight completely edentulous patients were selected. A single implant was inserted at the symphysis of the mandible and left to osseointergrate. During the osseointegration period a conventional complete denture was fabricated and inserted. Retention and biting force was measured at insertion and after 3 months of service. After osseointegration attachment was connected, space was formed in the fitting surface of the existing mandibular denture and retention silicon was placed. Retention and biting force were measured at insertion and after 3 months follow-up. Data were collected and statistically analyzed. Results: The retention mean values for the complete denture was 2.420±0.360, however the retention mean values for the single implant over-denture was 6.395±0.289.  F=3.80 with statically significance difference between the groups of P <0.01. The biting force mean values for the complete denture was 52.62±2.71, however the biting force mean values for single implant over-denture was 71.45±2.73. F= 1.790 with statically insignificance difference between the groups of P >0.01 Conclusion: Single implant overdenture improved the retention and the biting force when compared with the complete denture, this has improved the quality of life and happiness.

The purpose of this multicenter randomized controlled trial was to compare the clinical, radiographic, and patient-centered outcomes of early loaded mandibular overdentures deliberately placed on two or three implants. The outcomes were: implant and prosthesis success and survival rates; biological and technical complications; marginal bone loss; patient satisfaction; and periodontal parameters. The results showed no differences between the groups in any of the outcomes analyzed. With the limitations of the present study, and looking at long-term follow-up, the gold standard of prosthetic rehabilitations with attachments, in agreement with the scientific community, should prefer two non-splinted implants.

Aim: To compare new design implant-supported removable partial dentures retained with anterior fixed prosthesis with a conventional locater and bar attached implant overdenture prostheses retained by two or four implants via photoelastic stress analysis. Materials and Methods: Seven edentulous mandibuler acryclic models prepared and divided into two main groups; two to four implant models, subgroup separation as stated; for two implant models overdenture with locator attachment, crown design retained removable prosthesis with clasp retention, bridge design retained removable prosthesis with clasp retention, bridge design retained removable prosthesis with precision attachment retention; and for four implant models prosthesis with bar attachment overdenture, fixed bridge design retained removable prosthesis with clasp retention, fixed retained removable prosthesis with precision attachment retention. A 300 N load was applied to the first premolars. Photoelastic stress analysis method that is a specific method concerning stress visualization, and does not require statistical analysis, was used. The stress distributions were seen in optically using a poloriscope. Results: In the models with two implant-retained removable partial dentures, the stress distribution was found to be lower than that with the four implant-retained removable partial dentures. Nonsplinted implants caused high stress around the distal implant on the loading side. Conclusion: The stress loads were transmitted to other implants by splinting. Implant-supported removable partial dentures with an anterior fixed prosthetic design show lower stress distributions compared with bar retained prosthesis. These dentures appear to be advantageous in terms of stress transmission.
