A state of relative immunosuppression exists in normal pregnancy. In this issue of JEM, Hong et al. (https://doi.org/10.1084/jem.20190185) perform blood immunomonitoring in pregnancy, in both healthy women and women with lupus, and observe early and sustained transcriptional modulation of lupus-related pathways in both groups. When signatures of inflammation did not normalize in lupus, risk of pregnancy complications was increased.

Peripartum cardiomyopathy is a serious, potentially life-threatening heart disease of uncertain aetiology in previously healthy women. We report a morbidly obese woman who presented with peripartum shortness of breath. We discuss the differential diagnosis of dyspnoea in pregnancy and highlight the complexity of care of the morbidly obese woman.

British recommendations (DRVs and RNIs) include hardly any increments for pregnancy. British recommendations for protein are likely to cause unsatisfactory birthweight. What is the normal nutrient intake in pregnancy of healthy women? What evidence is there of special requirements for folate, iron, magnesium or iodine? New recommendations for nutrient intakes in pregnancy should be the responsibility of the Food Standards Agency and a survey of the diets of pregnant women is needed.
