ABSTRACT Objective: To evaluate the healing of the modified inverted “Y” incision in patients with scoliosis due to myelomeningocele. Methods: Retrospective study through medical records review of patients with myelomeningocele surgically treated with a modified inverted “Y” approach between January 2013 and December 2015. Results: We analyzed the medical records of six patients. Two patients progressed with skin complications in the immediate postoperative period and only one of them required surgical intervention for debridement and suturing. In another patient, it was necessary to perform two surgical reviews due to material failure without skin complications in these interventions. Conclusions: The modified inverted “Y” technique is a great alternative to traditional incision and inverted “Y” because it has good results in patients with spina bifida associated with poor skin conditions treated surgically for correction of spinal deformities. Slip inception mechanism is very important for modeling of static friction and understanding of some experimental observations of friction. In this work, slip inception was treated as a local competence of interfacial bonding failure and weaker material failure. At any contacting point, if bond shear strength is weaker than softer material shear strength, slip inception is governed by interfacial bonding failure. Otherwise, it is governed by softer material failure. Considering the possible co-existence of these two slip inception mechanisms during presliding, a hybrid static friction model for smooth dry contact was proposed, which indicates that the static friction consists of two components: one contributed by contact area where bonding failure is dominant and the other contributed by contact area where material failure is dominant. With the proposed static friction model, the effects of contact pressure, the material properties, and the contact geometry on static friction were discussed.

This paper addresses the problem of improvement of mechanical component reliability by the systematic identification of material failure mechanisms. Experience shows that, in many cases of service failure, failure was caused by a known mechanism which was overlooked, either by design, or elsewhere in the planning process. This paper describes one approach to designing mechanical components against failure by material deterioration, but may have application to other fields. It is based on a finding from the examination of case studies which shows that material failures follow logic structures which can be described by Boolean algebra expressions. These structures are defined as Material Failure Logic Models (MFLM’s), and can be used as a means of systematically identifying potential failure mechanisms in a complex process. The identification technique is based on the observation that MFLM’s are insensitive to the precise causes of the individual events. The paper deals primarily with problems of defining MFLM’s. Some examples of MFLM’s are given. A brief discussion is presented of a CAD system under development at the University of Illinois at Urbana-Champaign.

Abstract Background The high-oblique sagittal osteotomy (HOSO) is an alternative to a bilateral sagittal split osteotomy (BSSO). Due to its novelty, there are no long-term studies which have focused on describing the incidence and type of complications encountered in the post-operative follow-up. The aim of this retrospective study is to analyze patients operated on with this surgical technique and the post-operative complications encountered. Patient and methods The electronic medical records of all patients treated with orthognathic surgery at the Department of Oral, Maxillofacial and Facial Plastic Surgery, University Hospital Frankfurt, Goethe University, Frankfurt, Germany, between the years 2009 and 2016 were retrospectively reviewed. Results A total of 116 patients fulfilled the inclusion criteria. The cases operated on with the standard osteosynthesis (X, Y, and straight) showed a complication rate of 36.37% (n = 4/11). The cases operated on with the HOSO-dedicated plates (HOSO-DP) showed, in total, a complication rate of 6.67% (n = 7/105). The most common post-operative complication resulting from both fixation methods was a reduction in mouth opening and TMJ pain for 4.3%. During the first years of performing the surgery (2009–211), a variety of standard plates had material failure causing non-union or pseudarthrosis. No cases of material failure were observed in the cases operated on with the HOSO-DP. The statistical results showed a highly significant dependence of a reduction in OP-time over the years, when the HOSO was performed without additional procedures (R2 > 0.83, P < 0.0015). Conclusion The rate of complications in the HOSO were shown to be comparable to the rate of complications from the BSSO reported in the literature. Moreover, the use of the ramus dedicated plate appears to provide enough stability to the bone segments, making the surgery safer. Clinical relevance The HOSO needs to be considered by surgeons as an alternative to BSSO. Once the use of the HOSO-DP was established, the rate of complications and the operation time reduced considerably.
