ABSTRACT The teeth become very close to each other when they are crowded, but their structures remain individualized and, in this situation, the role of the epithelial rests of Malassez is fundamental to release the EGF. The concept of tensegrity is fundamental to understand the responses of tissues submitted to forces in body movements, including teeth and their stability in this process. The factors of tooth position stability in the arch - or dental tensegrity - should be considered when one plans and perform an orthodontic treatment. The direct causes of the mandibular anterior crowding are decisive to decide about the correct retainer indication: Should they be applied and indicated throughout life? Should they really be permanently used for lifetime? These aspects of the mandibular anterior crowding and their implication at the orthodontic practice will be discussed here to induct reflections and insights for new researches, as well as advances in knowledge and technology on this subject.

Introduction: Crowding in mandibular arch increases with increasing age and etiology of crowding is multifactorial in nature. Role of mandibular third molar in lower anterior crowding remains a topic of controvery over a period of century. The objective of this study was to assess whether there is any correlation between presence of mandibular third molar position and lower anterior crowding. Materials and Method: On the basis of third molar position one hundred and six sample of pretreatment orthodontic patients aged between 17 to 38 years were divided into erupted, erupting and agenesis group. The study was conducted in Peoples Dental College and Hospital from January 2020 to March 2020 after obtaining ethical approval from the institutional review committee. This cross-sectional study was done on dental casts and orthopantomogram; brass wire was used for measuring arch length and digital vernier caliper for measuring tooth material with modified segmental arch analysis method. Result: Among 106 samples collected 55 (51.88%) were female and 51(48.11%) were male and the mean age was 21.53 ± 3.91 years, Chi square and ANOVA test was used for Statistical analysis. This study showed no statistically significant differences between third molar position and lower anterior crowding on right (P= 0.68) and left side (P = 0.45). The study also showed that association between the third molar position and lower anterior crowding is more on left side compared to right side. Conclusion: Mandibular third molars have not statistically significant difference in lower anterior crowding so, extraction of third molars for relieving the anterior crowding is not justifiable

<p class="abstract"><strong>Background:</strong> The purpose of this study was to find the effect of chronic rhinosinusitis on dental malocclusion.  A malocclusion is defined as an irregularity of the teeth or a mal relationship of the dental arches beyond the range of what is accepted as normal. Malocclusion may not be life-threatening, but it is an important public health issue as it compromises the health of oral tissues and also can lead to psychological and social problems. There are numerous studies in the literature about prevalence of the dental malocclusion but there is no study regarding association of CRS and malocclusion. Several factors related to malocclusion, such as anterior crowding, midline malalignment, and facial asymmetry, have strong effects on the perception of facial aesthetics, which influence the psychological development and in turn, influence social acceptance and self‑perception.</p><p class="abstract"><strong>Methods:</strong> This was a prospective cross-sectional observational study was conducted from September 2015 to August 2017 and 57 patients with diagnosis of having chronic sinusitis, were evaluated for dental occlusion. Malocclusion is then classified by angle’s classification.  </p><p class="abstract"><strong>Results:</strong> Most patients of chronic rhinosinusitis had dental malocclusion in grade II, followed by grade I, then     grade III.</p><p class="abstract"><strong>Conclusions:</strong> While treating the patients of CRS one must keep in mind that he may have class II malocclusion which may cause gum disease, poor oral hygiene, impaired speech, mastication and aesthetics of face and need orthodontics management as well.</p>

Background: Patients with congenital absence of a mandibular lateral incisor are often found having difficulty in achieving adequate functional occlusion. It may affect esthetics, mastication, speech, and occlusal balance. Purpose: This paper reported an agenesis treatment of one mandibular lateral incisor case using a space closure method. Case: A twenty-three years old female patient with agenesis of tooth 42, mandibular anterior crowding, multiple diastema on mandibular anterior teeth, and skeletal class III malocclusion. Case Management: Space closure method was chosen to correct the agenesis by considering the class III skeletal malocclusion and multiple diastema condition. Conclusion: Space closure method treatment improved the patient’s facial and dental esthetics, and it provided a good functional occlusion, despite the absence of a mandibular lateral incisor, which generally impairs the adequate incisal guidance.

Inadequate posterior anchorage is a serious limitation for aligner treatment involving extraction of four first premolars. Inappropriate axial inclinations may compromise intermaxillary occlusion and stability. OrthoBoneScrew® (OBS) anchorage is designed to augment the Invisalign® clear aligner G6 solution to produce more predictable outcomes as illustrated by the current case report. An 18-year-old female presented with two chief complains: (1) Protrusive, incompetent lips, and (2) excessive gingival exposure when smiling (“gummy smile”). Clinical evaluation revealed bimaxillary protrusion, hypermentalis activity, anterior crowding, and excessive anterior axial inclinations, particularly of the lower incisors (116°). The American Board of Orthodontic (ABO) discrepancy index (DI) was 21. The treatment plan was extraction of all four first premolars, and clear aligner (Invisalign®) therapy anchored with four OBSs: Infra-zygomatic crest (IZC), and between the roots of the upper central and lateral incisors (Incisal) bilaterally. Eighteen months of initial treatment with 45 aligners retracted and intruded the anterior segments in both arches by closing the extraction spaces with supplemental anchorage provided by IZC and Incisal OBSs. The final series of 20 refinement aligners achieved an excellent outcome as evidenced by an ABO cast-radiograph evaluation (CRE) score of 10, and a pink and white (P&W) dental esthetic score of 3. Post-treatment analysis revealed multiple opportunities for improvement. The patient was well satisfied with the final outcome.
