Due to progressive muscle weakness, the arm function in boys with Duchenne muscular dystrophy (DMD) reduces. An arm support can compensate for this loss of function. Existing arm supports are wheelchair bound, which restricts the ability to perform trunk movements. To evaluate the function of a body-bound arm support, a prototype (based on the Wilmington robotic exoskeleton (WREX) arm support) that allows trunk movements was built. In order to examine the effect of this device and to compare it with an existing wheelchair-bound device, three healthy subjects performed single joint movements (SJMs) and activities of daily living (ADL) with and without the devices. The range of motion (RoM) of the arm and the surface electromyography (sEMG) signal of five different arm muscles were measured. The range of motion increased when compared to the wheelchair-bound device, and the trunk motion was perceived as important to make specific movements easier and more natural, especially the more extreme movements like reaching for a far object and reaching to the top of the head. The sEMG signal was comparable to that of the wheelchair-bound device. This means that an arm support with trunk motion capability can increase the range of motion of the user, while the amount of support to the arm is equal.

PURPOSE: Walking, fitness, and balance deficits are common following acquired brain injury (ABI). This study assessed feasibility, acceptability, and usefulness of a modified motor-assisted elliptical (ICARE) in addressing walking, fitness, and balance deficits in children with chronic ABIs. METHODS: Three children (>  5 years post-ABI) completed 24 ICARE exercise sessions (exercise time, speed, and time overriding motor-assistance gradually increased) to promote mass repetition of gait-like movements and challenge cardiorespiratory fitness. Parents’ and children’s perceptions of ICARE’s safety, comfort, workout, and usability were assessed. Cardiovascular response, gait and balance outcomes were assessed. RESULTS: No adverse events occurred. Parent’s Visual Analogue Scale (VAS) scores of perceived device safety (range 80–99), workout (range 99–100), and usability (range 75–100) were high, while comfort were 76–80 given commercial harness fit and arm support. Children’s VAS scores all exceeded 89. Comfortable walking velocity, 2-Minute Walk Test, fitness, and Pediatric Balance Scale scores improved post-training, with many outcomes surpassing established minimal clinically important differences. CONCLUSION: Following engagement in moderate- to vigorous-intensity exercise promoting repetitive step-like movements on a specially adapted motor-assisted elliptical, three children with chronic ABI demonstrated improvements in walking, fitness and balance. Future research in community-based environments with a larger cohort of children with ABI is needed.

A number of dental hygienists have developed pathologies that cause them to be absent from work for long periods of time, and making it difficult for them to return to work. Hygienists' work involves extended static muscular effort in the neck and pectoral girdle, combined with recurrent dynamic movements of the wrist and fingers, associated with efforts to remove tartar from the teeth. Over the last two years, a dozen dental hygienists have consulted us for various shoulder, elbow, wrist, hand and finger problems. Changes in methods, instruments, equipment and the environment have allowed hygienists to adopt safer working positions. Reduced muscular activity was confirmed by EMG testing. The use of telescopic pivoting armrests has facilitated the adoption of new working methods by dental hygienists, and also provided needed arm support. A new concept for a pivoting armrest has been developed with round gel elbows-rests.
