Working Memory impairments are prevalent among patients with acquired brain injury (ABI). Computerized training targeting working memory has been researched extensively using samples from healthy populations but this field remains isolated from similar research in ABI patients. We report the results of an actively controlled randomized controlled trial in which 17 patients and 18 healthy subjects completed training on the N-back task. The healthy group had superior improvements on both training tasks (SMD=6.1 and 3.3) whereas the ABI group improved much less (SMD=0.5 and 1.1). Neither group demonstrated transfer to untrained tasks. We conclude that computerized training facilitates improvement of specific skills rather than high-level cognition in healthy and ABI subjects alike. The acquisition of these specific skills seems to be impaired by brain injury. The most effective use of computer-based cognitive training may be to make the task resemble the targeted behavior(s) closely in order to exploit the stimulus-specificity of learning.

Multiple tasks have been used to demonstrate the relation between numbers and space. The classic interpretation of these directional spatial-numerical associations (d-SNAs) is that they are the product of a mental number line (MNL), in which numerical magnitude is intrinsically associated with spatial position. The alternative account is that d-SNAs reflect task demands, such as explicit numerical judgments and/or categorical responses. In the novel ‘Where was The Number?’ task, no explicit numerical judgments were made. Participants were simply required to reproduce the location of a numeral within a rectangular space. Using a between-subject design, we found that numbers, but not letters, biased participants’ responses along the horizontal dimension, such that larger numbers were placed more rightward than smaller numbers, even when participants completed a concurrent verbal working memory task. These findings are consistent with the MNL account, such that numbers specifically are inherently left-to-right oriented in Western participants.

AbstractTraumatic brain injury (TBI) is associated with a wide range of physiological, behavioral, emotional, and cognitive sequelae. Litigation status is one of the many factors that has an impact on recovery. The aim of this study was to compare executive functions, postconcussion, and depressive symptoms in TBI patients with and without litigation. A sample of 30 patients with TBI, 15 patients with litigation (medicolegal case [MLC]), and 15 without litigation (non-MLC) was assessed. The tools used were sociodemographic and clinical proforma, executive function tests, Rivermead Post-Concussion Symptom Questionnaire, and Beck Depression Inventory. Assessment revealed that more than 50% of patients showed deficits in category fluency, set shifting, and concept formation. The MLC group showed significant impairment on verbal working memory in comparison to the non-MLC group. The performance of both groups was comparable on tests of semantic fluency, visuospatial working memory, concept formation, set shifting, planning, and response inhibition. The MLC group showed more verbal working memory deficits in the absence of significant postconcussion and depressive symptoms on self-report measures.
