Abstract The COVID-19 pandemic will be remembered as one of the worst catastrophic events in human history. Unfortunately, no universally recognized effective therapeutic agents are currently available for the treatment of severe SARS-CoV-2 infection. In this context, the use of convalescent plasma from recovered COVID-19 patients has gained increasing interest thanks to the initially positive clinical reports. A number of mechanisms of action have been proposed for convalescent plasma, including direct neutralization and suppression of viremia, anti-inflammatory and immunomodulation effects and mitigation of the COVID-19-associated hypercoagulable state. These immune and non-immune mechanisms will be critically discussed in this narrative review.

This study examines the process by which a change in institutional logic of a organizational field is initiated. More specifically, we are concerned with how the IBM Thinkpad University concept affects various business schools in the province of Quebec. The theoretical framework departs from new-institutional theories. We propose a multi-level analysis: institutional, organizational and operational. The finding examines the process by which organizations retain, adopt and discard practices and illustrates the influence of institutional prestige, organizational mission, autonomy and pedagogical expertise. We show that the source of the institutional logic of appropriation is a quest for reputation.

BackgroundFascial plane blocks (FPBs) target the space between two fasciae, rather than discrete peripheral nerves. Despite their popularity, their mechanisms of action remain controversial, particularly for erector spinae plane and quadratus lumborum blocks.ObjectivesThis narrative review describes the scientific evidence underpinning proposed mechanisms of action, highlights existing knowledge gaps, and discusses implications for clinical practice and research.FindingsThere are currently two plausible mechanisms of analgesia. The first is a local effect on nociceptors and neurons within the plane itself or within adjacent muscle and tissue compartments. Dispersion of local anesthetic occurs through bulk flow and diffusion, and the resulting conduction block is dictated by the mass of local anesthetic reaching these targets. The extent of spread, analgesia, and cutaneous sensory loss is variable and imperfectly correlated. Explanations include anatomical variation, factors governing fluid dispersion, and local anesthetic pharmacodynamics. The second is vascular absorption of local anesthetic and a systemic analgesic effect at distant sites. Direct evidence is presently lacking but preliminary data indicate that FPBs can produce transient elevations in plasma concentrations similar to intravenous lidocaine infusion. The relative contributions of these local and systemic effects remain uncertain.ConclusionOur current understanding of FPB mechanisms supports their demonstrated analgesic efficacy, but also highlights the unpredictability and variability that result from myriad factors at play. Potential strategies to improve efficacy include accurate deposition close to targets of interest, injections of sufficient volume to encourage physical spread by bulk flow, and manipulation of concentration to promote diffusion.

Purpose This paper aims to investigate the pattern of technological capability building in the gas turbine industry as a complex product system (CoPS) in an Iranian gas turbine producer named Oil Turbo Compressor Company (OTC) and to recognize multi-level (firm, industry and national) drivers influencing technological catching up in this company. Design/methodology/approach This paper used a qualitative approach and case study research strategy. A preliminary theoretical framework is proposed based on research background. Also, the data were collected from various sources, including the interview with 11 experts, studying many documents and participating in some relevant meetings and conventions. To analyze the data, the authors relied on their preliminary theoretical framework and applied the chronological sequence analysis technique. Findings Our findings show that, first, in contrast with mass-produced industries where capability building pattern often leads to product innovation, technological capabilities in OTC have evolved from assembling to manufacturing, upgrading and finally redesigning of existing models of gas turbines. Second, two firm-level (proper technology acquisition strategies and building organizational and managerial capabilities), two industry-level (networking, integration and collaboration among key actors and existence of local market and demand) and two national-level (government’s policies, supports and initiatives and institutional arrangement and political conditions) drivers have played indispensable roles in facilitating and accelerating technological catching up by OTC. Research limitations/implications Inevitably, the current research faces a few limitations. For instance, the difficulty of generalization is considered an inherent problem because it is a case study of only one Iranian latecomer company, as well as only one CoPS industry. Regarding implications, the findings suggest that technological catching up in CoPS industries in developing countries is not a simple and autonomous process and is influenced by multi-level factors, including national-, industry- and firm-level drivers. Originality/value In terms of theory, this paper tends to investigate and explain the catching-up process in OTC as an Iranian gas turbine producer by applying a multi-level theoretical framework that consists of firm-, industry- and national-level drivers. In terms of practice, this paper aims at investigating drivers affecting the catching-up process in a CoPS industry in a developing country that was faced with vast international sanctions, while many other studies in this area examined cases from developing countries such as Korea and China that had the opportunity of enjoying international collaborations and overseas knowledge flows.

Despite increased research interest on e-government, the field currently lacks sound theoretical frameworks that can be useful in addressing two key issues concerning the implementation of e-government systems: (1) a better understanding of the factors influencing the adoption of e-government systems, and (2) the integration of various e-government applications. The objective of this paper is to provide a foundation towards the development of a theoretical framework for the implementation of e-government systems via extensive literature review, which resulted in (1) a synthesis of existing empirical findings and theoretical perspectives related to e-government adoption, and (2) development of the premises of a conceptual model that would reflect the multi-level and multi-dimensional nature of e-government systems’ acceptance.
