Cigarette smoking: Effects on treatment efficacy and drug induced extrapyramidal syndrome in schizophrenic patients

Introduction & aimsEstimate the prevalence of cigarette smoking and some of the related factors among schizophrenic and other hospitalized psychiatric patients.MethodsThis was a cross-sectional study on 120 patients hospitalized in Shahid Beheshti hospital in Kerman in 2005. Patients were equally devided in two groups of schizophrenia and other psychiatric disorders. Sampling was based on statistical census and data were collected using a questionnaire including 27 questions on demographic data, psychiatric disorder, smoking cigarettes and other substances, and Fagerstrom test. Data were analyzed by Chi-square and ANOVA tests using SPSS software.ResultsPrevalence and severity of cigarette smoking was 71.6% and 6.47% among schizophrenic and 51.6% and 6.40% among other psychiatric patients, respectively and the difference was not significant. History of withdrawal was 25.6% and 58.1% in the schizophrenia and other disorders respectively and the difference was significant (P < 0.05). Addiction to other substances was 51.6% in schizophrenic and 45% in the other patients and the most prevalent substances in both groups were opium and alcohol. The severity of smoking cigarettes was 6.9 along with other drug abuses and 5.1 in cases with just smoking based on Fagerstrom test and the difference was significant (P < 0.05).ConclusionsThe prevalence of cigarette smoking in both schizophrenia and other psychiatric patients is higher than normal population, but there is no significant difference between these two groups. Schizophrenic patients need persistent supportive and supervising programs for cigarette smoking abuse treatment because of their cognitive, motivate and social problems.

The purpose of this work was to evaluate the degree of activation of platelets chronically schizophrenic patients with paroxysmal course of the disease during outcome from the attack of disease or in process of forming of the remission. Platelets in platelet-rich-plasma (PRP) and after PRP gel-filtration through the column with Sepharose CL-2B taken from the ulnar vein of males with chronic schizophrenia during outcome from the attack of disease or in process of forming of the remission were measured. Patients were investigated on the peak of the disease attack (visit 1), at the stage of significant improvement of clinical state (visit 2), and at the stage of stable drug-induced remission (visit 3). Platelet quantities in PRP prepared by centrifugation are equal in patients in visits 1, 2 or 3. These platelets quantities are much less than similar platelet quantities in healthy individuals matched in age. The additional decreases of platelet quantities were found after the elution of PRP platelets from the column with Sepharose CL-2B. These additional platelet quantity losses are increased from visit 1 to visit 3. Such losses of cells indicate decrease in their resistance to various influences, their activation in spite of remission development in this group of patients.

Abstract This paper presents the results of a study of patients with nonalcoholic steatohepatitis, alcoholic steatohepatitis, and drug-induced liver injury with concomitant colitis and the development of biliary insufficiency that manifests as a significant decrease in the volume of bile acids and bile. In patients with liver injury and concomitant colitis, changes in intestinal microflora were marked. After a combination therapy of this pathology with phosphatidylcholine and glycyrrhizic acid, an improvement of biochemical, clinical indicators was observed, as well as a decrease in fatty degeneration and reduction of elastographic indicators of liver fibrosis. The introduction of the drug formulations phosphatidylcholine and glycyrrhizic acid in the complex of conservative treatment of liver injury with concomitant colitis helps to prevent hepatic and intestinal complications and to increase the treatment efficacy of these diseases.

Three chronic paranoid schizophrenic patients were treated with clozapine, a new anti-psychotic agent. Retrograde ejaculation developed as a side-effect. Stopping the drug therapy or reducing the dosage caused the side-effect to disappear. Re-challenge caused the return of the side-effect. We discuss this rare side-effect to clozapine, and the pathophysiological mechanisms of ejaculation disturbances, with emphasis on those that are drug-induced.
