Objective To describe a new class of antihypertensive agents, the angiotensin II receptor antagonists, with emphasis on the prototype losartan. Pharmacokinetic data and clinical trials are reviewed, as well as adverse reactions, drug interactions, and dosing guidelines. Data Sources A MEDLINE search of English-language literature published from 1966 through 1995 was performed. In addition, Merck and Co. provided bibliographic data on file for losartan. Study Selection Emphasis was placed on clinical and pharmacokinetic studies in humans. Controlled, double-blind studies were evaluated to assess the efficacy and adverse effect profile of losartan. Data Synthesis Losartan is a nonpeptide, competitive antagonist of the type 1 angiotensin II receptor. In comparative clinical trials, losartan appears to have antihypertensive efficacy similar to that of the angiotensin-converting enzyme (ACE) inhibitors. Losartan is well tolerated, with an adverse effect profile similar to that of placebo and a reduced incidence of cough versus that with ACE inhibitors. A combination product consisting of losartan 50 mg and hydrochlorothiazide 12.5 mg has also received approval for the treatment of hypertension. The combination product is not indicated for initial therapy, but is recommended for patients who do not respond adequately to losartan monotherapy. The angiotensin II receptor antagonists are also being investigated for beneficial effects in patients with ventricular hypertrophy, renal disease, and heart failure. Conclusions Losartan, the first angiotensin II receptor antagonist to receive approval for use in the US, appears to be an effective new antihypertensive agent with an adverse effect profile similar to that of placebo. Losartan may be an alternative for patients who cannot tolerate ACE inhibitors. However, the effect of losartan on mortality remains to be evaluated. The role of the angiotensin II receptor antagonists in areas such as ventricular hypertrophy, renal function, and heart failure has yet to be determined.

Objective: To review the literature discussing the use of angiotensin-converting enzyme (ACE) inhibitors in the treatment of heart failure. Data Sources: English-language journal articles. Study Selection: Representative articles discussing the effects of ACE inhibitors on hemodynamics, symptoms, and survival. Data Extraction: Studies selected for review in the text were based on study design and clinical endpoints. Data Synthesis: Heart failure results in a series of compensatory responses that, although effective acutely, are ultimately maladaptive. A major mediator in this process is angiotensin II. The production of angiotensin II is dependent on the ACE. Inhibition of this enzyme by ACE inhibitors results in fewer symptoms, improved hemodynamic function, and prolonged survival in patients with heart failure. Conclusions: ACE inhibitors are beneficial in improving the survival of patients with symptomatic heart failure and of patients who have recently had an acute myocardial infarction (MI) and subsequently have a reduced ejection fraction. There appears to be no advantage for immediately initiating ACE-inhibitor therapy within the first few hours of an MI episode. With respect to patients with a reduced ejection fraction without symptoms of heart failure, current data suggest that ACE inhibitors delay the onset of symptoms of heart failure, reduce the need for hospitalization, and may possibly improve survival.

Die Therapie der Herzinsuffizienz umfasst drei Schritte: 1. Elimination der auslösenden Ursache bzw. Behandlung der Grunderkrankung 2. Behandlung der Herzinsuffizienz selbst (dabei soll hier die medikamentöse Therapie besprochen werden) 3. Prophylaxe von Komplikationen. Für die Therapie der Herzinsuffizienz werden Pharmaka aus vier Gruppen meistens in Kombination eingesetzt: 1. Diuretika 2. Vasodilatantien 3. Neurohumorale Inhibitoren 4. Inotrop wirkende Pharmaka Diuretika verbessern die Befindlichkeit und Leistungsfähigkeit, wahrscheinlich auch die Lebenserwartung. Sie sind in der akuten und der chronischen Herzinsuffizienz Medikamente der ersten Wahl. In der Langzeittherapie ist Rückwirkungen auf die Niere und den Kaliumspiegel Rechnung zu tragen. Der Kaliumsparer und Aldosteronantagonist Spironolacton besitzt möglicherweise einen besonderen Stellenwert: Er vermag die myokardiale Bindegewebsproliferation zu hemmen. Vasodilatantien entlasten das Herz akut und chronisch. Gesichert ist diese Wirkung akut für alle gebräuchlichen Substanzen; chronisch für die Kombination von Nitraten mit Hydralazin gemessen an der Verbesserung der Lebenserwartung. Besonders wirksam sind Angiotensin converting Enzym Hemmer (ACE-I), welche Vasodilatation mit neurohumoraler Inhibition verbinden. Hier werden Symptomatik, Leistungsfähigkeit und Lebenserwartung verbessert. Angiotensin II-Rezeptor-Antagonisten sind ähnlich wirksam. Sie können bei Nebenwirkungen der ACE-I austauschbar eingesetzt werden. Beta-Rezeptorenblocker haben einen hohen Stellenwert. Sie sollten, wenn immer verträglich, im Schweregrad II, III und IV (Vorsicht!) in ganz langsam einschleichender Dosierung gegeben werden. Der Wirkungsmechanismus ist nicht ganz geklärt. Wahrscheinlich spielt die Senkung der Herzfrequenz eine entscheidende Rolle. Inotrop wirkende Pharmaka werden kaum verwendet, nur in der akuten Herzinsuffizienz und im kardiogenen Schock. Digitalisglykoside sind wirksam. Sie verbessern die Symptomatik und die Leistungsfähigkeit und reduzieren die Zahl der Dekompensationen im Verlauf. Eine Wirkung auf die Letalität ist nicht gesichert, wohl im Zusammenhang mit der engen therapeutischen Breite dieser Pharmaka. Zur Prävention von Komplikationen wird oft eine antiarrhythmische Therapie notwendig. Diese stützt sich im Wesentlichen auf Amiodaron bzw. die Beta-Blocker. Ferner ist bei Herzinsuffizienz NYHA III und IV, wann immer möglich, eine prophylaktische Antikoagulierung erforderlich. Die Fortschritte im Verständnis der Pathophysiologie und damit der Therapie sind außerordentlich. Neuentwicklungen sind besonders auf dem Gebiet der Zytokine und der Gentherapie zu erkennen.
