The prevalence of segmental testicular infarction is extremely uncommon and very few cases have been reported in literature. Clinical and sonographic presentation of this condition can mimic testicular neoplasms or testicular torsion. Therefore, accurate diagnosis of segmental testicular infarction is imperative in the treatment process. This case study presents the sonographic diagnosis of testicular infarction in a 49-year-old man who reported mild testicular tenderness. A conservative treatment approach was used, saving the patient unnecessary surgical intervention.

BackgroundPlantar heel pain (PHP) is common. Foot orthoses are often applied as treatment for PHP, even though there is little evidence to support this.ObjectiveTo investigate the effects of different orthoses on pain, function and self-reported recovery in patients with PHP and compare them with other conservative interventions.DesignSystematic review and meta-analysis.Data sourcesA systematic literature search was conducted in MEDLINE, EMBASE, Cochrane Central Register of Controlled Trials, Web of Science, CINAHL and Google Scholar up to January 2017.Eligibility criteria for selecting studiesRandomised controlled trials comparing foot orthoses with a control (defined as no intervention, sham or other type of conservative treatment) reporting on pain, function or self-reported recovery in patients with PHP.ResultsTwenty studies investigating eight different types of foot orthoses were included in the review. Most studies were of high quality. Pooled data from six studies showed no difference between prefabricated orthoses and sham orthoses for pain at short term (mean difference (MD) of 0.26 (95% CI −0.09 to 0.60)). No difference was found between sham orthoses and custom orthoses for pain at short term (MD 0.22 (95% CI −0.05 to 0.50)), nor was there a difference between prefabricated orthoses and custom orthoses for pain at short term (MD 0.03 (95% CI −0.15 to 0.22)). For the majority of other interventions, no significant differences were found.ConclusionsFoot orthoses are not superior for improving pain and function compared with sham or other conservative treatment in patients with PHP.PROSPERO registration numberCRD42015029659.

Calcific metamorphosis teeth are a great challenge to the clinician. Its diagnosis and treatment procedures are of utmost importance in providing the best treatment. Understanding the mechanism behind the discoloration of teeth is the key to get the best treatment. Internal bleaching is one conservative treatment for complete calcific metamorphosis in certain conditions, where signs and symptoms of periapicalpathosis are absent. The present paper involves a case study of 36 years old Saudi woman who faced an injury 20 years ago and presented with yellow discoloration. The patient was thoroughly informed regarding the case; all the plan options were discussed with her prior implementation. Treatment plan comprised of internal bleaching without any endodontic root canal treatment. Vital bleaching is comparatively expensive and was not chosen since the patient had financial issues. The internal bleaching proved a more economic option for her. The patient signed an informed consent form after considering all the details of the procedure.

AbstractNecrotizing pneumonia is a severe form of pneumonia that is mainly treated with conservative treatment, including antibiotics. We report a unique case of necrotizing pneumonia due to group A streptococcus infection in an 18-month-old boy who required extracorporeal membrane oxygenation (ECMO) support. Following surgical lobectomy, the child was weaned off ECMO and recovered uneventfully.

In 1914, Freiberg described a condition of infraction of the second metatarsal head. At the time, he considered direct trauma to be the etiologic agent, but he later disproved this by demonstrating a case without a history of trauma. This article describes a case of Freiberg's infraction in a female collegiate heptathlete and the effects of conservative treatment of her condition with custom-made orthoses.
